A novel presentation of childhood systemic lupus erythematosus in a 6-year-old child.

Childhood systemic lupus erythematosus (cSLE) is an autoimmune disorder, characterized by the formation of autoantibodies, that most often presents in children around the age of 12. Antiphospholipid antibody syndrome (APS) is a condition in which antiphospholipid antibodies produced by the patient cause the formation of thromboses. While reports estimate that 30%-40% of cSLE patients have antiphospholipid antibodies, only 4% develop symptomatic APS, with venous thromboses being more common than arterial. Overall, thrombosis has an incidence of about 0.1/10,000 in the pediatric population and u

Reconstruction of Tumor-Induced Pelvic Defects With Customized, Three-Dimensional Printed Prostheses.

BACKGROUND: Reconstruction of pelvis girdle stability after tumor-induced hemipelvectomy remains challenging. We surgically treated 13 patients with custom-made, three-dimensional printed hemipelvic prostheses. We aim to identify the preliminary outcomes for patients who have been managed with more mixed regions of prosthetic pelvic reconstruction and the feasibility of two reconstructive systems. METHODS: Seven male patients and 6 female patients treated at our center between January 2019 and May 2021 were included. There were 11 primary sarcomas and 2 solitary bone metastases. After en bloc

Clinical Manifestations of Constriction Band Syndrome.

BACKGROUND: Constriction band syndrome (CBS) is a congenital limb anomaly with varying presentation. We sought to characterize the clinical manifestations of CBS by analyzing a large cohort of patients. Our secondary aim was to evaluate potential risk factors for CBS. METHODS: We retrospectively reviewed the records for all patients with CBS who had presented to our tertiary medical center between 1998 and 2018. Examination by a pediatric orthopaedic surgeon and the presence of pathognomonic features were inclusion criteria. Clinical notes, photographs, and radiographs were reviewed to determ

Anticardiolipin Antibodies Presenting With Acute Renal Infarction in a Healthy 26-Year-Old Female.

A 26-year-old female presented to the emergency department with right lower quadrant pain. This pain lasted a couple of days, worsened, and was associated with nausea but no vomiting. Upon presentation, physical examination showed tenderness to palpation in the right lower quadrant with bilateral purple non-blanching discolorations present on her toes and no costovertebral angle tenderness. Contrast-enhanced computed tomography (CT) showed acute right renal infarction. Thrombophilia workup was done, which showed elevated antinuclear antibodies and anticardiolipin antibodies. Elevated lactate d

Functional outcome after digit replantation versus amputation.

BACKGROUND: The success of digit replantation is mainly based on survival rates. The functional outcome as well as the recovery of sensibility are essential parameters for judging the outcome after digit replantation but have been poorly assessed in previous studies. PATIENTS AND METHODS: Forty-eight patients with 56 complete traumatic digit amputations occurring between 2008 and 2013 returned for a follow-up examination, the earliest being 6 months postoperatively. Each patient's range of motion, fingertip-to-table distance, fingertip-to-palm distance, grip and pinch strengths, static two-po

External rotation of the foot position during plantarflexion increases non-uniform motions of the Achilles tendon.

The medial (GM) and lateral gastrocnemius (GL) muscles enroll to different subparts of the Achilles tendon to form their respective subtendons. The relative gastrocnemii activations during submaximal plantarflexion contraction depend on the position of the foot in the horizontal plane: with toes-in, GL activation increases and GM activation decreases, compared to toes-out. The aim of the current study was to investigate whether horizontal foot position during submaximal isometric plantarflexion contraction differently affects the subtendons within the Achilles tendon in terms of their (i) leng

Effect of vibrotactile stimulation of the hallux nail on segmental coordination: A secondary analysis using uncontrolled manifold analysis.

Controlling center of mass (CoM) movement in the mediolateral direction is imperative for stable walking. During walking, CoM movement is adjusted by the coordination of each body segment, which can be evaluated using uncontrolled manifold (UCM) analysis. UCM analysis evaluates segmental coordination by analyzing variablity in motor movement among the different segments of the body. The vibrotactile stimulation of the hallux nail can augment the sensory information of the plantar surface for necessary motor control. This study aims to investigate the effect of the vibrotactile stimulation of t

Development of a Standard Push-up Scale for College-Aged Females.

