This study reports the late results of eight palmar bypass procedures performed between 1983 and 1988 in eight men patients with a mean age of 43 years old, who presented with unilateral digital trophic disorders. The origin of the arterial lesions was post-traumatic in four cases, embolic in three cases, and aneurysmal in one. The procedures performed included four radiopalmar bypasses, two ulnar-palmar bypasses, one distal radial bypass and one palmopalmar bypass. The causes of vascular failures of a series of 183 injured fingers treated by microsurgery (80 reimplantations, 103 revascularisations) were investigated. This consecutive series was characterised by the fact that the operation was performed even in the presence of classical factors of poor prognosis: patients over the age of 40 years (27%), smoking (51%), crush injury or avulsion (32%), ischaemia time longer than 7 hours (46%). Sixty-one patients underwent 68 digital pulp amputations involving the distal phalanx (Zone 2 and 3). 46 unipedicular Vankataswami-Subramanian island flaps (VS flap) and 22 bipedicular Moberg-O'Brien flaps (MOB flap) were performed. Two cases of amputated digits are reported. One involved a period of ischemia at freezing temperatures, and the other at unusually high temperatures. Both fingers survived upon replantation. The implications of extreme temperature upon ischemic tissues are discussed.

We report a retrospective review of our series of thirty-one single joints harvested from the toe in twenty-six patients and transferred to the metacarpophalangeal or proximal interphalangeal joint of the finger. Sixteen arterialized venous flaps, free flaps that have arterial blood only flowing through the vein of the flap, were applied to reconstruct skin defects of the big toe resulting from wrap-around flap transfers. There were seven complete successes, six partial successes, and three complete failures. The clinical course of the cases indicated that the donor site itself and flap tension due to postoperative edema might play key roles in flap survival. The deformity of the hallux that is described in this article is most probably more common than we realize and is often seen in patients with mycotic nails and subungual exostosis. This pathology is easily corrected with a tenotomy-capsulotomy at the level of the distal interphalangeal joint. This procedure, when accomplished as described, does not affect the function of the hallux at the level of the metatarsophalangeal joint.

A recessive lethal mutation in axolotls that involves the kidneys, the Mullerian ducts and the limbs was described by Humphrey (1967). In the present experiments, we have examined the structural defects that lead to kidney malfunction and subsequent death in homozygous mutants and compared the defects with those observed in other axolotls lacking this mutant gene. This is a presentation of 34 patients with traumatic or intraoperative vascular injuries treated at the Rural Clinic am Plattenwald during the timeperiod from 1978 until 1988. Limb amputation was performed in three cases. Two patients died. Our goal is to show the importance in early recognition and treatment of such injuries which requires an efficient interdisciplinary cooperation between orthopedic and vascular surgeons, as well as the radiologist. The employment of angiography should be generous.

Over the past 18 years we have operated on 50 patients who had sustained distal amputation of fingers. Their average age was 30 years (range 2-72 years). Replantation of radial digits on the right hand was most common and the thumb was involved in nearly half the cases in our series. Out of the 50 cases we were able to repair both arteries in 35 patients and only one artery in the other 15. Vibration perception threshold (VPT) is increasingly used as a measure of large nerve fibre function in studies of patients with diabetes and in other disorders. This is the first report of the clinical use of direct intra-arterial streptokinase infusion to promote the salvage of a failing thumb replant. Anastomotic revision was performed on two occasions following thrombosis in the vein grafts used to replant an avulsed thumb. A series of 55 second toe to long finger transfers in 42 patients are reviewed. The mean age is 17 years. The mean follow-up is more than 4 years. Two-point discrimination averages 8 mm. Dorsal compound traumatic thumb loss can be reconstructed in selected young and well-motivated patients with a custom-made partial great toe transfer. The transfer, including the nail complex and vascularized piece of bone, can be fashioned by longitudinally cutting the distal phalanx to approach the appearance of the opposite thumb and to allow for more normal nail appearance. Ten patients illustrate this technique and emphasize its reconstructive potential as well as cosmetic acceptance.
