Idiopathic Toe Walking: An Update on Natural History, Diagnosis, and Treatment.

Toe walking is a common presenting report to an orthopaedic practice. Evaluation of a child with toe walking includes a thorough history and physical examination to elucidate the diagnosis. When no other diagnosis is suspected, a diagnosis of idiopathic toe walking is often given. Despite the high prevalence of the condition, there is notable controversy of the nomenclature of the disease. Recent research has shed more light on both the natural history and the genetic basis. The use of motion analysis, including EMG and multisegment foot model, may help both differentiate children with mild fo

Children with idiopathic toe walking display differences in lower limb joint ranges and strength compared to peers: a case control study.

BACKGROUND: Idiopathic toe walking (ITW) is an exclusionary diagnosis. There has been limited exploration of lower limb active range of motion and strength measures in children with ITW. This researched aimed to determine any differences in lower limb muscle active range of motion and strength in children who have ITW, compared to normative data collected from children who displayed typical gait. METHODS: Children were recruited with had a diagnosis of ITW, aged between 4 and 10 years, and no recent treatment. Data collected included parent reported data such as time spent toe walking, percen

Disability, economic and work-role status of individuals with unilateral lower-limb amputation and their families in Bangladesh, post-amputation, and pre-rehabilitation: A cross-sectional study.

BACKGROUND: Amputation has significant negative impacts on physical, psychological, social and economic wellbeing of individuals and families. This is potentially compounded by significant delays to rehabilitation in Bangladesh. OBJECTIVE: To quantify disability, occupation and socioeconomic status of people with unilateral lower-limb amputation (LLA) and their families in Bangladesh, post-amputation and pre-rehabilitation. METHODS: Between November 2017 and February 2018, people with unilateral LLA attending two locations of Center for the Rehabilitation of the Paralyzed, Bangladesh, for pr

Incisional negative pressure wound therapy to reduce surgical-site infections in major limb amputations: a meta-analysis.

PURPOSE: Incisional negative pressure wound therapy (iNPWT) has shown effectiveness in the treatment of high-risk surgical wounds. Especially patients with diabetes-induced peripheral arterial disease undergoing major limb amputation have a high intrinsic risk for post-surgical wound infections. While normal gauze wound dressings do not cause stimulation of microvasculature, iNPWT might improve wound healing and reduce wound complications. The purpose of this study was to systematically review the literature for rates of wound complications and readmissions, as well as post-surgical 30-day mor

Cardiovascular System Modeling Using Windkessel Segmentation Model Based on Photoplethysmography Measurements of Fingers and Toes.

BACKGROUND: Photoplethysmography (PPG) contains information about the health condition of the heart and blood vessels. Cardiovascular system modeling using PPG signal measurements can represent, analyze, and predict the cardiovascular system. METHODS: This study aims to make a cardiovascular system model using a Windkessel model by dividing the blood vessels into seven segments. This process involves the PPG signal of the fingertips and toes for further analysis to obtain the condition of the elasticity of the blood vessels as the main parameter. The method is to find the Resistance, Inductan

Using Deep Learning to Predict Minimum Foot-Ground Clearance Event from Toe-Off Kinematics.

Efficient, adaptive, locomotor function is critically important for maintaining our health and independence, but falls-related injuries when walking are a significant risk factor, particularly for more vulnerable populations such as older people and post-stroke individuals. Tripping is the leading cause of falls, and the swing-phase event Minimum Foot Clearance (MFC) is recognised as the key biomechanical determinant of tripping probability. MFC is defined as the minimum swing foot clearance, which is seen approximately mid-swing, and it is routinely measured in gait biomechanics laboratories

Hair tourniquet syndrome: A retrospective study.