The ACSM/CESP push-up test exemplifies the limiting nature of the gender binary in fitness. Males perform the standard push-up (from toes) while females perform the modified push-up (from knees), even if capable of multiple standard push-ups. Differences in upper body strength are used to justify the test protocol. Though the load difference between modified and standard positions is substantially less than the gender strength gap. Additionally, current fitness ratings are over 30 years old. The purpose of this study was to develop a new standard push-up rating scale for college-age females. C

Geometric Morphometrics Reveal Shape Differences in the Toes of Urban Lizards.

Urbanization, despite its destructive effects on natural habitats, offers species an opportunity to colonize novel niches. Previous research found that urban Anolis lizards in Puerto Rico had increased adhesive toepad area and more ventral toepad scales, traits that are likely adaptive and genetically based. We further investigated these phenotypic changes using geometric morphometrics to measure differences in toe shape, toepad shape, and lamellar morphology. Our results indicate that the increased toepad area of urban Anolis cristatellus lizards in Puerto Rico is not simply an isometric incr

Dynamic assessment of self-regulated learning in preschool.

The assessment of self-regulated learning is a relevant research topic in early childhood development. However, there are few ecologic measures to assess self-regulated learning in preschool as a dynamic and multidimensional process. This study aims to fill this gap by presenting the development and validation of the Dynamic Assessment of Self-regulated learning in Preschool (DASP) method. A dynamic assessment of the construct may constitute an important contribution as it enables the acquisition of cross observational, verbal, and performance data. The DASP method was developed within a theor

Novel mutation in the SALL1 gene in a four-generation Chinese family with uraemia: A case report.

BACKGROUND: Approximately 10% of adults and nearly all children who receive renal replacement therapy have inherited risk factors or are related to genetic factors. In the past, due to the limitations of detection technology and the nonspecific manifestations of uraemia, the etiological diagnosis is unclear. In addition to common monogenic diseases and complex disorders, advanced testing techniques have led to the recognition of more hereditary renal diseases. Here, we report a four-generation Chinese family in which four individuals had a novel SALL1 mutation and presented with uraemia or abn

Reversed sexual dimorphism in the leg muscle architecture of the Eurasian sparrowhawk.

Reversed sexual dimorphism (RSD) in size is a deep issue in evolutionary biology. RSD in body mass and linear measures is pronounced in diurnal predatory bird species, especially in those that feed on other birds. Size differences between males and females in internal organs or systems, such as the appendicular musculature, are less well known. In this study, 14 muscles related to toe closure in the Eurasian sparrowhawk (Accipiter nisus), a bird-eating species, were selected for dissection and architectural measurement. The muscle mass (MM), physiological cross-sectional area (PCSA), and fiber

Rapid Progression of Heterotopic Ossification in Severe Variant of Fibrodysplasia Ossificans Progressiva with p.Arg258Gly in ACVR1: A Case Report and Review of Clinical Phenotypes.

Fibrodysplasia ossificans progressiva (FOP) is a rare skeletal disorder characterized by congenital malformation of the great toes and progressive heterotopic ossification. Malformation of the great toes appears at birth, while heterotopic ossification generally occurs during childhood and rarely occurs during infancy. Classical FOP results from the heterozygous p.Arg206His variant of the ACVR1 gene, which encodes Activin A receptor type 1. Recently, some atypical FOP patients with other ACVR1 gene variants and clinical features that are not observed in classical FOP patients have been reporte

Outcomes of Hallux Amputation Versus Partial First Ray Resection in People with Non-Healing Diabetic Foot Ulcers: A Pragmatic Observational Cohort Study.

There are few data comparing outcomes after hallux amputation or partial first ray resection after diabetic foot ulcer (DFU). In a similar context, the choice to perform one of these two surgeries is attributable to clinician preference based on experience and characteristics of the patient and the DFU. Therefore, the purpose of this study was to determine the more definitive surgery between hallux amputation and partial first ray resection. We abstracted data from a cohort of 70 patients followed for a 1-year postoperative period to support clinical practice. We also attempted to identify pat
