Single-Center Vascular Hub Experience after 7 weeks of COVID-19 Pandemic in Lombardy (Italy).

BACKGROUND: The aim of this study was to report the experience of one of the major "hubs" for vascular surgery in Lombardy, Italy, during the first 7 weeks after total lockdown due to COVID-19 pandemic. METHODS: Data from all patients treated at our Department since the decision of the regional healthcare authorities of Lombardy to centralize surgical specialties creating a hub/spoke system (March 9, 2020) were prospectively collected and compared with a retrospectively collected cohort from the same period of year 2019. Primary study end point was defined as primary clinical success. Seconda

A Simple, Versatile Device for Multiplanar Photography.

BACKGROUND: Taking pictures of protruding body parts (such as fingertips, toes, nipples, auricles, hands, and feet) from multiple directions is difficult. To solve this problem, we developed a simple and easy-to-use device, using mirrors. METHODS: The device is composed of 4 trapezoidal plane plastic mirrors firmly connected to each other. It is possible to photograph the object from 5 directions at once, including the real image of the object at the center, and 4 images that are reflected in the mirrors around the sides. RESULTS: The device allowed photographing various body parts from mult

Reconstruction of motorcycle spokes wheel injury fingertip amputations with reposition flap technique: a report of 40 cases.

PURPOSE AND BACKGROUND: Replantation of the distal fingertip amputation poses a technical challenge to the treating hand surgeon especially dealing with motorcycle spokes wheel injury causing Allen's type III and IV amputation. The reason being the pulp charred and severely crushed. Various reconstructive options have been described in the literature. We analyze the surgical option of repositioning the amputated stump with local pedicle flaps in such injuries and interpreting the results and functional outcome. METHODS: Forty patients with motorcycle spokes wheel fingertip amputations involvi

Multi-segment foot model reveals distal joint kinematic differences between habitual heel-toe walking and non-habitual toe walking.

Toe walking is observed in pathological populations including cerebral palsy, stroke, and autism spectrum disorder. To understand pathological toe walking, previous studies have analyzed non-habitual toe walking. These studies found sagittal plane deviations between heel-toe and toe walking at the hip, knee, and ankle. Further investigation is merited as toe walking may involve altered biomechanics at more distal joints, such as the midtarsal joint. The purpose of this study was to examine biomechanical differences between rearfoot strike walking (RFSW) and non-rearfoot strike walking (NRFSW)

Structural and functional motor cortex asymmetry in unilateral lower limb amputation with phantom limb pain.

OBJECTIVE: The role of motor cortex reorganization in the development and maintenance of phantom limb pain (PLP) is still unclear. This study aims to evaluate neurophysiological and structural motor cortex asymmetry in patients with PLP and its relationship with pain intensity. METHODS: Cross-sectional analysis of an ongoing randomized-controlled trial. We evaluated the motor cortex asymmetry through two techniques: i) changes in cortical excitability indexed by transcranial magnetic stimulation (motor evoked potential, paired-pulse paradigms and cortical mapping), and ii) voxel-wise grey mat

[A MULTIFACETED APPROACH FOR TRAUMATIC AURICULAR AVULSION WITH VENOUS CONGESTION].

Choice of operative technique is often a major factor in the success of replantation of traumatic avulsions of the auricle. While microsurgery is considered to produce the best aesthetic results, this approach may not be an option due to vascular damage. We present the case of a 66-year-old Caucasian male with a subtotal traumatic left auricle avulsion. Microsurgical repair was not attempted as vessels amenable to anastomosis could not be found. Instead, the auricle was replanted in a non-microsurgical manner as a composite graft using Monocryl sutures. As a preventative measure for clot forma

Lateral Versus Medial Hallux Excision in Preaxial Polydactyly of the Foot.

BACKGROUND: In preaxial polydactyly of the foot, the choice for excision of the lateral or medial hallux is not straightforward, in particular with proximal phalangeal (type IV) and metatarsal (type VI) duplication, because of anatomical characteristics. We evaluated whether medial or lateral hallux excision gives better outcomes in these duplication types, to help clinical decision making. METHODS: Children with type IV or type VI duplication (n=14, age: 4.4-17.2 years), who were operatively treated by excision of the lateral or medial hallux, were assessed for foot function using plantar pr

Comparative Analysis of Dorsal Nerve Relocation versus Dorsal Neurectomy in the Surgical Management of Morton's Neuroma.