OBJECTIVE: Hair thread tourniquet syndrome (HTS) is a pediatric condition in which human hair or synthetic fiber encircles and strangulates a body appendage causing tissue necrosis. Few epidemiologic studies have been done. Our objective was to better define the demographics, clinical features, and predisposing factors for this condition. MATERIALS AND METHODS: Medical records of all infants up to 1 year old seen in the orthopedic emergency department of Galilee Medical Center were searched for the diagnosis of HTS or its ICD equivalent. RESULTS: Forty-one cases of hair tourniquet syndrome (

Outcomes of bipedicle V-Y 'cup' flap for repair of volar oblique fingertip amputations.

We used bipedicle V-Y 'cup' flaps to reconstruct 13 consecutive volar oblique fingertip amputations encountered in a nearly 2-year period. All flaps survived completely with no bone protuberance, scarring or neuroma evident at the end of follow-up (mean, 7 months; range 4-13 months). Two of the 13 patients had moderate or severe tenderness, three had moderate or severe cold intolerance and two had noticeable hook nails. There were no significant (≥10°) flexion contracture in the interphalangeal joints. The mean static two-point discrimination score was 4 mm (range 3-6) and the modal Semmes-Wei

Deep vein thrombosis in donor or recipient veins encountered during lower extremity reconstruction with a free anterolateral thigh perforator flap: How do we deal with it?

BACKGROUND: The free anterolateral thigh perforator (ALTP) flap has been successfully adopted to reconstruct traumatic soft tissue defects in the lower extremities. However, the occurrence of deep vein thrombosis (DVT) in donor or recipient veins has been overlooked, and there has been no reliable guideline to manage it. Therefore, in this study, we review our cases where the ALTP flaps were transferred to traumatic lower limbs even though DVT was found in the pedicle or recipient veins. Furthermore, based on our experiences, we suggest an algorithmic approach for dealing with DVT. PATIENTS A

Extra-anatomic temporary intravascular shunting to assist with replantation of an amputated hand with prolonged ischemic time.

Although temporary intravascular shunting has been reported to assist with hand revascularization and replantation after traumatic amputation, most reports have described using ipsilateral upper limb inflow. We present a case of a traumatic hand amputation in a patient who had presented with 4 to 5 hours of warm ischemic time. Because the replantation team believed that extra-anatomic revascularization would be necessary, we performed a novel type of temporary intravascular shunting by connecting two arterial sheaths placed in the ulnar and superficial femoral arteries. This permitted the hand

Alignment of the metatarsal heads affects foot inversion/eversion during tiptoe standing on one leg in demi-pointe position: A cross-sectional study on recreational dancers.

Classical ballet dancers stand on tiptoe in the demi-pointe position where the ankle is plantarflexed, and the toes extend around a mediolateral axis passing through the second metatarsal head. Foot sickling, the foot inversion/eversion when the forefoot is grounded, should be avoided to achieve esthetics and prevent injuries during tiptoe standing. The foot inversion/eversion angle may change depending on the metatarsal heads through which the toe extension axis passes. This study investigated the relationship between metatarsal alignment in both load positions and foot inversion/eversion ang

[Penile glans amputation during non-hospital circumcision: about two cases].

Penile glans amputation during circumcision is a tragic operator-related complication. Standard treatment is based on microsurgical reimplantation with vascular and nerve anastomosis. We here report two cases of penile glans amputation in two children. The first patient was aged five years and was admitted urgently after circumcision. Penile glans was reimplanted without microsurgical anastomosis within one hour of the date of the accident. The other patient was aged 11 years and was received 3 years of the date of the accident. During these three years he had received psychological therapy. H

Symmetrical Peripheral Gangrene: Report of Three Cases.

Symmetrical peripheral gangrene (SPG) is a rare clinical syndrome characterized by an acute onset of ischemic damage in two or more extremities without obstruction or vasculitis of supplying vessels. Body parts commonly affected include toes, hands, scrotum, and earlobes, increasing the risk of limb amputation and impairing the quality of life. The vascular injury mechanism is disseminated intravascular coagulation. SPG may manifest unpredictably in conditions associated with septic shock, low output states, vasospastic conditions, myeloproliferative disorders, or hyperviscosity syndrome. A re

The factorial structure of executive functions in kindergarten children: an explorative study.

