Restoration of sensibility to anesthetic scarred digits with free vascularized nerve grafts from the dorsum of the foot.

Five cases of segmental vascularized nerve grafts that bridge scarred beds for digital sensory nerve reconstruction where previous nonvascularized nerve grafts have failed are reported. Average follow-up in this study was 27 4/5 months. Three patients were men and two were women. Average age was 35 1/2 years. The thumb was the recipient digit in one patient; the index finger in two patients; and the long finger in two patients (primary opposing digit). Three digits had suffered amputation and two had crush lacerations. Average graft length was 6.6 mm. Pin prick, touch, and vibratory sensation

[Problems in the surgical technic used in free toe transfer].

Based on the experiences of sixteen toe-to-hand transfers (nine for thumb reconstruction, seven for finger replacement), some technical problems of the operation are discussed. On the foot the incision should be planned in such a way that the wound can be closed directly to achieve a better aesthetic result The excision of the distal third of the second metatarsal bone marrows the foot, which improves the later appearance. On the hand deeper skin defects with exposed bone are easily closed by a forearm-island flap based on the distal radial artery. The rotation of the head of the transferred m

Toe-to-hand transfer based on the medial plantar artery.

Two patients had toe-to-hand transfers based on the medial plantar artery system. The dorsalis pedis and first dorsal metatarsal arteries were absent in both cases. Although the procedure is slightly more complicated than currently described techniques for toe-to-hand transfers based on the dorsal system, the procedure was performed in both cases without complication.

Thumb reconstruction pollicisation or toe-to-hand transfers. A comparative study of functional results.

A statistical study of thumb reconstruction after traumatic amputation, based on the records of 33 pollicisations and 21 microsurgical toe-to-hand transfers was made. A precise method of evaluation of results is presented. The types of lesions are divided into 4 groups; 1: amputation of the thumb without injuries of the other fingers; 2: amputation of the thumb associated with amputations or mutilation of other fingers; 3: metacarpal hand; 4: amputation of the thumb distal to the metacarpophalangeal joint. The aim of this study was to determine the respective indications of the two methods. It

[Free transplantation of the 2d toe. Technical features, problems and solutions].

On the basis of seven transfers of the second toe and one "en bloc" transfer of the second and third toe, technical features are discussed. The use of a plate for osteosynthesis is proposed. It has been found very useful, particularly when a MP-arthrodesis is performed between the first metacarpal and the second toe. It gives stability and therefore secondary displacement and/or rotation of the toe are avoided. It enables more accurate tension to be placed on the repaired tendons. For the flexor tendons it is suggested to repair the communis longus only, whereas both extensors, longus and brev

[Free toe transfer to the forearm stump following wrist amputation--a current alternative to the Krukenberg operation].

A new and simplified technique for reconstruction of the gripping ability after total loss of the hand is introduced. Previous techniques have not gained popularity because of the rather complicated procedures trying to extend the stump with two additional toes. Poor skin in the distal end of the stump has been a limiting factor and technically transplantation of two separate toes has been a difficult procedure. The stability in such a grip may be compromised. In the author's modification only one toe is used with an island flap from the big toe. The opposing post in this grip is the stable re

Bilateral hand reconstruction: report of three cases.

Although it has been established that a single lost hand can be reconstructed by autogenous bilateral toe transplantation, the problem of how to reconstruct bilateral hand loss still remains. The authors present a novel approach to solve this intricate problem. A big toe free skin-nail flap, along with the second digital ray or second and third digital rays, with a common vascular pedicle, is taken from the donor foot and transferred to the forearm stump by microsurgical technique, thereby creating a hand with two or three digits. Either a piece of the iliac bone or an ulna block cut during th

[Growth behavior of microvein and artery grafts].

In an experimental study with young rats the authors were able to prove that arterial and venous grafts show almost normal growth. With respect to patency rate, growth and integrity of the vessel wall the results with arterial grafts were more satisfactory than those with venous grafts. Examination of thirty-four patients who had undergone replantation or transplantation in infancy or childhood, showed that after digital replantation the extremities grew almost normally and that there was no indication of lag in vessel growth. Three times a slight change in growth after major limb replantation

Syme's amputation in adults: a long-term review.

A retrospective study was undertaken in two patient populations to establish the failure rate of Syme's amputation. Failure was defined as an amputation requiring revision to a more proximal level. For traumatic lesions of the foot the failure rate was 29% and for dysvascular lesions it was 41%. The long-term functional results in 55 patients who underwent Syme's amputation for traumatic, dysvascular or congenital lesions were studied. Overall, 73% had good function. The ideal Syme's stump, where the fat pad is centred securely over the distal tibia, was noted in only 22% of patients. The auth

[Swanson silastic implant arthroplasty in pathology of the metatarsophalangeal joint of the great toe].

