Index transposition after resection of the long finger ray.

Ten patients underwent index-to-long-finger ray transposition for repair after resection of the long finger ray. Care was taken to preserve the second web space and prevent painful neuromas. Rigid internal fixation was achieved with a T plate, and mobilization was started immediately. There were no nonunions. Postoperative motion was equal to preoperative motion in acute cases and equal or better in chronic cases. Mean grip strength was 0.65, and pinch strength was 0.78 compared with the uninjured hand. There were no painful neuromas, hypersensitive scars, or deterioration of monofilament or t

Pulse oximeter waveforms from the finger and toe during lumbar epidural anesthesia.

BACKGROUND AND OBJECTIVES: To determine whether lumbar epidural anesthesia affects pulse oximeter signals in the upper or lower extremity, 13 ASA I patients were studied. METHODS: Temperature and pulse oximeter probes were placed on the finger and the toe. RESULTS: After epidural injection, the amplitude of the pulse oximeter waveform on the toe increased eight-fold but declined by 50% in the finger. The increase in amplitude of the pulse oximeter waveform in the foot preceded the temperature rise. CONCLUSIONS: More reliable pulse oximeter signals may be obtained from the toe than the finge

Digit amputation in cattle: 85 cases (1971-1990).

Medical records of 83 cattle, which had 1 or more digit amputations performed at Kansas State University and The Ohio State University veterinary hospitals between 1971 and 1990, were reviewed. Signalment, duration of lameness, prior treatment, digit involved, and pathologic condition were evaluated. Owners were contacted with regard to the animal's duration in the herd after amputation and reason for exiting the herd, level of production attained, and degree of lameness. The animal was judged to have a good, fair, or poor recovery on the basis of this information. Septic arthritis of the dist

Morphology and somatotopy of the central arborizations of rapidly adapting glabrous skin afferents in the rat lumbar spinal cord.

The central arborizations in the dorsal horn of the spinal cord of 23 rapidly adapting (RA) A-beta primary afferent neurons innervating different regions of the glabrous skin of the hindpaw were studied by the intra-axonal injection of horseradish peroxidase in adult rats. A total of 284 arbors of the complex, simple, and blind-ending variety were recovered. The arbors of RA afferents innervating the toes, paw pads, and non-pad hindpaw differed from each other in branch pattern and dimensions. The simple and complex arbors, which are both bouton-containing, were distributed mainly in laminae I

Fibro-osseous pseudotumor of the digits: report of a case with immunohistochemical and ultrastructural studies.

A polypoid fibro-osseous nodule arising from the soft tissues of a toe of a 17 yr old man is described. Initial partial excision of the nodule led to a wrong diagnosis of pyogenic granuloma. The mistaken diagnosis was based on finding an extensively ulcerated polypoid nodule with a core of fibrovascular tissue which resembled granulation tissue. The fast growing lesion recurred twice within 4 mths following incomplete excisions. Bony destruction of the distal phalanx was noted radiologically at the second recurrence. The rare lesion is an example of fibro-osseous pseudotumor of the digits, som

Treatment of post-replantation retardation of bone growth by callus distraction. A report of two cases.

Hand and forearm replantation in children may lead to premature closure of the distal radial or ulnar growth-plates. Following successful replantation, bone growth may be retarded, resulting in a length discrepancy between the radius and ulna with secondary deviation of the replanted hand. We present two cases in which callus distraction was employed to correct a length discrepancy. The relative merits of the various treatment options are discussed.

Hallux limitus/rigidus deformity: a new great toe implant.

The authors present a new alternative to the problem of hallux limitus/rigidus deformity. The new Bioaction great toe implant (Orthopaedic Biosystems, LTD, Scottsdale, Arizona) appears to be superior to the popular hinged implant, since it is comprised of two separate components and therefore appears to be more resilient to implant breakage/failure. This manuscript will examine the etiologic factors contributing to this deformity, a brief retrospective review of traditional surgical treatment and a case presentation utilizing the Bioaction great toe implant.

Two cases of 5q deletions in patients with familial adenomatous polyposis: possible link with Caroli's disease.

Two cases are reported of patients with deletions of chromosome 5q. Both have familial adenomatous polyposis (FAP) and mild mental retardation. In both, macroscopic polyposis was confined to the proximal colon in adult life (in their thirties) although microscopic adenomatosis was shown in the more distal colon with occasional single polyps. Both subjects had dermoid cysts, and congenital hypertrophy of the retinal pigment epithelium (CHRPE) was seen in case 2. Case 1 has gastroduodenal polyps and desmoid tumours; case 2 has a marfanoid habitus with an abnormal pectus, wasted calf muscles and

Salvage replantation of two fingers from a nonreplantable midpalm.

To achieve better functional results following mutilating multidigital trauma, every effort should be made to maximize the ultimate function of the hand, and the plan of replantation should be guided accordingly. All usable parts should be salvaged from the amputated nonreplantable areas. A patient is reported in whom two fingers taken from the nonreplantable midpalm were used to reconstruct the thumb and ring fingers, ultimately improving the overall quality of the patient's life.

Comparative anatomical studies of arteriographs of the hand and foot in primates including man.

