Description of two cases of badly injuried hands, where the grasp-function has been restored. In one case, the traumatically amputated thumb was reconstructed in the conventional way, transposing the index finger on its neurovascular bundle. The other case demonstrates the use of modern microsurgical methods in substituting a freely transplanted toe with microvascular anastomoses for four lost fingers. Resulting function was satisfactory in both cases.

Two cases are described, one of replantation of an amputated thumb, the other of late reconstruction by toe transfer. A possible reason for the good results in thumb replantation is mentioned, and the advantages of toe transfer in thumb reconstruction are discussed. Suggestions are made as to the course of action to take when confronted with a patient with an amputated thumb.

Simultaneous transfer of the second toes of the right and left feet was done to reconstruct the index and middle fingers in a 23-year-old man. A two-team microsurgical procedure approach was used. Technical aspects of the procedure are described. Long-term functional results have been good, both cosmetic and functional. Only one previous case of this type was described by Ohtsuka in 1977.

The follow-up results 1 to 11 years (mean 5 years) after resection arthroplasty of the forefoot or arthrodesis of the first metatarsophalangeal (MTP) joint performed on 100 patients (179 feet) with classical or definite rheumatoid arthritis are presented and analysed. Twenty-six patients with a pauciarticular arthritis have been studied clinically, radiologically and with histocompatibility typing. An increased frequency of HLA B27 was found (p = 1.87 x 10(-12)). Low back and buttock pain, Achilles tendinitis and dactylitis of the toes were more frequent in HLA-B27 positive patients. It is suggested that histocompatibility testing may be of some value in diagnosis and in the investigation of the possible 'reactive' nature of this type of arthritis.

Anterior transfer of the long toe flexors was carried out for the treatment of spastic equinovarus foot deformity in both adults and children. Adults included those with hemiplegia subsequent to a stroke, spastic hemiplegia due to cerebral palsy and spastic spinal paraplegia. Most of the children had cerebral palsy. From January 1960 to January 1974, 180 cases of chronic osteomyelitis were treated by the same surgeon in the infection unit of Notre-Dame Hospital in Montreal. From 1968 to 1978 more than 500 patients suffering from histologically verified malignant melanoma were seen. Only in 12 cases (appr. 2%) the tumour was situated on the tip of the fingers and toes. In such cases nowadays amputation and exarticulation of the whole extremity is no longer practiced. It is sufficient only to remove the corresponding finger or toe. Additional chemotherapy should supplement surgery.

The so-called two-stage procedure has been used in the reconstructive surgery for the severely injured flexor tendon of the finger. Thirteen cases of osteoid osteoma of the phalanxes are reported: 10 fingers and 3 toes. Any phalanx may be affected, with no preference for a particular site, in the young adult. Segmentary tumefaction of the soft parts near the lesion is a more consistent sign than pain, which is often quite minimal. Radiographies demonstrate the increase in the size of the phalanx, whose matrix is condensed. All radiographic techniques must be used to determine the exact location of the nidus, that is always very small (1 to 2 mm), and whose total removal is the only guarantee of cure.

The temperature gradient between the ventral surface of the first toe and the ambient temperature was measured and compared with established hemodynamic measurements in 71 critically ill patients. 100 diabetic and 100 diabetes-free patients were mycologically examined for the presence of pathogenic fungi in their toe-webs and toe-nails. While there were clinical signs of presumed mycotic infection in 73 of the diabetic and in 66 of the non-diabetic subjects, the examination of the KOH-treated specimens revealed fungal elements in only 70 of the former and in 53 of the latter group. A replantation of the hand was performed in an 83-year-old woman. There was good survival and the early functional return was satisfactory. However, after a personal tragedy and after the physiotherapy slowed down there was rapid deterioration in her hand function. Disuse atrophy became so marked after two years that the replanted hand became a stiff organ with little function.
