[Reconstructive surgery of the thumb after traumatic amputation (author's transl)].

The authors analysed the results of two series of patients treated either by lengthening (16 cases) or digital pollicization (45 cases). The conclusions about the indications for the two techniques are as follows: When the amputation is proximal and the sole lesion, pollicization of the ring finger is preferred. In distal amputations in children, pollicization is still indicated, but in adults, lengthening, with a pedicle island-flap to give sensation is preferable. In amputations of the thumb associated with lesions of other digits, pollicization is not feasible; in these cases, the authors r

Fetal limb deformities due to amniotic constrictions (a possible consequence of preceding amniocentesis).

The nature and origin of circular constrictions and intrauterine amputations can be studied more effectively in the fetus than in the newborn. Two cases of fetal micro- and syndactyly secondary to ring constrictions are discussed with respect to morphological processes. The amniotic membranes display active chorionic proliferation in addition to amniotic rupture and passive string formation. Moreover, association between the origin of amniotic bands and preceding amniocentesis seems to be possible in one case though it is difficult to ascertain.

Arterial thrombosis in essential thrombocythemia.

The course of essential thrombocythemia has been observed in ten patients, ages 46 to 83, of whom nine were followed for a period of 4 months to 9 years. In contrast to the experience with essential thrombocythemia recorded in the literature, manifestations of arterial thrombosis were far more common than hemorrhage. In six of the ten patients, the presenting complaints were ascribable to incipient gangrene of the toes and several of these patients additionally developed occlusion of tibial and larger arteries while under our observation. All patients with incipient gangrene showed marked clin

Digital sensibility following replantation.

The amputated digit serves as an excellent model for the examination of digital nerve repair. When amputation is complete, there is no question of cross-over or anomalous innervation. The sensibility of 35 replanted digits in 29 patients was evaluated. Sensory return was related most closely to restored digital vascularity, as measured by Allen test, pulse volume flow, and Doppler. Level of amputation, mechanism of injury, and age of patient also affected ultimate sensation. All patients experienced cold intolerance and some were considerably disabled because of it. The severity of cold intole

Limb salvage by inverted Y vein grafts to below-knee arteries.

Distal Y branches to other below-knee arteries were added to standard femorotibial bypass grafts to increase runoff, more effectively perfuse the ischemic limb, and improve limb salvage rate. Y grafts were performed in 16 lower limbs in 15 patients. Flow through the graft was increased significantly with each additional Y limb. Preoperative angiograms demonstrated filling of the distal vessels. Surgical indications were gangrenous toes or ischemic ulcers in 12 patients, and 13 limbs were saved. Three patients received bypass grafts for acute ischemic changes, limbs were salvaged in two of the

Incidence, severity, and time-course of motoneurone dysfunction in myotonic dystrophy: their significance for an understanding of anticipation.

The numbers of functioning motor units and the amplitudes of the maximum evoked muscle responses have been measured in 198 muscles of 102 patients with myotonic dystrophy. Losses of units could be demonstrated in most of the extensor digitorum brevis and thenar muscles but less commonly in the hypothenar groups. A more proximal limb muscle, the soleus, was also shown to be involved frequently. Investigation of two premature infants withmyotonic dystrophy also revealed reductions of functiong units; in one infant clinical improvement was associated with increased muscle innervation. Repeated ex

Simultaneous double toe transfer for severely disabled hands.

Two cases are presented in which a severely damaged hand was reconstructed by the simultaneous transfer of two toes using microvascular techniques. A two year evaluation of the functional result is made in both cases. Conventional reconstructive methods have little place in the reconstruction of these severe disabilities.

Technique of surgical sex reassignment for micropenis and allied conditions.

