Global Overview of Acquired Upper Limb Amputation Epidemiology: A Systematic Review of Prevalence, Incidence, Level, and Etiology.

Upper limb amputation leads to significant physical and psychosocial burden; however, it remains underrepresented in global epidemiological and rehabilitation studies. This study aimed to systematically review the global epidemiological data on upper limb amputation, focusing on its prevalence, incidence, anatomical levels, and etiology. According to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, a comprehensive literature search was conducted using PubMed, Embase, Cochrane Library, and Regional Information Sharing Systems (up to May 2023). Anterior approach to posterior auricular artery in microvascular anastomosis - a new concept in replanting amputated ears - experience from four successful cases and literature review.

INTRODUCTION: Auricular amputation is rare but devastating to the patient's aesthetics and psychology. Only about 60 cases of successful microsurgical replantation were reported in the literature. There is currently no complete procedure for auricular replantation, including a recommended operating position. MATERIALS AND METHODS: Four cases of microsurgical auricular replantation from February 2012 to July 2022 were studied. Data included age, cause, duration of ischemia, patient position, sequence of vascular anastomosis, method of arterial/venous anastomosis/grafting, amount of blood trans

Introducing a novel technique for auricular replantation following subtotal amputation combining Sexton and Mladick pocket methods: a case report.

BACKGROUND: Traumatic auricular amputations present a unique surgical challenge, particularly in cases where microsurgical anastomosis is not feasible due to vessel damage or absence. Traditional reconstruction techniques often yield suboptimal aesthetic or functional results. The pocket technique, as described by Sexton and Mladick, has shown promise in such scenarios, particularly in partial auricular amputations. CASE PRESENTATION: We report a case of a 52-year-old Caucasian male who presented with traumatic amputation of the cranial third of the auricle following a dog bite. The injury wa

Time sequence of autoimmune processes in the trajectory to rheumatoid arthritis development: what do we know?

The trajectory to RA development is a multi-step process that can take a decade. Understanding which processes occur at which moment during this trajectory is crucial when designing interventions to prevent disease development. Clinically, the period before RA onset can be divided into an asymptomatic and a symptomatic risk phase. Published visual representations of RA development often display the maturation of the autoimmune response occurring in the symptomatic risk phase or relatively short before RA development, but evidence underlying this notion is limited. To obtain more detailed insig

Tuft resorption in patients with psoriatic arthritis.

OBJECTIVES: Tuft resorption (TR) is an important radiographic feature of psoriatic arthritis (PsA). We aimed to define the prevalence of TR in patients with PsA, the clinical and radiographic features associated with it, and the risk factors for its occurrence. METHODS: We included patients with PsA followed at our prospective observational cohort. We defined TR as resorptive changes in the terminal tufts of the fingers or toes. We used generalized estimating equations to characterize clinical and radiographic features cross-sectionally associated with TR. We used multivariate Cox regression

Nucleic Acid-based Therapy in Effective Management of Rheumatoid Arthritis.

Rheumatoid Arthritis (RA) is a chronic autoimmune disease that prominently affects the joints of the body, including one or more types, such as wrists, knees, fingers, and toes. This condition is characterized by specific auto-immune responses, i.e., inflammation, swelling, stiffness, and pain in the body's joints. This results in partial or complete immobility. Several treatment options are available in the pharmaceutical market, commonly immunosuppressants, anti-inflammatory drugs, corticosteroids, anti-antibody drugs, etc. However, such drug treatments provide only symptomatic relief and la

Large bone defects in the tibia secondary to Aeromonas hydrophila infection in open fractures: Report of two cases.

This paper reports on two cases of post-traumatic osteomyelitis (OM) caused by Aeromonas hydrophila in immunocompetent patients, a rare but severe condition. A. hydrophila, a gram-negative bacterium typically found in aquatic environments, is seldom reported as a cause of OM. The first case involved a 42-year-old male with a Gustilo-Anderson grade II open tibial fracture exposed to sewer water, leading to persistent infection despite initial treatment. The second case described a 38-year-old male inmate with a gunshot-induced tibial fracture managed externally, later presenting with purulent d

PMP22-Related Neuropathies: A Systematic Review.

