Functional comparison between pollicization and toe-to-hand transfer for thumb reconstruction.

The functional results of two methods of thumb reconstruction, pollicization, and toe transplantation were evaluated in four groups of patients: Group I, those missing the thumb but with four other normal digits; Group II, those missing the thumb with partially mutilated or amputated other digits; Group III, those with a metacarpal hand; and Group IV, those with a distal thumb amputation. In each group, results were compared in six categories: mobility, strength, sensibility, cosmetic appearance, pinch accuracy, and grasping power. In Group I, pollicization provided superior sensibility and mo

Preaxial polydactyly type 4: variability in a large kindred.

A five generation family with type 4 preaxial polydactyly is reported. The 21 affected individuals demonstrated variability in expression without apparent sex influence and penetrance which was complete. The deformities were more severe in the feet than the hands. Anteroposterior flatness of the thumbs was the only manifestation of the trait in the hands of several affected family members.

One-stage reconstruction of the penis with free skin flap: report of three cases.

Three cases of penile defect of varying extent were reconstructed with free skin flaps from the forearm based on the radial artery. In two cases, part of the forearm flap was used to reconstruct the urethra, and the rest of the flap to reconstruct the corpus penis. A piece of autogenous cartilage was then inserted between the two layers. All procedures were completed in a one-stage operation. In the third case, a free skin graft was used for urethral reconstruction. Surgery in all three cases was successful, providing a penis of acceptable appearance as well as excellent urinary function.

Open flexor tenotomy for hammer toes and curly toes in childhood.

Sixty-two children were reviewed between 3 and 14 years (average 9.8 years) after flexor tenotomy for curly toes or hammer toes. No patients were aware of loss of flexor power in the toes. In only 5% of 188 toes was the operation unsuccessful. When the cause of failure was identifiable it proved to be that the scar crossed one or more flexor creases. None of the operated toes had an abnormally extended posture; only one toe was stiff and this resulted from tethering by a scar. It is concluded that open flexor tenotomy is an effective method for correcting curly toes and hammer toes in childhoo

Class IIA ring avulsion injuries: an absolute indication for microvascular repair.

The class II ring avulsion category, includes those patients in whom only digital arteries are damaged but all other structures are intact and functional (here labeled class IIA). Current literature suggests this is a rare lesion. Seven patients with this specific injury in whom the affected digits were nonviable are reported. Four of the seven were misdiagnosed on initial emergency room evaluation. Two did not seek additional medical attention and the condition progressed to necrosis and amputation. The other two, who sought additional treatment because of progressive ischemia, and three addi

Mixed polydactyly. An unusual case of a patient with seven-toed feet.

Polydactyly is an entity of great etiologic heterogeneity, and many facts about its genetics remain unknown. Therefore, the relative terminology and classification in the literature are rather obscure. Mixed polydactyly, i.e., polydactyly with both preaxial and postaxial involvement in the same person, is very rare, and crossed polydactyly is even more so. A seven-year-old girl with six fingers on each hand and seven-toed feet, lower-mixed and crossed polydactyly with syndactyly, underwent operation mainly for cosmetic reasons. The report of this rare case confirms the great variability and ex

Amputation neuroma of the orbit. Report of two cases and review of the literature.

Two patients developed proptosis and a slowly enlarging mass with increasing discomfort in the orbital socket 17 and 25 years following enucleation. Preoperative CT scans revealed a single cystic structure within the orbits of each case with distinct soft tissue tumors adjacent to the cysts. The cystic structures and the adjacent solid masses were removed en bloc necessitating reconstruction of the orbits with a dermal fat pad (case 1) and a mucous membrane graft (case 2). Postoperatively the patients were free of complaints. Microscopically, the cystic structures were identified as conjunctiv

Dilemmas in dealing with the blue toe syndrome: aortic versus peripheral source.

Six patients with unilateral blue toe syndrome presented a diagnostic dilemma with regard to the source of embolization: central aortic versus peripheral. Two patients had moderately severe aortoiliac atherosclerosis associated with focal stenoses in the superficial femoral arteries, and four patients had mild aortoiliac disease associated with localized plaques confined to either the superficial femoral or popliteal arteries. In all patients, it was elected to explore the peripheral lesions first. At operation, ulcerated plaques or focal stenoses were found, and all lesions had adherent white

[Variability of hand and foot arteries].

In the hand the superficial palmar arch is variable and the communication between the radial and the ulnar artery is open in only 42% of cases. In 58% there is no connection, and in most cases the common digital palmar arteries arise from the ulnar artery. In 10% the median artery participates in the vascularization of the hand: 5% joining the superficial palmar arch, and the other 5% when the superficial arch is open continue into the first or second palmar digital arteries. The deep palmar arch is constantly found closed (only 3% are open). The dorsum of the hand is usually supplied by the r

Bringing the feet in from the cold: thermal biofeedback training of foot-warming in Raynaud's syndrome.

