The recovery of sensibility following digital replantation is essential in the restoration of hand function. We reviewed 12 series of digital replantations between 1977 and 1989. Three hundred sixty-seven fingers and 87 thumbs were successfully replanted. Mean age was 32.5 years. Mean follow-up was 33.5 months. Mean static two-point discrimination (S2PD) was 9.3 mm in clean thumbs vs. 12.1 mm in crush/avulsion thumb replantations. This paper describes a case in which a young woman's two lower limbs were crushed and amputated from the upper part of the thighs by a train. She was in serious hypovolemic shock at the time. With rapid infusion of intravenous fluids, she recovered from shock quickly, avoiding acute renal failure and ARDS. Heterotopic transplantation was performed. Two years later, the limb which had undergone heterotopic transplantation had partly restored function.

The Syme amputation is an old operation that has been used during this century primarily as a means of treating traumatic injuries to the forefoot in military patients. In 1984 we made a deliberate attempt to perform the operation in a highly selective group of dysvascular patients with forefoot necrosis who happened to have a palpable posterior tibial pulse. Two patients with hitherto unrecognized spells of involuntary movements in legs are described. They occurred on dropping off to sleep, were preceded by a burning sensation and resembled myoclonus. Fifty-five patients with griseofulvin-unresponsive dermatophytosis caused by Trichophyton rubrum were treated with itraconazole. They had either tinea corporis or "dry type" infections of the palms, soles, or nails. 16 patients (75% women) were operated for a claw nail involving one of the three middle fingers. Thirty-seven replanted digital units and four thumb replantations had a flexor tendon tenolysis at an average of 10 months after replantation. The results were assessed by measuring total active motion, potential active motion, and by the formula of Strickland and associates. In a study of 27 underground miners exposed to hand or whole-body vibration, cold or other vasoconstrictive environmental factors, higher prevalences of Raynaud's phenomenon in both fingers and toes were found than in a control group not exposed to vibration. There were no Raynaud-like phenomena among miners not exposed to vibration. There was a positive correlation between the two locations. This may mean a general susceptibility of vasospasm, but it seems more probable that the disorders are caused by vibration exposure to fingers and toes.

This study reports the results of 30 patients who entered a program of sensory reeducation following toe-to-thumb transfer. Results were analyzed after subdividing the patients into those whose injury had produced severe scarring (fibrotic group, N = 15) and those with clean, more distal amputations (non-fibrotic group, N = 15). Complete ring avulsion amputation (class III) injuries are difficult management problems, especially when they occur in heavy manual workers. By excising the bit of bone detached with the skin tube, opening up the skin tube into a flap and replanting it onto the proximal stump, two such cases were salvaged successfully, with early recovery of function. The fingers were shortened by 1.5 cm, had a normal nail, touch sensation and absence of cold intolerance and pain. Both of the patients returned to work in 4 months.
