Three cases of hand injury caused by meat grinders are presented. All 3 injuries involved the dominant hand and resulted in varying degrees of deformity. Two of the 3 patients arrived in the emergency room with the injured hand still firmly wedged in the meat grinder. Although these injuries continue to prove very mutilating, maximum restoration of the injured hand can be accomplished by careful extrication, followed by preservation and reconstruction of all viable tissues. Soft-tissue chondromas are benign, relatively rare lesions. The clinicopathologic and radiologic features in six cases are presented and the literature is reviewed. Vital capillary microscopy was employed in a study of the toe dorsum capillaries in 92 middle-aged diabetics and 96 controls of similar age and sex distribution. As a general finding most vision fields in the same toe showed an almost identical capillary pattern. The volar V-Y plasty is a well accepted method of management for transverse fingertip amputations. It has been suggested by some authors and assumed by some surgeons that fingertip sensation is nearly normal following the procedure. Minimal traumatic surgery is considered the preferred treatment for painful digital deformities including hammertoes, overlapping toes, mallet toes, underlapping toes, and painful corns. With the introduction of technology such as intraoperative x-ray monitoring, these procedures can be performed more efficiently, safely, and without unnecessary hospitalization in most cases. Mobility is kept to a minimum, yet the patients can be comfortably mobile while they are recovering.

In 42 patients with arterial occlusive disease of the leg, the hemodynamic effects of prostaglandin E1 (PGE1) given intravenously were studied. Blood pressure of the leg and crest time of the pulse wave did not change significantly, while increases in blood flow at the calf and foot, amplitude of the pulse wave, velocity, and skin temperature at the foot and toes were significant. The relationship between the number of digital nerves sutured and sensory recovery was investigated in 50 fingers of 33 patients, with follow-up of more than five years after replantation of the amputated fingers. We studied the reproducibility of four tests of peripheral occlusive arterial disease in 54 subjects, 32 of whom had this disease. We have observed mutilative self-biting leading to multiple finger amputations in two patients following C4 complete spinal cord injury (SCI). Both men were of normal intelligence without psychosis and each had a neurotic personality and history of fingernail biting. They related the self-biting to anxiety and depression. We believe these to be the first English language reports of multiple finger amputations due to self-biting following SCI.

Osteocutaneous flaps from the foot are being utilized more for thumb and digit reconstruction. We have presented a few unique situations in which these flaps are beneficial. Not all patients with deformities similar to those described in this presentation should have these reconstructive procedures. The patient must understand the risks of donor site morbidity and possible complications. However, the advantages of this technique are that only one surgical procedure is necessary, the hospitalization is short, the cosmetic result is good, and the functional restoration is satisfactory.

Scapulothoracic dissociation represents a closed traumatic forequarter amputation associated with major neurovascular injury. Radiographic findings consist of lateral displacement of the scapula on a well-centered plain film of the chest. Associated abnormalities include disruption of the acromioclavicular joint or, as in our two cases, distracted fractures of the clavicle. Persons in whom scalpulothoracic dissociation is diagnosed should undergo emergency angiography and surgery.
