Re-amputation and survival following toe amputation: outcome data from a tertiary referral centre.

PURPOSE: Toe amputation is a commonly performed procedure for irreversible foot sepsis. However, outcome and predictors of outcome are poorly understood. Our aim was to determine survival and rate of progression to further amputation following index toe amputation. METHODS: Consecutive patients between 2010 and 2015 were included. Progression to further minor amputation, major amputation or death was recorded. Multivariable Cox regression analyses were undertaken to determine independent predictors of outcome and survival. RESULTS: One hundred forty-six patients were included, with mean age

In vivo operating lengths of the gastrocnemius muscle during gait in children who idiopathically toe-walk.

NEW FINDINGS: What is the central question of this study? What are the in vivo operating lengths of the gastrocnemius muscle in children who idiopathically toe-walk? What is the main finding and its importance? Children who idiopathically toe-walk operate at more plantarflexed positions but at longer fascicle lengths than typically developing children during gait. However, these ranges utilised during gait correspond to where children who idiopathically toe-walk are optimally strong. This should be considered when prescribing clinical treatments to restore typical gait. ABSTRACT: Children who

Thalamic deep brain stimulation for post-traumatic neuropathic limb pain: Efficacy at five years' follow-up and effective volume of activated brain tissue.

Chronic neuropathic pain affects 7%-10% of the population. Deep brain stimulation (DBS) has shown variable but promising results in its treatment. This study prospectively assessed the long-term effectiveness of DBS in a series of patients with chronic neuropathic pain, correlating clinical results with neuroimaging. Sixteen patients received 5 years' post-surgical follow-up in a single center. Six had phantom limb pain after amputation and 10 had deafferentation pain after traumatic brachial plexus injury. Patient-reported outcome measures were completed before and after surgery, using VAS, U

What constrains adaptive radiation? Documentation and explanation of under-evolved morphologies in Anolis lizards.

Adaptive radiations fill ecological and morphological space during evolutionary diversification. Why do some trait combinations evolve during such radiations, whereas others do not? 'Required' constraints of pleiotropy and developmental interaction frequently are implicated in explanations for such patterns, but selective forces also may discourage particular trait combinations. Here, we use a dataset of 351 species to demonstrate the dearth of some theoretically plausible trait combinations of limb, toe and tail length in Anolis lizards. For example, disproportionately few Anolis species disp

[Exosomes derived from PMN-MDSCs preconditioned by hypoxia attenuate arthropathy of collagen-induced arthritis mice].

Objective To investigate the role of the exosomes (EX) derived from polymorphonuclear myeloid-derived suppressor cells (PMN-MDSCs) preconditioned by hypoxia in the treatment of the collagen-induced arthritis (CIA) mouse model. Methods CIA mouse model was induced by bovine type II collagen(Col2) and Freund's adjuvant. PMN-MDSCs were isolated from CIA mouse spleen by magnetic beads. PMN-MDSC-derived exosomes (PMN-MDSC-EXs) were extracted from the supernatant of PMN-MDSCs under normal (210 mL/L O2) and hypoxia (10 mL/L O2) conditions. PMN-MDSC-EXs were identified by transmission electron microsco

Comparative outcome analysis of osseointegrated reconstruction and replantation for digital amputations.

The long-term outcomes of osseointegration for digital amputations are not well established, and it is not known whether osseointegration can achieve similar function and patient satisfaction to conventional surgical options such as replantation and microsurgical toe transfer. We compared the long-term outcomes after digital osseointegration and replantation. Six patients treated by osseointegration and seven patients treated by replantation were included, with median follow-ups of 8 years and 4.6 years, respectively. Outcomes were assessed using the Michigan Hand Outcomes Questionnaire, grip

COVID-19-Induced Left Sciatic Neuropathy Requiring Prolonged Physical Medicine and Rehabilitation.

A growing number of case reports and series have described a wide spectrum of neurological manifestations of COVID-19 disease including encephalopathy, cerebrovascular disease, and Guillain-Barre syndrome (GBS). However, peripheral neuropathy associated with COVID-19 disease has been uncommonly reported. Here, we describe a young patient with a COVID-19 infection who developed unilateral sciatic neuropathy during the course of treatment requiring prolonged physical medicine and rehabilitation stay. She was treated in the intensive care unit (ICU) for hypoxic respiratory failure for 22 days tot

Digit melanomas are associated with poor prognostic factors and unfavorable survivals.