Executive functions (EF) are higher cognitive processes involved in the management of new, complex tasks with changing rules and/or the necessity to inhibit interferences. EF are central for academic success; they are necessary for planning, monitoring, and error detection. For adults, a three-factorial structure is often assumed, with the interrelated factors updating, shifting, and inhibition. Results regarding kindergarten EF are heterogeneous. Studies often do not include a direct comparison between different models. Some results support a unitary EF factor, while there is also evidence fo

Effects of Flexor Digitorum Longus Muscle Anatomical Structure on the Response to Botulinum Toxin Treatment in Patients with Post-Stroke Claw Foot Deformity.

(1) Background: The purpose of this retrospective case-control study was to determine the relationship between the control of toe movements by flexor hallucis longus (FHL) and flexor digitorum longus (FDL) muscles and the response to treatment with botulinum toxin (BoNT) in post-stroke patients with claw toe. (2) Methods: Subjects with stroke-related leg paralysis/spasticity and claw toes received multiple injections of BoNT (onabotulinumtoxin A) into the FHL or FDL muscles. We investigated the relationship between the mode of transmission of FHL and FDL muscle tension to each toe (MCT) and tr

Advanced serrated polyps as a target of screening: detection rate and positive predictive value within a fecal immunochemical test-based colorectal cancer screening population.

BACKGROUND : Advanced serrated polyps (ASPs) have a comparable risk to advanced adenomas for progression to colorectal cancer (CRC). The yield of most CRC screening programs, however, is based on advanced adenomas and CRC only. We assessed the ASP detection rate, and positive predictive value (PPV) including ASPs in a fecal immunochemical test (FIT)-based screening program. METHODS : We analyzed the findings of follow-up colonoscopies of FIT-positive screenees in the Dutch CRC screening program from 2014 until 2020. Data were retrieved from the national screening and pathology database. An ASP

Comprehensive overview of autoantibody isotype and subclass distribution.

The presence of autoreactive antibodies is a hallmark of many autoimmune diseases. The effector functions of (auto)antibodies are determined by their constant domain, which defines the antibody isotype and subclass. The most prevalent isotype in serum is IgG, which is often the only isotype used in diagnostic testing. Nevertheless, autoantibody responses can have their own unique isotype/subclass profile. Because comparing autoantibody isotype profiles may yield new insights into disease pathophysiology, here we summarize the isotype/subclass profiles of the most prominent autoantibodies. Desp

An unique unilateral tendon variation of the extensor digitorum longus muscle.

PURPOSE: The double tendon of extensor digitorum longus (EDL) muscle was observed on the left leg of a 80-year-old male cadaver. METHODS: A male cadaver 80 years old was subjected to routine anatomical dissection for research and teaching purposes. RESULTS: The main tendon of the EDL muscle started just at the end of the muscle belly. However, the accessory tendon started at the tendinous end of the muscle as a continuation of the muscle. The main and accessory tendons split into two more slips after passing through the same tunnel below the extensor retinaculum. There was also difference on

A comparative study between bone transport technique using Ilizarov/LRS fixator and induced membrane (Masquelet) technique in management of bone defects in the long bones of lower limb.

INTRODUCTION: In earlier times due to difficulty in managing segmental long bone defects, amputation was the preferred treatment. Nonunion with bone loss of long bones is a challenging problem, requiring serious attention. Post-traumatic segmental bone defects can have severe long-term ill impact on patient's lives. Reconstruction is more difficult and functional outcome is usually less satisfactory compared to bony outcome. Distraction osteogenesis and induced membrane technique are the techniques that can be used. AIMS AND OBJECTIVES: To find out and compare clinical, radiological, and func
