Contralateral grafts were performed on the larval limb buds of the anuranBufo bufo. The dorsoventral axis of 80 buds at stages IV or V was inverted. Ten tadpoles were used as controls. Fifty-two supernumerary structures developed, all of them in dorsal or ventral locations on the host stump. The majority (32 out of the 44 outgrowths with more than 3 toes) were normal limbs of stump handedness. However, the following abnormal structures were also observed: 2 double-posterior, 3 mixed-symmetric, and 7 undetermined cases. Two cases of successful digital replantation are presented in which arterial inflow was provided by a proximal arterial-to-distal venous anastomosis. This technique provides an alternative means for replantation when no suitable artery for anastomosis is available in the amputated part.

The use of muscle flaps for treatment of chronic wounds has become more widespread. The specific use of intrinsic muscle flaps of the foot for treatment of diabetic ulcerations has the potential to provide alternative ways of avoiding amputations. A case report is presented that illustrates the use of the flexor hallucis brevis flap for a chronic ulceration beneath the first metatarsal head. The technique and indications are discussed.

Forty-two complete thumb replantations performed between 1980 and 1984 were reviewed. The mean follow-up time was 14 months. Replantation was attempted for all thumb amputations regardless of mechanism or severity of injury. Sixteen (38%) failed intraoperatively or postoperatively. Thumbs with narrow zones of injury showed a significantly higher survival rate than those with wide zones of injury. We reviewed 18 patients and two traumatic amputation specimens with injuries involving the distal ulnar physis. Type 1 growth mechanism injuries were the most common fracture pattern with premature physeal closure and ulnar shortening occurring in 55% of the patients. Other consequences included radial bowing, ulnar angulation of the distal radius, and ulnar translocation of the carpus. Most of the patients, however, were asymptomatic. 43 microvascular procedures were performed in 38 children, (age limit 15 years) over a three year period. The average age was 5.4 years. The majority of the procedures were elective (63%), the remainder urgent or emergency. The overall vascular success rate was 93%, confirming previous reports. In traumatic proximal forearm amputation where replantation is not possible, "spare parts" from the amputated segment can be used to maintain adequate length for successful use of a below-elbow prosthesis. Two patients with long-term follow-up are presented in whom forearm length was preserved with the use of a sensate palmar skin free flap.

We studied the induction of leg paresthesias by magnetic stimulation of the brain in seven patients with thoracic T9-12 spinal cord injury and in four normal volunteers by delivering transcranial magnetic stimulation over scalp positions 1 cm apart with a Cadwell MES-10 magnetic stimulator and an 8-shaped magnetic coil at 100% stimulus intensity. Traumatic hemipelvectomy is an uncommon and devastating injury with few patients alive when they reach the hospital. There are only 19 reported survivors of this injury.
