Effect of positioning of the feet in posturography.

Static posturography has been developed from a basic research test to a widely used clinical tool for evaluating dizzy patients. Before any actual standardization can be achieved, however, several aspects of the test situations have to be evaluated, including the position of the feet. The quantitative significance of the standing position in posturographic measurements was evaluated with healthy volunteers studied under visual and nonvisual conditions, using 4 foot positions: heels together with the toes 30 degrees apart or at an angle of the volunteer's own choice, and the feet parallel and e

Bilateral severance of thumbs: report of two cases.

Management of hand injuries is a common practice but bilateral amputation of thumbs is very rare. We report on two cases of both thumbs amputated by a similar machine. In case 1, the man's left thumb was closed with a composite skin graft because the severance was through the middle of the nail. A wrap-around procedure was performed to reconstruct the right thumb which was infected after a primary V-Y advancement flap. The final results were acceptable. Replantation of detached thumbs, if possible, achieves the best functional recovery, which was well demonstrated in Case 2. Although the inter

Autoradiography of lymph nodes with 99mTc-dextran in rabbits.

99mTc-dextran (D) was further evaluated in the present study as a lymphoscintigraphic agent compared to radiocolloids. It was injected intra-dermally into the web space between the second and third toes in both hind feet of two rabbits. 99mTc-nanocolloid (NC) was injected subcutaneously in both hind feet of two other rabbits. Popliteal lymph nodes were taken out and frozen in liquid nitrogen after the animals were sacrificed at 2 h post-injection. Three frozen sections in 10 microns thickness were prepared from each node for autoradiographic studies. The lymph node slices were exposed for 18 h

Oscillometry of radial artery in acrocyanosis and cold sensitivity.

Acrocyanosis is reassessed on the basis of 34 new patients with diagnostic clinical features. The most useful tests are the skin temperature differences (STD) between wrist and end of fingers (or ankle and end of toes) and oscillometry over the radial and dorsalis pedis and posterior tibial arteries. Capillaroscopy of the conjunctival and nailfold vessels is useful in half the patients. Capillary resistance was not helpful. STD of 1.5 degrees C or more between skin of wrist or ankle and fingertip or toetip, is usually associated with more severe disease and capillary abnormalities on microscop

Laser Doppler measurements of skin blood flow before, during, and after lumbar sympathetic blockade in children and young adults with reflex sympathetic dystrophy syndrome.

Changes in the skin capillary blood flow (SBF) and temperature before, during, and 1 hour after unilateral lumbar paravertebral sympathetic blockade (LSB) were studied simultaneously with laser Doppler flowmetry and thermometry in patients with reflex sympathetic dystrophy syndrome. The baseline flow measurements in the toes on the affected limb were significantly lower than in the contralateral limb (p less than 0.01). During LSB, a 10-fold increase in SBF was detected within 4 minutes after injection of a local anesthetic agent when the sympathetic blockade was effective; an increase of more

Tips of the trade #42. An improved technique for creating a digital tourniquet.

A bloodless operating field for digital surgery is usually achieved by using either a pneumatic tourniquet or a soft-rubber catheter tied around the base of the digit. We present an improved technique for creating a digital tourniquet using a surgical glove. It is more effective and simple to use.

Morphological considerations of the first metatarsophalangeal joint.

This study was designed to examine the external morphology of the metatarsophalangeal joint and of the hallux. Approximately 187 first metatarsals and their associated proximal phalanges where measured for ten linear variables from human skeletal specimens housed in the Cleveland Museum of Natural History. The variables represented seven articular characteristics of the head of the first metatarsal, the length of first metatarsal, and the height and width of the base of the first proximal phalanx. Descriptive statistics for each of the variables are presented along with comparisons on gender a

Microvascular digit transposition following a two-digit amputation in an infant.

Multiple digit amputations in children demand special consideration to make subsequent hand function optimal. Replanted digits in children have a comparatively lower viability rate, but those that do survive usually go on to excellent function. In this case, an amputated thumb was severely mangled and not suitable for replantation. An amputated index finger was transposed to the thumb position. A six-month postoperative follow-up of the transposed digit confirmed satisfactory joint motion, restored sensibility, and unimpaired digit growth. The 11-month-old infant described is the youngest pati

Reverse flow fillet flap from a supernumerary finger.

We report a new technique for making a fillet flap from a supernumerary finger in three cases. These flaps are unique in possessing retrograde blood flow or both orthograde and retrograde blood flow and have a wider arc of rotation than provided by conventional methods. This technique permits one to design more freely. We believe this concept is also applicable to repairing other anomalies of the hand in situations where standard methods are not effective.

Reconstruction of traumatic absence of the thumb by microvascular free tissue transfer from the foot.