Background. PMP22-related neuropathies comprise a spectrum of predominantly demyelinating disorders, most commonly Charcot-Marie-Tooth type 1A (CMT1A; 17p12 duplication) and hereditary neuropathy with liability to pressure palsies (HNPP; 17p12 deletion), with rarer phenotypes due to PMP22 sequence variants (CMT1E, Dejerine-Sottas syndrome [DSS]). Methods. We conducted a PRISMA-compliant systematic review (PROSPERO ID: 1139921) of PubMed and Scopus (January 2015-August 2025). Eligible studies reported genetically confirmed PMP22-related neuropathies with clinical and/or neurophysiological data.

Pre-Chemotherapy D-Dimer Levels as Predictors of Survival Outcomes in Advanced Gastric Cancer.

BACKGROUND Gastric cancer is a common malignancy of the digestive system. There are presently no efficacious indicators to evaluate its curative effect and prognosis. Increased plasma D-dimer was reported to have a very strong association with neoplasm in advanced stages and poor overall survival (OS) for some malignant tumors in morbidity. MATERIAL AND METHODS Using propensity score analysis, we examined the potential effect of pre-chemotherapy plasma D-dimer level (PDL) on OS and progression-free survival (PFS) in patients with advanced gastric cancer (AGC). We divided 134 patients with AGC

IgA2 ACPA Drives a Hyper-Inflammatory Phenotype in Macrophages via ATP Synthase and COX2.

IgA can form immune complexes (ICs) and activate myeloid cells via Fc alpha receptor-mediated signalling to secrete pro-inflammatory cytokines. It was previously described that of the two IgA subclasses (IgA1 and IgA2), IgA2 is more inflammatory than IgA1. However, the mechanisms underlying this differential pro-inflammatory potential remain poorly defined. Using anti-citrullinated protein IgA1 and IgA2 antibodies (ACPA) that are commonly found in rheumatoid arthritis (RA) patients and linked to chronic inflammation, we show here that, in macrophages, IgA2-ICs boost TLR-induced TNF and IL6 sec

The Autoantibody response in rheumatoid arthritis; what makes it unique?

Many autoimmune diseases respond well to therapies targeting B cells, emphasizing the importance of autoreactive B cells in disease induction and progression. In some cases, autoantibodies from plasma cells are the main effectors, while in others, memory B cells are thought to modulate inflammatory responses through antigen presentation or cytokine secretion. Tolerance mechanisms usually prevent the development of autoantibodies, but when these systems fail, autoimmunity and subsequent autoimmune diseases can arise. Understanding the dynamics of autoreactive B cell responses is crucial for del

Development of bispecific antibodies that drive selective targeted local complement inhibition on rheumatoid arthritis-related antigens.

OBJECTIVES: In rheumatoid arthritis (RA) autoantibodies can trigger complement activation in the joint, contributing to tissue damage. Complement inhibitory drugs have shown important benefit in other clinical conditions. However, their use is limited because these drugs act systemically, not only requiring high doses but also rendering patients more susceptible to infections. Therefore, we designed a strategy to only inhibit complement locally, while leaving the systemic complement system intact to fight infections. METHODS: We designed bispecific antibodies (bsAbs) using controlled Fab-arm

Brachioradialis tendon transfer and palmaris longus tendon graft for thumb avulsion: A case report and review of literature.

BACKGROUND: Thumb replantation following complete traumatic avulsion requires complex techniques to restore function, especially in cases of avulsion at the level of the metacarpophalangeal joint (MCP I) and avulsion of the flexor pollicis longus (FPL) at the musculotendinous junction. Possible treatments include direct tendon suture or tendon transfer, most commonly from the ring finger. To optimize function and avoid donor finger complications, we performed thumb replantation with flexion restoration using brachioradialis (BR) tendon transfer with palmaris longus (PL) tendon graft.
