The reconstruction of lobster-cleft foot is no longer considered from a functional point of view only. The aesthetic appearance of the repaired foot is now considered to be equally important. Since we reported the reconstruction of five toes in 1976, we have operated on 8 patients, totaling 16 cleft foot anomalies. We have followed-up these patients from 3 years to 7 years. Monodactyly is a sign of at least 3 different types of autosomal dominant ectrodactyly. In the first type only the 1st and the 5th or only the 5th toes are present on both feet. The trait is fully expressed in all affected children of patients. No skipping of a generation has been observed. Two cases, to which a free first web space flap or a free hemipulp flap from the foot were applied for restoring sensation to the mutilated hand are reported. The free neurovascular flap from the foot has advantages in reconstruction of the mutilated hand, when a traditional neurovascular island flap for sensory recovery of the hand is not available.

The authors report a case of bilateral symmetrical macrodactyly affecting the toes in a 17 year old patient. The deformity was treated by bilateral transmetatarsal amputation. After a review of the literature, the authors recall that macrodactyly is more frequently seen in the hand than in the foot. No etiological factor such as neurofibromatosis or a genetic or teratogenic agent was found.

A comparative study of direct radiological magnification technique was made of 22 patients with primary hyperparathyroidism, 15 patients with secondary hyperparathyroidism, and 17 patients without changes in bone structure. Only regarding the summation of all radiologically visible changes in bone structure, a hyperparathyroidism can be suspected. Previous work had shown that some of the effects of catechol could be via the fusimotor system. In order to determine the extent of fusimotor involvement, recordings have been made from muscle spindle afferents in split dorsal root filaments of anaesthetised rats. The author has studied 194 cases of polydactyly in the Japanese, and found 87% of the cases to be preaxial polydactyly. Preaxial polydactyly differed from postaxial polydactyly in many points. In preaxial polydactyly, the more proximal the level of bifurcation, the more frequent was the complication by a brachymesophalangy 5. Hypoplasia of the thenar muscles, while also varying with the level of bifurcation, was more closely connected with the presence of triphalangism. In almost all cases of preaxial polydactyly, the apical ectodermal ridge may be the site of pathogenesis.

A retrospective study was carried out of 48 patients who underwent ankle arthrodesis for tramatic arthritis from January 1967 through January 1976. Following ankle fusion, the most common complaint was subtalar pain and the most frequent finding was limitation of subtalar motion (56%). Triamcinolone acetonide (TAC), a long-acting synthetic glucocorticoid, markedly reduces survival rates in chick embryos when injected in small (10--100 ng) single-injection doses on days 5 or 9 of incubation. The patient sustained accidental amputation of the entire thumb and index finger and subtotal amputation of the middle finger of her left hand. A review of 245 patients requiring replantation surgery from 1970 to 1978 revealed a 70% survival rate for complete amputation. In this group, there were 347 replanted parts, including the lower extremity. Upper-extremity survival rates alone (greater than 90%) are higher than for lower extremity replantation. Knee problems are the most common injury sustained in both male and female runners. Other problems include toe and forefoot injuries, inferior heel and arch pain, shin splints, ankle pain, calf and Achilles tendon pain, groin and hip pain, and stress fractures. The average total treatment for all injuries required 4.2 physician visits. Orthotics were used for more than 60% of the injuries, with successful results. A thorough understanding of running injuries should assist the physician in the treatment of these problems.

Description of two cases of badly injuried hands, where the grasp-function has been restored. In one case, the traumatically amputated thumb was reconstructed in the conventional way, transposing the index finger on its neurovascular bundle. The other case demonstrates the use of modern microsurgical methods in substituting a freely transplanted toe with microvascular anastomoses for four lost fingers. Resulting function was satisfactory in both cases.

Two cases are described, one of replantation of an amputated thumb, the other of late reconstruction by toe transfer. A possible reason for the good results in thumb replantation is mentioned, and the advantages of toe transfer in thumb reconstruction are discussed.
