The etiology, prevention, and previous therapies for ingrown toenails are presented. A simple, quick, relatively painless method for treating ingrown toenails is described, in which a cotton wick saturated with iodine tincture is inserted into the affected sulcus, separating the nail plate from the skin. Granulation tissue is treated with a cotton applicator saturated with a 50 percent silver nitrate solution. This method works on more than 80 percent of the ingrown toenails seen by this dermatologist, and is well tolerated by the patient.

Detailed evaluation of sensibility and function on 10 patients was carried out on an average of 42 months after thumb reconstruction by microsurgical free tissue transfer. Sensibility based on modified two-point discrimination and ridge criteria was found to be directly related to the patient's age at time of neurosuture. Various types of the more common forms of polydactyly are discussed regarding both genetic background and surgical treatment. Two cases involving three different forms of polydactyly are presented. Surgical treatment of the condition is presented on an individual basis as to the type of polydactyly and the overall health status of the patient. Primary goals of surgery are discussed, including relief of symptoms, future shoe wear consideration, cosmesis and avoidance of growth center damage in children.

We describe a baby with external and internal anomalies of the Majewski form of the short rib-polydactyly (SRP) syndromes. Previously unreported abnormal vertebral bodies, delayed ossification of the sternum and fibulae, and a diencephalic hamartoma are noted. These abnormalities and minimal histologic abnormality at the chondro-osseous junction suggest that this syndrome may be heterogeneous or more variable than previously known.

The aim of this investigation has been to study the magnitude of the reduction of thigh muscle strength of below-knee amputees and to correlate the strength to the degree of atrophy and to the clinical function. Railyard or train-related accidents are a common cause of traumatic amputations in children. Sixty-two replantations of totally severed body parts in 25 patients were followed via serial radiography. Most of these replantations were of digits. Uncomplicated digital replantation showed a definite temporal pattern consisting initially of decreasing mineral content followed by increasing mineral content, decreasing soft-tissue swelling, and increasing callus formation. Deviations from the expected patterns were helpful in predicting complications such as infection and/or osteonecrosis.

The experience gained through the management of 43 patients with syndactyly of the toes is presented. The incidence appears to be similar to that of syndactyly of the fingers. Type 1 syndactyly, or zygodactyly, always presented itself as a cosmetic problem; its correction is occasionally indicated and the procedure used is discussed. This study is a functional analysis of 18 free tissue transfers with a 1-to 4-year follow-up including: Six free skin flaps, two neurovascular free flaps, four free fibula, one free muscle, and three toe-to-thumb transfers. This work describes a split gastrocnemius musculotendinous surgical transfer procedure that may be used to correct an equinosupinatus deformity. The procedure was used to realign the foot of a patient whose deformity was iatrogenically induced by an improperly placed gluteal injection. Even if good motor function returns in a replanted digit, if no useful sensation returns, the digit cannot be considered fully functional. In such cases sensory reconstruction with nerve grafts may be necessary, but it is difficult to determine the optimal time for such surgery.
