Long-term results with autogenous tissue repair of traumatic extremity vascular injuries.

With extensive vascular injuries in which a vascular conduit is required, there is controversy as to whether an autogenous or prosthetic graft is preferable. The authors reviewed their experience with 91 extremity arterial injuries in which autogenous tissue was used to repair vascular injuries of the extremities. Twenty-two patients also had concomitant repair of associated venous injuries with autogenous vein grafts. Ten patients required amputations, despite patent grafts in five patients, because of severe muscle necrosis. Two patients had thrombosis of their vein grafts develop in the ear

Dual innervated index to thumb cross finger or island flap reconstruction.

Partial volar amputations of the entire thumb pulp require soft tissue coverage that provides not only padding and protection for exposed bone and tendons, but also restoration of discriminative sensation. A modification of the radial sensory innervated cross finger flap from index to thumb is described, which includes separate reinnervation of the flap by repair of the dorsal sensory branch of the index radial digital nerve to the ulnar digital nerve of thumb. The anatomical basis for this flap is discussed and two example cases presented. Sensory reinnervation is rapid and does not require c

Digital replantations distal to the distal interphalangeal joint.

Microvascular replantations of digits distal to the distal interphalangeal (DIP) joint were reviewed in 33 digits of 29 patients. Twenty-five digits were completely amputated and eight were incompletely amputated. Ten of 33 replants failed and 23 digits survived. Even without venous reconstruction, good results were obtained in Zone I. Of eight completely amputated digits in Zone 1 in which venous reconstruction was not performed, seven digits survived. Of four completely amputated digits in Zone 2 without venous reconstruction, all became necrotic. In digits followed for six months or more af

Sports injuries in women: a one-year prospective follow-up study at an outpatient sports clinic.

A one-year prospective follow-up study of all patients visiting Tampere Research Station of Sports Medicine (TRSSM) was carried out in order to determine the specific features of women's sports injuries compared to those of men. During this period 334 women (31%) and 745 men (69%) visited the station. Women were significantly younger than men and the ten most usual sports events causing the injury differed from those of men. In women acute dislocations, contusions, and fractures were significantly less common in men, while women had more frequent stress-related sports injuries. In both sexes t

Free flaps to preserve below-knee amputation stumps: long-term evaluation.

Five patients with insufficient soft-tissue coverage on below-knee amputation stumps have been treated with free-tissue transfer surgery to preserve a functional below-knee prosthetic level. The flaps employed include one latissimus dorsi myocutaneous flap, two latissimus dorsi muscle-skin graft flaps, one groin flap, and one foot-fillet flap. All five flaps survived; one patient required early venous anastomosis reexploration and revision. The patients have been followed for a mean duration of 5.5 years (range 3 to 8 years). The mean duration to first ambulation with a prosthesis was 3.6 mont

The wraparound procedure for thumb and finger reconstruction.

Nineteen thumb and finger reconstructions were performed using the wraparound procedure; there were five early arterial failures. Thirteen of the 14 successful digital reconstructions were reviewed at an average of 26 months after surgery. There were complications related to the upper limb in seven patients, several of whom had more than one problem; these included radial skin breakdown (2), malrotation (2), and infection (2). One patient underwent late deletion for chronic infection following corrective osteotomy. Time until toe donor site healing averaged 2 months. We considered that cosmesi

Preoperative and postoperative gait analyses of patients undergoing great toe-to-thumb transfer.

Microvascular transfer of the great toe is a reliable method of thumb reconstruction for many patients, but the precise deficit incurred by elective amputation of the great toe has not been fully defined. Twelve patients who had this procedure were evaluated with preoperative and postoperative gait analyses and clinical examinations. The mean age was 29.3 years, with a mean clinical follow-up of 30.3 months, and a comparative follow-up gait study at a mean of 21.1 months postoperatively. Only one patient exhibited a significant motion change postoperatively, which was a reduction in ankle plan

[Mini condylar plate osteosyntheses of the hand].

The mini condylar plate fixation system is a new method for stable internal metaphyseal bone fixation about the MP and PIP joint areas of the hand. Indications for its use are rigid stabilization of fractures, osteotomies, and bony components of composite tissue transfers. The device should be used in deference to other techniques when early active range of motion is pursued. Its best use is in the face of combined injuries, or osteotomies with tenolysis/capsulotomy. The advantages of this implant's design permit stabilization of small epiphyseal fragments with minimal interference of joint fu

[A combination of amputation of the upper arm and a plexus lesion. Case report].

A 17-year-old male sustained an amputation of his left arm and a simultaneous complete brachial plexus lesion on the same side. The median nerve was torn nearly completely out of the amputate and the ulnar nerve was avulsed of the stump. The severed extremity was replanted without primary nerve reconstruction. In a further operation three and one-half months later, a neurolysis of the left brachial plexus was performed and the proximal stump of the median nerve was connected to the distal stump of the ulnar nerve using nerve grafts. Two years post trauma the patient has regained sensibility do

Microsurgical reattachment of totally amputated ears.

