A study was undertaken to evaluate the ability of the high-frequency transvaginal scanning method to consistently image first- and early second-trimester fetal structures, such as body contours, long bones, fingers, face, palate, feet, toes, and the four-chamber view. Ninety-seven low-risk pregnancies were scanned from 9 weeks to 14 menstrual weeks inclusive. Accurate dating was ascertained. We undertook a prospective study of standard peripheral pulse oximetry versus a modified pulse oximeter probe applied to the tongue in order to determine the efficacy of this alternative monitoring site in children with thermal injuries. Finger transplantation from the contralateral hand for the purpose of thumb reconstruction is rarely performed, more widespread use of this procedure being prevented by the psychological problems for the patients. Two cases are described, in which a previously damaged index finger of the contralateral hand was transplanted. The metacarpophalangeal joint of the index finger replaced that of the thumb in both cases. A strong pinch grip with sensibility was achieved. The aesthetic result was substantially improved, both on the donor and the recipient hand.

During the period 1980-1989 35 patients with successfully replanted digits could be selected. Only 17 patients with 32 long fingers and 2 thumbs replanted were prepared to cooperate for this retrospective study, looking for the late functional results. The association between medical risk factors and the outcome of foot ulcers was evaluated in 208 consecutive diabetic patients with severe peripheral vascular disease (systolic toe blood pressure < or = 45 mm Hg). All patients were treated and followed by the same foot care team. Eighty patients healed primarily, 83 healed after a minor or major amputation, and 45 died. A comparison of two fundamentally different techniques of multiple digit replantation is studied: a digit-by-digit versus structure-by-structure method. We conclude from this review that the structure-by-structure method is advantageous in terms of the shorter duration of the surgical procedure and possibly in terms of the survival rate of the replanted digits.

43 children treated by flexor-to-extensor transfer for a total of 130 curly toes were retrospectively reviewed after a mean period of 8 years. According to an objective scoring system, 37 patients had a satisfactory result and 6 patients had a poor result of one or more toes. Thirty-five consecutive applications of external fixation to the hand, including 27 acute cases and 8 reconstructive procedures, were studied. In both settings, external fixation was used not only for skeletal stabilization but also for management of the soft tissues. Twenty of the 22 acute fractures healed, and six arthrodeses with interposition bone grafts resulted in fusion. Three septic nonunions resolved, and two united successfully.