The biofeedback-assisted treatment of a case of Raynaud's syndrome was examined in order to determine the relationship between learning of the hand-warming response and the subsequent ability to produce foot-warming responses. It was found that fluctuations in dermal hand and foot temperatures were not significantly related, either within or across treatment sessions. The obtained hand-warming response was of high magnitude and rapidly learned, while the foot-warming response was more modest and took substantially more trials to learn. These results indicate that an easy generalization of the

Muscle but not cutaneous C-afferent input produces prolonged increases in the excitability of the flexion reflex in the rat.

Stimulation of cutaneous afferent fibres in the sural nerve and muscle afferent fibres in the gastrocnemius-soleus nerve at a strength that excites C axons produces a delayed and long-lasting burst of activity in posterior biceps femoris/semitendinosus flexor motoneurones. Following a 20 s stimulation at 1 Hz to the sural nerve the flexor motoneurones continue to fire for 20 s while a similar stimulus to gastrocnemius-soleus nerve results in an after-discharge lasting three times longer. Using stimuli to the sural and gastrocnemius-soleus nerves as conditioning stimuli (20 s, 1 Hz) changes in

Blunt injury to the subclavian artery.

This paper reports two cases of closed injury to the subclavian artery, one resulting from avulsion and the other from compression from a seat-belt. Diagnosis may be delayed because of the absence of both haemorrhage and distal ischaemia. The surgical approach may be via a standard supraclavicular incision, but frequently a combined cervicothoracic approach is necessary.

In a series of 10 patients with traumatic dislocation of the knee joint, closed reduction could be accomplished in nine. Conservative treatment was employed in four and operative repair of the ligaments and capsule in six patients. In two of three patients with complicating artery injury, vascular repair was successful. Above-knee amputation was performed in one patient because of delay in the diagnosis of vascular injury and in another patient because of arteriosclerotic gangrene. At follow-up examination, on average 6 years after the accident, the stability and motion of the knee were evalua

[An experimental study of the effect of fluorocarbon perfusion for amputated limb preservation. The effect of inhibition for replantation toxemia and muscle tissue destruction].

In replantation surgery, it is widely accepted that replantation toxemia or muscle destruction in replanted limbs might occur after a long time of ischemia. Their possibilities are particularly high after replantation of the amputated limbs which contain more muscle tissue than tendon and bones. The present study was performed to investigate the efficacy of fluorocarbon (FC: artificial blood) perfusion to the amputated limbs in preventing these problems after replantation. The hind limbs of dogs were completely amputated at mid-thigh. Amputated limbs were divided into two groups. One was store

The findings are compared with those of well documented cases with a similar deleted segment of the long arm of chromosome 13. Although it seems obvious that a clinical syndrome for the distal deletion 13q appears to ex

Toe-to-hand transfers in reconstructive surgery of the hand. Experience with seventy-one cases.

Based on a homogeneous series of 71 toe-to-hand transfers performed between February 1975 and December 1982 the same surgeon, the authors have tried to define their present day indications. Technical modifications have been described for the second toe transfer : isolated dorsal approach of the toe with early-stage resection of the second metatarsal bone, utilisation of the plantar metatarsal artery of the second interosseous space, utilisation of the volar vessels for the anastomosis at the recipient site, dorsal subcutaneous tunnel for the venous anastomosis of the hand. [Reconstruction of grip function in cases of a fingerless or missing hand].

In two cases a grip reconstruction by double toe-to-hand transfer is demonstrated. In the first patient all fingers of the left hand were amputated at the metacarpo-phalangeal joint level. The second and the third finger were reconstructed by transplantation of the second toe from the left foot and the third toe from the right foot. The distal and middle phalanx of the transplanted third toe had to be re-amputated because of mummification two weeks after the transplantation and was covered with a groin flap. Good function of the newly formed grip could be demonstrated. The second patient was a

Prenatal diagnosis of Meckel syndrome: case reports and literature review.

We report on 2 couples at risk to have Meckel syndrome (MS) offspring. Three pregnancies were monitored by ultrasonography and alpha-fetoprotein (AFP) assay in midtrimester. Of the 2 involving an affected fetus, one was thought to be unaffected. A literature survey was undertaken. Sources of error seem to lie in the heterogeneity of clinical manifestations and the variable onset and rate of progression of clinical components of MS and limitations in ultrasonographic resolution inherent in the scanner or due to the associated oligohydramnios. The following observations/suggestions are made conc

Hallux varus--a study of thirty cases.

Although both congenital and acquired hallux varus have been described, the deformity has been considered uncommon. Contrary to this belief, we noted that it is fairly common in the unshod population. Analysis of 30 cases shows that congenital hallux varus is the most common type encountered and it is of three types, namely: primary, secondary, and teratogenic. The congenital primary and secondary varieties are apparently easy to treat in infancy, but if left untreated, they progress with age in unshod persons. The more severe grades of hallux varus are associated with medial deviation of the

Surgery for congenital deformities of the feet.

There are a multitude of congenital deformities that can occur in the feet. Some are very common and others are more remote. They can frequently be corrected with conservative care, such as splinting, casting, and various exercises over a period of time. Those that are not resolved by conservative measures can generally be surgically improved or corrected. A few conditions should be left for surgical resolution after maturity occurs.