The development of microvascular surgical techniques during the last quarter century has advanced the ability of the hand surgeon to reconstruct the traumatically amputated thumb. The use of tissue from the foot has become the mainstay of therapy for this previously exceedingly difficult reconstructive problem. Although numerous minor variations of thumb reconstruction with use of the toes from the foot are available, three main techniques--the complete great toe transfer, wraparound flap, and second toe transfer--provide a predictable outcome. With multiple donor sites available, the surgeon

Reconstruction of traumatic absence of the thumb in the adult by pollicization.

Amputation of the thumb is a severe handicap. In an emergency situation, thumb amputation must be treated by means of reimplantation when possible. If reimplantation cannot be performed or fails, several methods of thumb reconstruction can be used according to various factors. These include the number of surviving fingers and the level of the thumb amputation. Pollicization is the first choice for amputations proximal to the metacarpophalangeal joint when four and even three fingers are present. It is the easiest and safest operation that supplies the best results both from the motor and senso

Reconstruction of traumatic or congenital amputation of the thumb by distraction-lengthening.

Reconstruction of the traumatically or congenitally amputated thumb presents a significant challenge. The individual needs of the patient must be matched with the armamentarium available to the surgeon. Metacarpal elongation via the distraction-lengthening technique, which uses local tissue without sacrificing any adjacent digits or toes, is a reliable means of increasing thumb length by 3 to 3.5 cm. The optimal situation for performing this procedure is the presence of an amputation in the perimetacarpophalangeal joint region. Although neo-osteogenesis has been shown to be useful in pediatric

Electron microscopic observations of degeneration of human Pacinian corpuscles in amputated fingers.

Amputated human fingers were used to observe the morphologic changes in degeneration of Pacinian corpuscles, and postoperative moving two-point discrimination of the replanted fingers was examined to analyze sensory recovery after replantation. Normal corpuscles are composed of an axon terminal and inner and outer cores, resembling a sliced onion. The inner core is composed of thin, multilayered lamellar cells, and the outer core consists of multiple layers of thin perineurial cells. Based on our morphologic findings, following mitochondrial degeneration in the axon terminal, the terminal and

[Clear connection between gun ownership and suicide. 92 per cent of attempts result in death].

In a retrospective study involving 20 police districts in Sweden, 348 completed or attempted suicides with firearms were found to have occurred during the period, 1976-88. Statistics on civilian firearms ownership were obtained from police authorities. Firearm suicide was found to be significantly correlated to gun ownership, through the 25 per cent increase in gun ownership during the period was not accompanied by a corresponding increase in the overall suicide rate. The head was the primary target in suicide attempts, both with longarms (rifles and shotguns) and with handguns (pistols and re

Phantom limb pain induced in amputee by strong magnetic fields.

An amputee (traumatic) experienced increased phantom limb pain when exposed to the magnetic fields of two magnetic resonance imagers. With a visual analog scale used to measure pain, electric stimulation studies demonstrated that the residual limb was unusually sensitive to subthreshold (for muscle twitch) levels of current. The painful symptoms produced mimicked those experienced in the presence of the imagers.

Opioidergic inhibition of flexor and extensor reflexes in the rabbit.

1. Recordings were made from gastrocnemius medialis (GM), semitendinosus (ST) and tibialis anterior/extensor digitorum longus (TA/EDL) motor nerves during mechanical and electrical stimulation of the skin of the foot in decerebrated and spinalized rabbits. 2. GM motoneurones were excited from the heel and not from the toes, whereas TA/EDL responded to stimulation at the toes but not at the heel. ST also responded to electrical and mechanical stimulation at the toes, but there was a disparity between the effects of the two types of stimuli when they were applied at the heel: ST motoneurones fir

Functional benefit of an adaptive myoelectric prosthetic hand compared to a conventional myoelectric hand.

Eight patients with a traumatic unilateral upper limb amputation, who used conventional myoelectric prostheses, were also fitted with a commercially available myoelectric prosthetic hand with an adaptive grip, in order to compare the functional benefit of the two types of prostheses. Comparisons were made regarding width of grip, force of grip, scores in a standardised grip function test and prosthesis preference. The conventional prosthesis showed significantly better results regarding these parameters. The adaptive hand does not appear to be fully developed for practical use in prosthetic re

[Use of autologous veins and streptokinase in vascular complications after replantation of the right forearm].

The authors present the case-history of a 20-year-old worker with an amputation of the forearm in the distal third. After shortening of the bone by 2.5 cm and osteosynthesis by means of grooved splints the blood vessels of the forearm were reconstructed by means of three autoveins. After 12 hours venous and arterial thrombosis developed. During reoperation the thrombotic portions of the vessels were resected and were again reconstructed by means of new autovenous grafts. Streptokinase was instilled into the amputate via the ulnar artery. During the subsequent postoperative period no serious va
