Functional performance and patient satisfaction comparison between a 3D printed and a standard transradial prosthesis: a case report.

BACKGROUND: The delay between amputation and prosthesis fitting contributes to the high rate of prosthetic abandonment despite advances in technology. Three-dimensional (3D) printing has allowed for the rapid fabrication of prostheses. Allowing individuals with amputations to interact with a prosthesis shortly after their procedure may reduce rejection chances. The purpose of the current investigation is to compare functional outcomes and patient satisfaction between a standard transradial prosthesis fitted in a clinic with a 3D-printed prosthesis fitted remotely. The standard prosthesis featu

Supermicrosurgical Experience pertaining to Replantation of Tamai Zone 1 Fingertip Amputations.

With the advent of supermicrosurgery, the replantation of very distal finger amputations has become possible. The choice between replantation or flap surgeries in the treatment of Tamai Zone I finger amputations continues to be a matter of debate. This study aims to evaluate replantation results in Tamai Zone 1 finger amputations. The demographic characteristics and clinical results of 39 patients undergoing replantation for a Tamai Zone 1 finger amputation between 2018 and 2021 were retrospectively analyzed. The Cold Intolerance Severity Score (CISS), two-point discrimination test (2-PD) and

Diminished coordination skills may predispose injury to lesser toe fractures-a pilot study.

INTRODUCTION: Lesser toe fractures of the foot are a common lower extremity injury. The common mechanism of injury is a direct impact of force on the toe due to a collision of the toe with an object, often related as accidental injury or clumsiness. MATERIALS AND METHODS: This is a case-control study. We compared patients with lesser toe fractures to a healthy control group. We used a motor imagery tool to evaluate the proprioception and function of the pre-motor, center of motion planning cortex. Forty images of the left/right feet in various positions were incorporated into a dedicated soft

Traumatic Dislocation of the Proximal Tibiofibular Joint: A Systematic Review and 10-year Experience From a Level 1 Trauma Center.

INTRODUCTION: Traumatic proximal tibiofibular joint dislocations occur infrequently and are typically the result of high-energy trauma. These injuries can be a marker of limb injury severity because patients often sustain vascular injury and are at high risk of amputation. The purpose of this study was to present a systematic review of traumatic proximal tibiofibular joint dislocations and compare rates of associated injuries with a retrospective series of patients at a level 1 trauma center. The secondary objective was to report rates and clinical predictors of limb amputation. METHODS: A sy

Comparison of Early versus Late Below Knee Amputation After Trauma With Standardized Prosthetic Care.

BACKGROUND: High energy, lower extremity trauma is associated with longstanding pain and functional limitations. The clinical decision to proceed with early amputation or limb salvage is often controversial. This study was designed to compare differences in complications, costs, and clinical outcomes of below knee amputation (BKA) performed early after injury or after attempted limb salvage in a hospital with standardized prosthetic care following amputation. METHODS: This is a retrospective comparative study of subjects who underwent BKA for a traumatic injury at a single level 1 trauma cent

Long-Term Outcomes After Toe-To-Thumb Transfers for Burn Reconstruction in Children.

Children are one of the most vulnerable populations to burns, and hands are frequently burned anatomical structures. Restoring function in a severely burned pediatric hand is challenging. We present our experience with pediatric toe-to-thumb transfers for burn reconstruction. A retrospective review was conducted of all pediatric toe-to-thumb transfer patients between 2009 and 2014. Children younger than the age of 18 who underwent secondary reconstruction after electrical or thermal burn injuries with at least a 5-year follow-up were included. Functional outcomes were measured with the modifie

Long-term outcomes of toe replantation: A review of ten cases.

BACKGROUND: Foot injuries due to vehicular or other accidents are common. However, complete toe amputation is rare. This study explored the current protocols and clinical significance of toe replantation. METHODS: From December 2011 to December 2018, ten patients with 13 severed toes underwent toe replantation in our hospital. Seven cases were replanted antegrade, and three cases were replanted retrograde. RESULTS: All patients were followed for two to three years after toe replantation. One big toe underwent necrosis, while the other 12 toes survived completely. The appearance and feel of t

The Role of Amputation and Myoelectric Prosthetic Fitting in Patients with Traumatic Brachial Plexus Injuries.