BACKGROUND: The anatomic location of primary melanoma is significantly associated with outcome. OBJECTIVE: To evaluate the prognostic factors and survival outcomes associated with melanomas on hand and foot digits. METHODS: The data of 106 patients with digit melanomas were analyzed retrospectively. RESULTS: Median age of patients was 55 years, and male-to-female ratio was one. Majority of the patients had skin melanomas (74.5%); and 25.5% of them had ungual melanomas. The lesions slightly more frequently affected toes (53.8%) and thumbs (55.9%); and 57.4% of the lesions were right-sided. A

Fingertip injury epidemiology: an Indian perspective.

Fingertip injuries result in significant morbidity. It is associated with pain, loss of work and recreational hours, financial burden, and distortion of body image. Although it is a common injury, the epidemiology of fingertip injury is a subject with scanty literature. This study is aimed at providing epidemiology of fingertip injury in India. Epidemiological data were collected and analysed through a retrospective review of all fingertip injuries over a period of four years treated in our department. In the paediatric population, among the 241 injuries in 221children, most occurred at home (

Ulnar Parametacarpal Flap for Small Finger Avulsion Injury.

In difficult cases of replantation following small finger avulsion injury, in which amputation occurs at the proximal interphalangeal joint, the ulnar parametacarpal island flap, rotated 180° (propeller flap), can be used as an alternative method for covering a skin defect of the proximal phalanx. This flap can prevent metacarpophalangeal joint dysfunction and additional finger shortening. We propose the use of an ulnar parametacarpal flap for this purpose and report the outcomes of two successful cases treated with this method and followed up for 12 months.

Femoral condylar fractures in four continental giant breed rabbits.

Four male neutered continental giant rabbits aged between 10 and 30 months were presented with femoral condylar fractures, which developed without an observed traumatic injury. Stabilisation of the condylar fracture was achieved with screw fixation in all cases, which was supplemented with additional fixation in three cases. Complications consequent to the surgical intervention occurred in two cases: a femoral fracture and loss of fixation. Three rabbits were reported to have recovered normal limb function, and the rabbit that developed a femoral fracture as a consequence of its surgical inter

Going toe-to-toe with a rare case of a complex regional pain syndrome limited to the hallux.

We report the first case of a complex regional pain syndrome (CRPS) limited to the hallux using the Budapest criteria. Limited forms of CRPS are scarce in the literature and probably overlooked. There is currently no consensus to define these forms. Due to the particular metameric topography, common to the hand and the foot, we suggest the term 'metameric' CRPS to describe them. A uniform nomenclature would promote future research to study its prevalence and specific treatment in more detail.

Congenital diplopodia-A rare case of duplicated lower limb: A case report.

INTRODUCTION AND IMPORTANCE: Diplopodia is an extremely rare case in medical history, with an even fewer cases being reported in literature. We intended to enrich the literature about diplopodia with our own case report. CASE PRESENTATION: We present a case about A boy, aged one year and four months old brought by his mother to the hospital with a chief complaint of a duplicated foot in his right lower leg. Physical examination demonstrated a normal left lower extremity and a relatively well-developed duplicate foot emanating from the posterior-lateral aspect of the mid-lower right leg. CLIN

The Head-Toes-Knees-Shoulders Revised: Links to Academic Outcomes and Measures of EF in Young Children.

The measurement of self-regulation in young children has been a topic of great interest as researchers and practitioners work to help ensure that children have the skills they need to succeed as they start school. The present study examined how a revised version of a commonly used measure of behavioral self-regulation, the Head-Toes-Knees-Shoulders task (HTKS) called the HTKS-R, and measures of executive function (EF) was related to academic outcomes between preschool and kindergarten (ages 4-6years) in a diverse sample of children from families with low income participating in Head Start in t

Purpura fulminans following hump-nosed pit viper (Hypnale hypnale) envenoming: A rare complication of snakebites.

Hump-nosed pit vipers of the genus Hypnale are highly venomous and reputed for the commonest venomous snakebites in Sri Lanka. They frequently cause local manifestations and less commonly cause systemic effects such as acute kidney injury and coagulopathy. There is no antivenom currently available in Sri Lanka for their envenoming. However, more and more complications of Hypnale bites are being recently described. Purpura fulminans, one of the rare complications of snakebites that we report following authentic Hypnale hypnale bite. A 58-year-old female was bitten by a hump-nosed viper and deve

Effect of textured foot orthoses on walking plantar pressure variables in children with autism spectrum disorders.

The aim of this study was to evaluate the effect of textured foot orthoses on plantar pressure variables in children with autism spectrum disorders (ASDs). Thirty boys were divided into two groups based on their health status, namely: autism spectrum disorder and healthy matched controls. Plantar pressure data were captured during stance phases of shod walking with and without textured foot orthoses. Remarkably larger peak force under the toe1 and metatarsal1 and peak pressure under the toe1 and toe2-5 regions were observed in the autism group comparing with the healthy group, while lower peak
