[Inflammatory foot disease].

The pathology of the foot is frequent in the inflammatory rheumatisms. It can finally contribute to the diagnosis. In rheumatoid arthritis, the metatarso-phalangeal joint involvement appears earlier and more obvious. The lesion of the midrearfoot is later and the beginning is more indisious, they often evolve in pes plano valgus deformity. In spondylarthropathies the involvement of the heel and of the toes predominates, but there are more specific symptoms for each disease particularly for the psoriasic rheumatism. Local cares, plantar orthoses, casts, corticosteroid injections, synoviortheses

Injuries in Australian Army recruits. Part III: The accuracy of a pretraining orthopedic screen in predicting ultimate injury outcome.

Three hundred fifty male recruits were divided into two groups: Walk (N = 170) and Run (N = 180). A physical screen was performed before the commencement of recruit training consisting of a visual assessment of the feet looking for pes planus, pes cavus, and deformities of the toes. Each recruit was also asked if he had sustained any injury in the preceding 2 years. The visual findings and history of prior injury were noted and compared with actual injuries sustained during the 12-week training course. Fifty-three subjects in the Walk group and 54 in the Run group were identified as being at r

Prevalence of radiographic foot abnormalities in patients with diabetes.

Clinicians are increasingly aware that mechanical aspects of foot deformities, such as Charcot changes, clawtoes, bunion deformities, or cavus or planus foot deformities, might have an impact on the occurrence, potential healing, and recurrence of foot ulcers. We report the prevalence of plain radiographic changes and attempt to rate the severity of those deformities in the feet of 456 diabetic veteran medicine clinic enrollees. All 456 radiographs were reviewed by orthopaedic surgeons to specifically identify Charcot changes, presence of arterial calcification, dislocation of the lesser toe m

Diagnostic findings in the 1992 epornitic of neurotropic velogenic Newcastle disease in double-crested cormorants from the upper midwestern United States.

Neurotropic velogenic Newcastle disease (NVND) occurred in juvenile double-crested cormorants, Phalacrocorax auritus, simultaneously in nesting colonies in Minnesota, North Dakota, South Dakota, and Nebraska and in Lakes Michigan, Superior, Huron, and Ontario during the summer of 1992. Mortality as high as 80%-90% was estimated in some of the nesting colonies. Clinical signs observed in 4- to 6-wk-old cormorants included torticollis, tremors, ataxia, curled toes, and paresis or weakness of legs, wings or both, which was sometimes unilateral. No significant mortality or unusual clinical signs w

Kinematics and contact characteristics of the first metatarsophalangeal joint.

We studied six fresh frozen cadaver feet to define the three-dimensional motion of the hallux proximal phalanx in relation to the first metatarsal and to describe the contact features of the first metatarsophalangeal joint. Six tendons to the hallux were loaded to simulate dynamic loading of the hallux. A magnetic tracking system was used to monitor the three-dimensional movement of the proximal phalanx while the toe position was changed from a neutral position to full extension by adjusting the tendon loads. The average surface area was 0.38 +/- 0.08 cm2 in the neutral position; it decreased

Hallux rigidus: the long-term results of dorsal wedge osteotomy and arthrodesis in adults.

A variety of treatments are recommended for hallux rigidus. These include arthrodesis of the first metatarsophalangeal joint and dorsal wedge osteotomy of the proximal phalanx. This latter operation is not commonly performed in adults. A retrospective review of a series of 10 osteotomies and 20 arthrodeses after an average of 12 years for hallux rigidus in the adult was undertaken. Patients were assessed at a follow-up clinic and their foot pressures were analyzed. Although the two groups were not matched, some conclusions could be drawn. Both operations were successful in terms of pain relief

Hallux limitus in the athlete.

Hallux limitus is one of the more common conditions occurring around the great toe, characterized by a progressive decrease in the dorsiflexion range of motion at the first metatarsophalangeal joint. Bony proliferation and articular degeneration of the joint result in pain and possible disability. Any force or condition which results in the restriction of dorsiflexion at the 1st metatarsophalangeal joint may, over time, produce typical changes around the metatarsophalangeal joint including the formation of dorsal exostosis, marginal osteophytes, erosion of articular cartilage and possible anky

The Mayday distal first metatarsal osteotomy for hallux valgus: a review after the introduction of a new instrument.

We present the short-term follow-up of 55 symptomatic hallux valgus deformities in 38 patients, treated operatively with a modification of the spike distal first metatarsal osteotomy, as described by Gibson and Piggott in 1962. The age range of the patients was 17 to 72 years at the time of surgery. The postoperative follow-up period was 12 to 55 months. Excellent and good clinical and radiographic results were recorded in 96.2% of our patients. Two of the patients (3.8%) were dissatisfied; one of them complained of metatarsalgia after the procedure, and the other had stiffness of the metatars

Two-stage treatment of flexor tendon ruptures. Silicon rod complications analyzed in 109 digits.

Complications during two-stage flexor tendon reconstruction may jeopardize the function of the repaired tendons. We reviewed complications encountered in 89 patients (109 digits) treated with a two-stage flexor tendon reconstruction using either silicone or Hunter rods. The complications could be distinguished according to the stage of reconstruction in which they occurred. Complications during stage I included rod buckling, rupture of the distal end of the rod, rod migration, synovitis and infection, and during stage II included distal rupture of the graft juncture, loose graft, tight graft,

Hypertrophic osteoarthropathy and intrathoracic Hodgkin disease of childhood.

