A retrospective review of all cases of sideswipe elbow fractures (SSEFs) treated at two community hospitals from 1982 to 1992 was conducted to determine the functional outcome of the operative treatment of SSEFs. All five injuries involved the left elbow, and they included open fractures of the olecranon, the radius and ulna, the ulna and humerus, the humerus, and traumatic amputation of the arm. Concomitant injuries included three radial nerve palsies and two injuries each to the median nerve, ulnar nerve, and brachial artery. Bone growth and the development of epiphyseal plates are disturbed after the replantation of amputated extremities in children, but the potential for continued skeletal growth is almost always maintained in the replanted part. We describe the indications and the results in 18 bone lengthenings carried out on 14 patients in our department during the period January 1991 to September 1993. Seven patients had limb length inequality, five patients had dysplasia or aplasia, one patient had Turner's syndrome and one patient had suffered traumatic amputation of all the fingers of one hand. Trauma to the upper extremity can be a cause of significant morbidity and disability to otherwise productive people. Traumatic amputation can be particularly devastating, but many of these extremities can be saved by replanatation of the amputated parts. We reviewed the upper extremity replantations performed at the Medical Center of Central Georgia in the 6-year period from September 1987 to August 1993. Injury to the deep peroneal nerve in the foot and ankle may result from trauma, repetitive mechanical irritation, or iatrogenic harm. The nerve is most susceptible to injury along its more distal anatomic course. A 66-year-old patient sustained a severe right hand injury resulting in a thumb pulp defect and unreplantable amputation of the index, long, and ring fingers at the metacarpophalangeal joint with joint exposure. There was also an 8 cm defect of the radial digital nerve of the little finger. Immediate one-stage total reconstruction of all of the defects was accomplished by a free groin flap and a free glabrous skin flap from the foot. Opposable function of the hand was thus preserved.

Over a 12-year period between 1979 and 1991, 27 patients were operated on at the New York University Medical Center for salvage of below-knee amputation stumps utilizing free flaps. We report our results using University of Wisconsin (UW) cold storage solution in two cases of major replantation of the upper extremity. We employed a technique of a complete vascular washout with 1000 ml UW solution at 4 degrees Celsius at 120 cm hydrostatic pressure into the artery via a size 18 cannula. Fifteen thumb reconstructions were performed using the wrap-around procedure. A free wrap-around flap was taken from the big toe, and included the nail, and a part of the distal phalangeal skeleton. A technique is described for reconstruction of a pincer, by a second toe transfer, in traumatic and congenital deformities, leaving only the wrist. An anterior transfer on the radius allows the wrist mobility to compensate for the limited range of mobility of the second toe. The authors present an overview of varus or curly toe deformity. This congenital deformity can be seen in many generations of the same family, with no greater incidence in males or females. The hereditary influences on the inheritance of this deformity have not been clearly defined but appear to follow an x-linked or autosomal dominant pattern. The deformity is very often dismissed by physicians, and if left untreated can lead to more serious digital pathology later in life.

We examined the relationship between prevalence and severity of tinea pedis and the length of service and the width of the spaces between the toes in 74 members of the Japanese Self-Defence Forces (SDF) undergoing special training. Patients with prostatic carcinoma were randomised to treatment with surgical castration (n = 47) or oral oestrogen therapy (n = 53). Seventy-one patients with thumb amputations, 45 complete and 26 incomplete nonviable, have been treated at the Microsurgical Unit of the Department of Orthopaedic Surgery at the University of Ioannina Medical School over the past 15 years. Fourteen patients with ring avulsion injuries were treated at the University of Ioannina from 1982 to 1991.