BACKGROUND: A cohort of patients with traumatic brachial plexus injuries (BPIs) underwent elective amputation following unsuccessful surgical reconstruction or delayed presentation. The results of amputation with and without a myoelectric prosthesis (MEP) using nonintuitive controls were compared. We sought to determine the benefits of amputation, and whether fitting with an MEP was feasible and functional. METHODS: We conducted a retrospective review of patients with BPI who underwent elective upper-extremity amputation at a single institution. Medical records were reviewed for demographics,

[Management of the partial avulsions of the auricle].

Injuries of the auricle can range from simple lacerations to complete avulsions. Many techniques of ear replantation have been described in the literature in addition to the type and extent of the involved auricular structures. A direct reattachment of the amputated pinna without microsurgery is rarely successful due to necrosis of the avulsed fragment. Whereas, reconstructions with pocket methods and their variations might lead to better results. In this article we would like to discuss some of these approaches and demonstrate a two-stage reconstruction technique for subtotal avulsion of the

Medical and Biomechanical Risk Factors for Incident Bone Stress Injury in Collegiate Runners: Can Plantar Pressure Predict Injury?

BACKGROUND: Bone stress injury (BSI) is a common reason for missed practices and competitions in elite track and field runners. HYPOTHESIS: It was hypothesized that, after accounting for medical risk factors, higher plantar loading during running, walking, and athletic movements would predict the risk of future BSI in elite collegiate runners. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: A total of 39 elite collegiate runners (24 male, 15 female) were evaluated during the 2014-2015 academic year to determine the degree to which plantar pressure data and medical history (includ

Foot arch rigidity in walking: In vivo evidence for the contribution of metatarsophalangeal joint dorsiflexion.

Human foot rigidity is thought to provide a more effective lever with which to push against the ground. Tension of the plantar aponeurosis (PA) with increased metatarsophalangeal (MTP) joint dorsiflexion (i.e., the windlass mechanism) has been credited with providing some of this rigidity. However, there is growing debate on whether MTP joint dorsiflexion indeed increases arch rigidity. Further, the arch can be made more rigid independent of additional MTP joint dorsiflexion (e.g., when walking with added mass). The purpose of the present study was therefore to compare the influence of increas

Right Foot Trans Metatarsal Amputation Following COVID-19 Infection.

A 60-year-old male patient with a prior coronavirus disease 2019 (COVID-19) pneumonia diagnosis presented with a right foot ulcer. The ulcer progressed to osteomyelitis of his right fifth metatarsal with eventual amputation and resection of the affected digit. The infection recurred two months later and spread to the right fourth metatarsal and gangrene, leading to the amputation and partial metatarsal head resection of the fourth toe. A month later, the infection recurred for a second time and a decision to perform a right trans metatarsal amputation of the foot was evaluated to avoid further

Long-term outcomes of lower limb post-traumatic osteomyelitis.

PURPOSE: Whilst recurrence and amputation rates in post-traumatic osteomyelitis (PTOM) are described, limb specific functional outcomes are not, leading to a knowledge gap when counselling patients prior to management. We aim to investigate the patient reported outcomes (PROMS) of this patient group to provide reference for discussions with patients prior to embarking on treatment. METHODS: Single institution cross-sectional retrospective study of all patients presenting with PTOM of the tibia/femur over a 7-year period. Alongside recurrence and amputation rates, patient reported outcomes wer

Evaluation of the activities of the intrinsic and extrinsic muscles of the foot during toe flexion with or without interphalangeal joint flexion using ultrasound shear wave elastography.

The intrinsic muscles of the foot are important to maintain the arch of the foot and to participate in sports activities. Using ultrasound shear wave elastography, we investigated the effect of different toe flexion methods on the activity of the intrinsic and extrinsic muscles of the foot. The study included 15 healthy adults who performed toe flexion under 2 conditions: with interphalangeal (IP) joint flexion and without IP joint flexion. The applied load during flexion was 500 g. Muscle stiffness was measured in the abductor hallucis, flexor hallucis brevis, flexor digitorum brevis, quadrat