BACKGROUND: Hypertrophic osteoarthropathy (HOA), well known in adults, is rarely encountered in children. The clinical features include clubbing of the fingers and toes, arthritis, and painful periosteal new bone formation of the tubular bones. The association of malignant disorders with HOA is more common in adults than in children. CASE: In this paper, a 12-year-old boy with intrathoracic Hodgkin disease and HOA is presented and four other children with HOA and Hodgkin disease in the literature reviewed. DISCUSSION: The presence of HOA has been thought to be a bad prognostic sign, but comp

Pathological anatomy and dynamic effect of the displaced plantar plate and the importance of the integrity of the plantar plate-deep transverse metatarsal ligament tie-bar.

Normal and deformed forefeet have been investigated by cadaver anatomical dissections and experiments, by radiographs, CT and MRI scanning, and by clinical studies. Evidence is presented to show that the skeleton of the foot rests on and is controlled by a multi-segmental ligamentous and fascial tie-bar system. Transversely across the plantar aspect of the forefoot, the plantar plates and the deep transverse metatarsal ligaments form a strong ligamentous structure which prevents undue splaying of the forefoot. Longitudinally, the five digital processes of the deeper layer of the plantar fascia

The great imitator: Rocky Mountain spotted fever occurring after hospitalization for unrelated illnesses.

We describe two patients who had Rocky Mountain spotted fever after they were admitted to the hospital for emergency and elective surgical procedures. We initially thought one patient had a hospital-acquired infection; the correct diagnosis was deduced from epidemiologic clues elicited by consultants. These two cases were also unusual in that one patient had a recurrent rash after an abbreviated course of low-dose doxycycline therapy and the other patient had transient and self-limiting postinfectious polyneuropathy. These cases illustrate that community-acquired infection with Rickettsia rick

Condyloma latum of the toe webs: an unusual manifestation of secondary syphilis. A report of two cases.

The incidence of primary and secondary syphilis in the United States peaked in 1990, exceeding the number of reported cases since 1948. Uncommon manifestations of secondary syphilis seen more frequently in the preantibiotic era may be less well recognized today. One such manifestation is the condyloma latum, or split papule of the toe web. Two patients with condyloma of the toe web are presented and the previous literature is reviewed. Condyloma latum is a common manifestation of secondary syphilis; however, condyloma latum of the toe webs is unusual. Failure to recognize toe web condyloma lat

Variation in the incidence and proportion of diabetes-related amputations in minorities.

OBJECTIVE: To identify the age-adjusted and level-specific incidence of amputations associated with diabetes in Hispanics, African-Americans, and non-Hispanic whites. RESEARCH DESIGN AND METHODS: We used a database from the Office of Statewide Planning and Development in California that identified all hospitalizations for lower-extremity amputations in the state in 1991. Amputation level was defined by ICD-9-CM codes 84.11-84.18 and were categorized as toe, foot, leg, and thigh amputations. RESULTS: The age-adjusted incidence of diabetes-related amputation per 10,000 persons with diabetes in

Thumb reconstruction.

Experience in 161 thumb reconstructions are reported. The results of pollicizations are evaluated according to function, cosmesis, and two-point discrimination. Of 49 pollicization, 32 were considered excellent, 10 very good, 5 good, and 2 fairly good. In the 16 wrap-around procedures, all showed a very good (less than 6 mm) or good (6 to 12 mm) two point discrimination. Similar results were seen after toe-to-hand transfers when careful nerve repair had been performed.

Prospective assessment of autonomic and peripheral nerve function in adolescents with diabetes.

Autonomic and peripheral nerve function were studied prospectively in 102 adolescents with Type 1 diabetes over a 5-year period. All adolescents were assessed three times; 54 were assessed four times. The median age at baseline was 14.5 (range 10.4-18.0) yr. The median diabetes duration at baseline was 6.8 (range 1.3-15.2) yr. Autonomic nerve function was assessed by measuring heart rate variation during deep breathing, valsalva manoeuvre, standing from a lying position (30/15 ratio), and the postural change in systolic blood pressure. Peripheral nerve function was assessed by determining the

[Increasing frequency of onychomycoses--is there a change in the spectrum of infectious agents?].

The recently developed antifungal drugs are not equally effective against the fungi causing onychomycoses: dermatophytes, yeasts and non-dermatophyte filamentous fungi (NDFF). Therapeutic failures may be due to the use of antifungal agents not primarily effective against the particular fungus. Considering the high costs of systemic antifungal therapy, it is necessary to know the frequency distribution of the different fungi causing onychomycosis. In a retrospective study, we have analysed results of fungal cultures performed between 1974 and 1994. In particular, we have compared the time perio

Microsurgical reconstruction of a stiff jointless thumb after an avulsion injury.

We demonstrate three types of reconstruction of a stiff jointless thumb after an avulsion injury. In the first type, the skeleton of the thumb was harvested from the iliac crest or from a rib, the cutaneous cover was formed from a forearm flap on the a. radialis. In the second type, the preserved skeleton of the thumb from crushed tissues was transferred to the forearm and from there subsequently used for reconstruction together with the forearm flap. These two types of reconstruction were carried out if the amputation line reached the metacarpophalangeal (MP) joint of the thumb. In an amputat

The teratogenic action of retinoic acid in rat congenic and recombinant inbred strains.

The teratogenic effect of all-trans retinoic acid (RA) was tested in the system of congenic and recombinant inbred (RI) strains of the laboratory rat carrying the mutant Lx allele which determines the polydactyly-luxate syndrome. It was demonstrated that the teratogenic effect of RA is influenced by both the genotype at the Lx locus and the modifying genes. The dose of 100 mg/kg administered by gavage on day 11 of pregnancy induced a statistically significant incidence of preaxial polydactyly in hind limbs of LEW/BN, +/Lx foetuses heterozygous in the mutant allele, whereas no specific limb def