Fifty-one disorders of the metatarsophalangeal joint of the Great Toe have been treated by implantation of a Swanson silastic prosthesis. A finger-joint device was used in 29 cases and a Great Toe device in 22 cases. The pathological lesions were degenerative arthrosis, rheumatoid arthritis or failure of preceding procedures. The mean follow-up was 3 years. In more than 2 cases out of 3 pain was abolished. Joint movement was only slightly improved but there was a better range of dorsiflexion and an improved gait. The axis of the Great Toe was generally satisfactory and pre-existing deformity w

[Therapy of severe hand infections with local penicillin-streptomycin administration].

The primary goal of treatment of severe hand infections (finger joint infection, tenosynovitis, infections of the palm and the dorsum of the hand) is nowadays not only a matter of bringing the infection under control, but also trying to get a good functional result. To reach good results the author regards two steps in treating severe hand infections as indispensable: a) the principle of early operation (the average interval between the first examination in the hospital and surgery was only 0,5 days) and b) the local instillation of antibiotics in high concentration (penicillin and streptomyci

Somatotopic organization of the human somatosensory cortex revealed by neuromagnetic measurements.

The primary projection areas in the human somatosensory cortex activated by electrical stimulation of the digits of the hand and the ankle were localized by measuring the magnetic field outside the head contralateral to the side of stimulation. Most of the spatial variation in the amplitude of the field component normal to the scalp could be accounted for by representing each source as a single current dipole in a spherical conducting medium with solely concentric variations in electrical conductivity, although the fit of this model to the data showed some statistically significant deviations.

Genetic and environmental variation of ridge-counts on fingers, palms, soles and toes: a twin study.

A multivariate procedure for estimating heritable components from twin data was applied to ridge counts obtained from the entire dermatoglyphic system. Covariance matrices of MZ and DZ within-pair differences were used to estimate genetic correlation matrices for 20 finger ridge counts, 6 palmar interdigital counts, 20 toe counts, 4 hallucal counts, and 6 sole interdigital counts. The proportion of genetic variation was found to be greater in ridge counts of patterns than in ridge counts of interdigital areas. On digits, finger counts are more highly heritable than toe counts. Each of the derm

[Reconstruction of the loss of substance of the lower lip. The "running stairstep" technic].

Reconstruction of near the totality of the lower lip after oncological resection or traumatic amputation, is a very challenging problem. The "step technique", introduced by Johanson, allows a good repair of the amputated lip, regarding amplitude, colour and function. The design of the flaps is recalled; and the authors present two cases which demonstrate the good result obtained with this simple but reliable method.

Radical excision of the nailfold for ingrowing toenail.

A simple method of treatment for ingrowing toenail by radical excision of the affected nailfold is described. The results in a prospective series of 50 patients are presented. This procedure, first described in 1872, does not destroy the nail, which heals to give an excellent cosmetic result. The recurrence rate after 18 months was 20 per cent, which compares favourably with procedures in which the nailbed is ablated. This method is proposed as an alternative to more complex procedures, especially when a cosmetic result is desired.

Gangrene localized to the feet in diabetic patients.

A consecutive series of 189 diabetic patients with manifest gangrenous lesions localized to the feet were studied. These patients had 293 gangrenous lesions localized to 5 different areas: toes 58%, interdigital spaces 6%, dorsa of the feet 10%, metatarsal heads 10% and heels 16%. Cardiac decompensation with edema and edema from other causes were the dominant precipitating factors for the gangrenous lesions. Arterial insufficiency alone or together with other precipitating factors was seen considerably less often. Gangrenous lesions localized to the toes were especially common in patients with

Anomalous contrahentes muscles in human feet.

Anomalous muscles were found in the third layer of plantar musculature in 2 human feet. Although they occupied the approximate position of m. adductor hallucis caput obliquum , they were larger than that muscle and showed more extensive insertions. In one specimen the muscle inserted into the areas of the metatarsophalangeal joint capsules of the first and second toes. In the second specimen the insertion was into the metatarsophalangeal joint capsules of the 4 medial toes. The m. adductor hallucis caput transversum was missing in both feet. Both muscles were supplied by ramus profundus n. pla

Lateral shrinkage of the myofilament lattice in chemically skinned muscles during contraction.

A toe muscle was isolated from a hind limb of the mouse and treated with saponin to make the sarcolemma more permeable to the solutes of the bathing medium. The equatorial X-ray diffraction pattern was recorded to determine the 1,0 spacing of the hexagonal myofilament lattice. The spacing in relaxed muscle at a sarcomere length of 2.5 microns was 408 A. When the muscle was maximally activated at pCa 4.4, a steady isometric tension of 1.3 kg/cm2 was produced and the spacing decreased to 384 A. A decrease in spacing of the same magnitude was observed when a relaxed muscle went into rigor, althou

Event related brain potentials and human pain: a first objective overview.

Since 1960 systematic studies of the human scalp-conducted cerebral slow-wave response to painful stimulation have shown only amplitude augmentation of the vertex components of the somatosensory evoked potential (SEP) to be indicators of subjective perception of a noxious or aversive quality in the stimulus. The vertex potential (VP) of the SEP occurs relatively late after onset of either transient or maintained stimuli (100-400 ms depending on stimulus mode and site), is amplitude-focal at vertex on scalp, and may be asymmetrically distributed hemispherically for unilateral stimulation of the