This clinical study compares the use of dorsal nerve relocation (DNR; also known as dorsal nerve transfer) and dorsal neurectomy (DN) in the surgical management of Morton's neuroma within the surgical directorate of an NHS Hospital Trust (Princess Royal University Hospital) in the South East of England between 2002 and 2009. Approaches to the surgical management of Morton's neuroma are dependent on the views of individual surgeons, rather than empirical evidence and varied considerably, so this study was essentially all about checking whether best practice is being followed and making improvem

Toe flexor strength is associated with mobility in older adults with pronated and supinated feet but not with neutral feet.

BACKGROUND: Older adults are known to have more pronated foot posture and decreased toe flexor strength (TFS), as well as decreased mobility in daily life compared to young adults. Although foot posture is reported to be an influential factor for walking biomechanics in young adults, there is less information on this subject in older adults. Age-related reduction in TFS is shown to be associated with impairments of functional performance, but it is poorly understood whether foot posture influences the relationships between TFS and functional performances. Therefore, the present study aimed to

The Early Report of Herpetic Whitlow by Bahal-Dawlah Razi in 15th Century CE.

A herpetic whitlow is a lesion (whitlow) on a finger or thumb caused by herpes simplex virus. It is a painful infection that typically affects the fingers or thumbs. Occasionally infection occurs on the toes or on the nail cuticle. Symptoms of herpetic whitlow include tenderness, swelling and reddening of the infected finger skin, fever and swollen lymph nodes. Although, it is believed that the first recorded observations were in 1909 CE by H. G. Adamson, in the medieval period, Bah?' al-Dawlah N?rbakhsh? Razi (1501 CE) described herpetic whitlow, under the title of Dakhes in Khulasat al-Tajar

Minimally invasive surgery versus an arthroscopic procedure for the first metatarsophalangeal arthrodesis: A comparative study of the effectiveness and safety profile.

INTRODUCTION: First metatarsophalangeal (MTP) arthrodesis is a common procedure for treating degeneration in the MTP joint. Open arthrodesis is a conventional procedure that provides acceptable results but is more invasive than minimally invasive or arthroscopic arthrodesis. However, little is known about the different outcomes between minimally invasive surgery (MIS) and arthroscopic surgery (AS) for MTP arthrodesis. This study was performed to compare the outcomes of the first MTP joint preparation for arthrodesis regarding the effectiveness of articular surface preparation and safety betwee

Soft-Tissue Nail Fold Arc Resection Combined with a Shaped Dressing for Ingrown Toenails.

The objective of this study was to evaluate the effectiveness of the surgical method of arc resection of soft-tissue nail fold combined with a shaped dressing for ingrown toenails. The surgical method involved the excision of nail fold granulation tissue and partial or total nail avulsion with preservation of its matrix. This surgical approach was applied to 20 consecutive patients (age range, 11-30 years) admitted to the hospital from January 1, 2014, to December 31, 2015. For assessment of the method, the operative technique, dressing change, and postoperative wound healing were recorded by

Time of Emergence and Large Ensemble Intercomparison for Ocean Biogeochemical Trends.

Anthropogenically forced changes in ocean biogeochemistry are underway and critical for the ocean carbon sink and marine habitat. Detecting such changes in ocean biogeochemistry will require quantification of the magnitude of the change (anthropogenic signal) and the natural variability inherent to the climate system (noise). Here we use Large Ensemble (LE) experiments from four Earth system models (ESMs) with multiple emissions scenarios to estimate Time of Emergence (ToE) and partition projection uncertainty for anthropogenic signals in five biogeochemically important upper-ocean variables.

Plantar pressure distribution and wearing characteristics of three forefoot offloading shoes in healthy adult subjects.

Forefoot offloading shoes are used to reduce pressure on specific regions of the foot. Aim of the pressure reduction is to aid healing of the soft and bony tissues and prevent complications by treating foot disorders. A great variety of forefoot offloading shoes are available. In a first step to investigate the appropriate use of these footwear in orthopedic settings, we studied plantar pressure distribution and wearing characteristics of three forefoot offloading shoes namely the Mailand, OrthoWedge and Podalux in a healthy population. Twenty subjects walked in a randomized order wearing thre