In this study we have tried not only to report the variations in the arteries of the hand in Hominoidea (Pongidae and Hominidae) and the frequency with which these deviations occur, but also to deal with them phylogenetically and ontogenically. The arterial system of the human hand and foot are considered to have changed from the ground plan of Old World monkeys. In this paper, the entire arterial system of the primates including man (Cercopithecidae, Pongidae and Hominidae) is discussed in a stereoscopic analysis of angiographs. In the hand, a three-dimensional angiographic analysis of the de

Toe-to-thumb transplantation in the rat.

A toe harvested from the hind foot of a rat was successfully transplanted to the rat thumb position, with vessel and nerve anastomoses. With this technique practiced, there will be more ease, confidence, and success when doing toe-to-hand or small tissue transplantations or replantations in humans, even in infants. The procedure is described and discussed.

[Forum: reconstruction of the traumatised thumb. Reconstruction of the amputated thumb by progressive lengthening of the 1st metacarpal using mini-fixator. Apropos of 2 clinical cases].

The authors report two cases of reconstruction of a mutilated thumb by progressive elongation of the first metacarpal without the need for bone grafts, using a minifixation elongation device, which is very easy to use. It allows distraction which can be performed by the patient himself and adjustment to the desired length, up to 40 mm.

[Forum: reconstruction of the traumatic thumb. Esthetics and post-traumatic reconstruction of the thumb].

The authors deal with the aesthetic problems of traumatic thumb reconstructions from two complementary points of view: surgical and psychoanalytical. From the surgical point of view, they discuss the aesthetic factors of repair ("aesthetics equals function") in the hand and in the donor site. The features of a "good" reconstructed thumb are recalled, together with the ways of achieving this result, taking into account several imperatives related to the various techniques: the residual scar, the proportions of the thumb (length and diameter) and the appearance of the nail. The morphological seq

Toe-to-hand transfers in the rehabilitation of frostbite injury.

In our experience digitless hands from frostbite injury are typically seen in young men as a result of an episode of intoxication. Such deformities occur bilaterally in 80% of cases and often lead to total loss of prehensile capability. Because of a uniformly high degree of motivation in this group of patients, toe-to-hand transfer carries an excellent prognosis. Microanastomoses in a frostbitten hand are as reliable as in a mechanically injured hand, provided the ulnar artery and its branches are intact. Because amputations occur at the level of the metacarpal heads, the preferred transplant

An outbreak of vampire bat bite in a Brazilian village.

An outbreak of 26 cases of vampire bat bite in one month in the rural area of Honorópolis, a Brazilian village, is reported. All patients were bitten during the night, when they were asleep, and most bites were on their toes. No complication attributed to the bite was reported. The patients were given prophylactic antirabies serum and rabies vaccine. Control of the outbreak was achieved by visiting bat roosting sites, smearing captured vampire bats (Desmodus rotundus) with a paste of 2% warfarin in vaseline, and releasing them. No case of human or animal rabies was diagnosed in a 5-month follo

Motor vehicle-pedestrian accidents in adults. Relationship between impact speed, injuries, and distance thrown.

Eighty-five fatal motor vehicle-pedestrian accidents involving adults were reviewed in order to establish the relationship of impact speed to amputation of limbs, atlanto-occipital dislocation of the spine, and distance a body is thrown by a vehicle from point of impact to point of rest. In five cases, there was amputation of a limb, in two, transection of the torso. All occurred at highway speeds (> 55 mph). Twenty-six (31%) of victims had atlanto-occipital dislocations. Half of these accidents occurred at street speeds, half at highway speeds. In cases where the point of impact and distance

Cataracts and crooked toes in Brahma chickens.

In 1991, 69% of the cockerels and 15% of the pullets in an inbred flock of approximately 200 dark and light Brahma chickens had unilateral or bilateral cataracts and crooked toes. Affected chickens were normal at hatching but developed cataracts with or without crooked toes by 6 months of age. The cataracts were initially focal and polar but progressed to be diffuse throughout the lenticular cortex. The crooked toes involved one or more of the second, third, and fourth digits and were due to a medial deviation of the distal aspect of the first phalanx. The cataracts and crooked toes were consi

Omentum as gliding material after extensive forearm tenolysis.

Tendon adhesion occurring after major replantation can be severe and extensive due to the nature of the trauma, ischaemia, prolonged oedema and/or infection. Therefore there is a high possibility of re-adhesion after tenolysis. In two cases of tenolysis after forearm replantation omentum was used as gliding material and good results were obtained.

[Amputation injuries of the upper extremity--early complications after replantation and revascularization].

Within a time of ten years 144 patients with difficult injuries on their hands were operated. These were 95 replantations and 102 revascularizations. 26 patients had to be operated again because of arterial and venous thrombosis. 46% of our patients had early vascular complications already within 24 hours. The frequency of thrombosis was increasing with the accompanying soft tissue injury and therefore with the gravity of the trauma. Avulsion injuries had of course the worst prognosis. After diagnosis of vascular occlusion within the replanted finger there should be a quick surgical revision o

Bilateral syndactyly. A unique case with surgical correction.

Cases must be treated on an individual basis, appreciating the complexity of the syndactyly, considering the patient compliance, and understanding the goals of the surgical correction. Extensive discussion with the patient and parents must occur to clarify the goals of the surgery, the expectations, and possible complications. Careful preoperative planning and incision design is paramount in obtaining satisfactory results. Skin grafting may be required, either full-thickness from a variety of donor sites or split-thickness grafting as in one case study reported. Adjunctively, manipulation and
