Heterozygotes for cleidocranial dysplasia (Ccd) and short digits (Dsh) were crossed to test whether synergistic interactions occur between different dominant mutations whose individual pleiotropic phenotypic effects exhibit a common feature. Because anterior knee pain syndrome is common in young paratroopers, we studied the role of the extensor mechanism in deceleration during the parachute landing fall (PLF) and the extent of knee flexion resulting from use of proper and variant landing styles. Solitary periungual angiofibromas can often be challenging in the diagnosis and treatment of such common lesions. Because of the various histologic presentations of fibrous skin tumor, a complete differential diagnosis would include hemangioma, seborrheic keratosis, fibroma, and digital fibrokeratoma. Treatment by total sharp excision can often be accomplished, depending on the morphological appearance of the lesion. An incisional biopsy of a more expansile lesion may be warranted.

A 42-year-old woman presented with avulsion amputation of her right leg 14 cm below the knee joint and a hemihand amputation of the opposite hand. Two free flaps, taken from the amputated foot, were used to cover the injured extremities. Forty-four patients who had had a total of forty-seven amputations of an upper extremity and who had had a myoelectric prosthesis for more than two years were evaluated retrospectively for the amount of use of the prosthesis, the use of any other prosthesis, and the demographic factors that might be related to use of the prosthesis. Advances during the last 15 years in microsurgical technique and in our understanding of vascular anatomy have led to the development of two new branches of plastic and reconstructive surgery. The first is the transfer of free composite flaps with microvascular anastomoses. The second is transposition of island flaps solely on a proximal or distal vascular bundle. Both types of operations require an adequate vascular pedicle to provide sufficient blood circulation for flap viability and survival in the recipient site.

In the early 1980s Japanese surgeons introduced very long free flaps based on two vascular pedicles from the thoracodorsal and groin territories. Such flaps, vascularized through two pedicles, may be called megaflaps. One or both bundles may be anastomosed as necessary to vascularize the flap. Anatomical studies and clinical experience point to new donor sites for such megaflaps.

Stump problems in amputations resulting from employment related injuries were investigated in 397 cases in the Chugoku and Shikoku districts of Japan between 1987 and 1991. Ninety-seven patients (24%) had stump problems which interfered the prosthetic fitting.
