This study on vascular complications in digital replantation and revascularization covers the six-year experience of a universitary replantation center. The material comprises 143 patients with 67 replantations and 137 revascularizations. Inadequate circulation was observed in 21.6% of the cases, or 17.6% of the digits. Six patients with no hemodynamically significant atherosclerotic lesions of the lower limb arteries but with ischemic changes of the feet or toes were studied and diagnosed as having atherothrombotic microembolism. Using stereoscopic angiography, the entire arterial system of the foot of the macaque was analyzed. The arteria saphena, instead of the a. tibialis anterior, reaches the dorsum of the foot, and its branches supply most of the foot. The dorsal arteries are dominant as far as the metatarsal spaces of the lateral four toes. The a. metatarsea dorsalis II showed a tendency to be the single dominant artery and to give rise to even the a. digitalis communis plantaris IV through the catella plantaris distalis. This tendency was observed more clearly in other macaques species than the Japanese monkey.

The present study was designed (1) to characterize the subliminal responses of dorsal horn neurons to stimulation of the sural nerve, and (2) to correlate the type of response to this stimulus with the responses to natural mechanical stimulation of the skin. A composite of nail and its skin appendages can be transferred for the foot to the hand by microvascular anastomosis. For technical reasons, the most suitable donor nail is from the big toe, and this most closely approximates the thumb nail. Vein-patch angioplasty is a well-established technique in vascular surgery to reduce the critical narrowing of the arteriotomy site. In microsurgical end-to-side anastomosis, although the arteriotomy is not closed, it is not known if anastomosis through a sclerotic segment contributes to narrowing at the anastomotic site. It is certainly technically difficult to perform a microanastomosis with fine sutures through a sclerotic segment. Two cases are illustrated in which vein-patch angioplasties were successfully used for microsurgical anastomoses.

Referred limb symptoms (RLS) in chronic low back pain patients without signs of root affections were studied in 212 men and 126 women, aged 36-55 years, who were at work, but suffered from chronic or recurrent low back pain. RLS during the past few months were experienced by 17% daily and 22% occasionally. Previous RLS were reported by 34%, whereas 27% had never had such symptoms. A total of 25 cases of acute injuries caused by a meat mincer are presented. Of these, 20 were children. The pattern of injury and the extent of damage to individual digits of the hand are defined. Due to the mutilating nature of the injury, attempts at replantation or revascularization failed in all but two of the ischaemic digits. Emphasis must be placed on the safety and prevention of these dreadful accidents.

All diabetic patients aged 15-50 years (n = 395) in the county of Umeå (population 118,500) were invited to have a standardized foot examination and 380 (96%) attended. Three-quarters (78%) had Type 1 diabetes, 20% Type 2 diabetes, and 1% secondary diabetes. They were compared with 100 healthy control subjects. We describe a 12-year-old boy with EPC (Epilepsia Partialis Continua) who showed well localized myoclonic jerks in the left toes continuing for more than 19 months.