At a time when microsurgical reattachment of amputated limbs is commonplace, successful reattachment of total ear amputations is rare. Avulsed vessels, small diameters (0.3 to 0.7 mm), and technical difficulties provide primary obstacles. Three clinical cases of ear reattachments are presented which encompass the clinical spectrum of an uncomplicated success, a salvage of an impending failure, and a late failure due to venous thrombosis. In all cases there was difficulty with identification of vessels, with differentiation of arteries from veins, and in the performance of venous anastomoses. V

Metatarsal osteotomy in rheumatoid arthritis.

Thirty-eight feet in 28 patients with rheumatoid forefoot deformity were operated on with a proximal valgus osteotomy of the first metatarsal bone to reduce splaying of the forefoot. Each patient also underwent additional surgical procedures for associated conditions of the forefoot. The mean follow-up period was 3.7 years. Both the entire forefoot and the medial border of the foot were substantially improved in all but 2 cases. Residual symptoms were slightly more pronounced in the anterior footpad and the lesser toes (5 cases). A proximal valgus osteotomy of the first metatarsal bone, perfor

Polydactyly.

Polydactyly is a fairly common, genetically induced deformity of the hands and feet found more often in blacks than in whites. Careful clinical and radiographic evaluation should be made prior to treatment because the complexity of the deformity ranges from a simple soft tissue appendage to a completely developed accessory ray. Treatment usually consists of surgical excision of the least functional, least developed digit. Hallux varus, angular deformities, and a splayed forefoot are common postoperative complications.

Intrinsic toe flexion deformity following correction of spastic equinovarus deformity in adults.

In the treatment of spastic equinovarus foot deformities in adults with neurologic impairment, various surgical procedures are used including the split anterior tibialis tendon transfer and tendo achilles lengthening. Release of the flexor hallucis longus and flexor digitorum longus tendons in the midfoot is routinely included with these procedures to correct or prevent toe curling. In follow-up, residual toe curling has been observed in some patients despite release of the long toe flexor tendons. This study was undertaken to investigate this problem and its consequences, treatment, and treat

Correlation of angiographic abnormalities with disease manifestations and disease severity in polyarteritis nodosa.

We evaluated angiograms of 26 patients with polyarteritis nodosa (PAN) as well as the associated clinical and laboratory features. All patients had visceral angiograms and either biopsy or angiographic proof of PAN. Severe disease was defined as central nervous system PAN, renal failure requiring dialysis, gangrene of fingers or toes, cardiomyopathy, bowel infarction, hepatic failure, or hepatic infarction. The presence of aneurysms was associated with severe hypertension, hepatitis B surface antigenemia and clinically severe disease. Angiography may therefore identify a subset of patients wit

[The elderly foot].

The aged foot suffers physiologic changes in form and structure to make it substantially more sensible for daily stress. Pathologic changes especially of the forefoot lead to stress-induced pain until final incapacity for stress. Special importance is due to a well-adapted clothing of the foot with a smooth-bedded sole of the foot. The operative treatment of forefoot diseases as there are hallux valgus, claw-toes and splayfoot leads to excellent functional results.

Thumb replantation or not?

Over a three-year period 14 consecutive cases of amputated thumbs, 10 complete and 4 incomplete, were replanted/revascularized. Eight thumbs (57%) survived. These cases were compared with a control group, in which replantation had failed or had not been attempted. The control group was chosen to be as similar as possible with respect to thumb amputation level, type of trauma, age, occupation and sex. Costs for hospital care, duration of sick leave and disability rate were calculated. Hand function was estimated as the ROM of the thumb, power of pinch and grip, two-point discrimination and abil

Myotonic dystrophy treated with selenium and vitamin E.

We have previously reported the successful treatment of a patient with myotonic dystrophy with selenium and vitamin E. This paper deals with the treatment of a further five patients with myotonic dystrophy in different stages. All five patients improved subjectively and objectively in two or more respects. All improved their grip strength according to vigorimeter measurements (Martin), two normalized their gait, another two can now sit down on their heels and stand up, one patient can now walk on his toes, one can now get up from lying on the floor without using a chair and two patients have i

[Periungual capillaroscopy of the toes. Criteria for norms].

This study was designed to determine criteria of normality of peri-ungual capillaroscopy of the toes. The equipment used was a Leitz microscope fitted with a 150 W cold light lighting system. The study involved a population of 84 healthy individuals. Intra-judge and inter-judge concordance tests were carried out, indicating results to be reliable. Capillaroscopy of the hand (reference norms) and capillaroscopy of the feet (with the subject sitting with the legs hanging) were carried out in each case. Examination of the feet was perfectly possible in only 87% of cases. Results showed that the m

Handedness, footedness and finger and toe movement-related cerebral potentials.

Sixteen right-handed and 16 left-handed subjects were compared with respect to their foot dominance and the topography of their pre-movement cerebral potentials (Bereitschaftspotential, BP). First, righthanders were usually also found to be right-footed. Lefthanders showed a similar trend in preferring their left foot. Second, the BP prior to volitional self-paced movements of fingers and toes on either side was examined. For finger movements, the BP always showed higher amplitudes over the contralateral hemisphere as compared to the ipsilateral one (contralateral preponderance of negativity,
