In 4 men with self-inflicted penile amputation, replantation was successful using microsurgical technique. Postoperative complications were minimal. In 1 man who had amputated his penis and then amputated his scrotum and testicles, a small skin graft was necessary to close a portion of the defect. The principles of Advanced Trauma Life Support (ATLS) were adopted by a Royal Navy surgical team deployed to northern Iraq. Over a 6-week period, 18 casualties of both military and civil trauma required active resuscitation, 10 being under the age of 16 years. Chilblains represents an idiopathic vascular cutaneous benign acral syndrome. Epidemiology and basic mechanisms remain hypothetic and the treatment is controversial. We report on nine patients with craniofrontonasal dysplasia (CFND). Seven classical cases had facial features suggestive of frontonasal dysplasia and coronal craniosynostosis. Extracranial abnormalities such as brittle nails with prominent longitudinal grooves or syndactyly of fingers and toes were observed in individual patients. In two families the father of classical cases showed a milder pattern of abnormalities, consistent with the diagnosis. We present a 2- to 13-year follow-up on our patients. In an eight-year period we treated 51 cases of vascular injury associated with fractures and/or dislocations or soft-tissue injuries of the limbs. We relied on a clinical diagnosis and immediate exploration of blood vessels rather than the time-consuming procedure of arteriography. All patients were operated on by the orthopaedic residents on duty and not by vascular surgeons. Only 17 (33%) were repaired within six hours of injury. Limb viability with good function was obtained in 38. Complications included six deaths, four amputations, two renal failures and delayed occlusion in one case.

Potential complications after replantation surgery are numerous and varied in their nature. The average replantation patient can expect somewhere between two to three subsequent operations to maximize function. Many surgical complications and unsatisfactory functional outcomes can be avoided through careful assessment of patients and their injuries.

In a retrospective study, 31 patients were identified over a period of 25 years as having sustained traumatic knee dislocations without associated fractures. Three patients had early amputation secondary to vascular complications. Ten popliteal artery disruptions and five peroneal nerve palsies were diagnosed. At the time of follow-up evaluation (average, 40 months), 16 patients were examined. Operatively treated patients (n = 6) tended to have better motion (129 degrees) than nonoperatively treated patients (108 degrees). Varus instability was seen in only two patients treated nonoperatively.

Gait analysis was performed on eight men who had unilateral traumatic below-knee amputation and on nine control subjects. Ten or nine-digit amputation is a very rare injury. From March, 1987 to June, 1988, we replanted two cases of ten-digit complete amputation and two cases of nine-digit complete amputation with success. The maximum ischaemic time was 39 h and the operation time was between 25 and 31 h. All digits survived completely except for partial necrosis of one finger. The follow-up has been 13-20 months, and all the patients have a pair of good, functional and aesthetically acceptable hands.

Two cases of upper extremity replantation at the level of the ulnar joint are presented. In both cases, the ulnar joint and adjacent anatomic functional structures were destroyed in accidents and excised during surgery. Some functional rehabilitation of the replanted extremities was obtained. In one case, the grasp function of the hand was recovered through primary use of the muscular apparatus of the upper arm. In another patient, the function of the lost ulnar joint was restored through microsurgical autotransplantation of the vascularized first metatarsophalangeal joint of the foot.

This study compares modifications of Regnauld's enclavement procedure, the in situ "hat-shaped" and in situ "inverted" osteochondral graft.