Although the etiology of micropenis is diverse, individuals who have in common a penile length of 2 cm. or less at birth do not function well as men. For this reason, during the last 18 years, 10 individuals with this problem were selected for surgical sex reassignment. The technique of the surgical procedure is described. The immediate results are considered to be cosmetically satisfactory, and the long-term results, as judged by two individuals who have been observed for 18 plus years seem to indicate that surgical sex reassignment is a useful procedure in selected patients with this disorde

Replacement of damaged arteries and veins with vein grafts when replanting crushed, amputated fingers.

Two cases are presented in which multiple vein grafts were used to revascularize several digits in hands injured by severe crushes. One case had partial amputations of 4 digits, and the other had 4 complete amputations. After debriding the damaged parts of the vessels, we interposed vein grafts in both the arterial and venous circulations to the digits. Seven of 8 revascularized digits survived. In crushing injuries, it is essential to debride the vessels proximally and distally to where the intima has not been injured. To bridge the gaps, vein grafts can be used successfully where other metho

Microvascular surgery in avulsive trauma to the external ear.

Ears amputated as part of scalping injuries have a good chance of survival in favorable circumstances. Survival of subtotally amputated ears is probably enhanced materially by arterial repairs. One experience with a completely avulsed ear that was replaced with two arterial repairs and no vein repairs showed initial perfusion, but eventually failed.

Skeletal lesions in the broiler, with special reference to dyschondroplasia (osteochondrosis). Pathology, frequency and clinical significance in two strains of birds on high and low energy feed.

The material consisted of 2,950 broilers of the variety Hybro Compact of two lines. An equal number of birds of both lines were given either a high energy feed (H) or a low energy feed (L), containing all nutrients known to be required by broilers. During the first 3 weeks, H and L starter feeds were given and thereafter H and L finishing feeds. At 21, 35, and 45 days of age, the birds were weighed in groups, and feed conversion calculated. At 21 days of age, the frequency of crooked toes and swollen hocks was registered. Birds were taken from each group for necropsy at regular intervals. Both

"Sausage-like" toes (dactylitis) and HLA B27.

When "sausage-like" swelling of the toes occurs in the absence of clinical Reiter's disease or psoriasis, definite classification is hardly possible. Nine patients with isolated "sausage toes" (dactylitis) and minor involvement of other joints are described. The relationship between this syndrome and HLA B27 permits better classification and more rational treatment.

Amputations of the foot and ankle. Current status.

Doppler assessment of ankle and foot systolic pressure is a practical aid in the evaluation of neurocirculatory problems. If the ratio of ankle pressure to arm pressure is over 0.45, more than 90 per cent of operative procedures will heal. Intraoperative testing of blood supply aids in determining level of amputation. Healing results in 80 per cent of cases if bleeding occurs in the skin within three minutes after release of tourniquet. With vigorous debridement of infected soft tissue and bone, the remaining tissue will often heal in local foot operations. A high percentage of patients with n

Replantation of digits and hands: analysis of surgical techniques and functional results in 71 patients with 86 replantations.

Defining replantation as the restoration of a completely amputated part as opposed to simply restoring circulation to an incompletely severed part, the results of replantation of 86 completely amputated parts in 71 patients performed from January, 1970, to December, 1975, were studied. Twenty-eight, or 32.5 percent, were the result of sharp severances of the part; localized crushing accounted for 56, or 65.1 percent. Two were classified as degloving injuries. Twelve amputations were transmetacarpal, six were at the metacarpophalangeal joints, 14 through the proximal phalanx, 15 at the proximal

[Simple examination of spastic movement disorders of the lower limbs in the infant age].

In 334 children active dorsiflexion of the feet was tested in order to screen for spasticity of the lower limbs. The conditions were: 1. The child lying supine. 2. The head turned actively and if this is not possible passively to the side of the examined foot. 3. Passive extension of the child's hip and knee of the examined side by the examinator. 4. Stimulation of the skin at the lateral and lower part of the shin, or the dorsum pedis or the anterior part of the toes and the planta pedis to provoke active dorsiflexion of the foot. Stimulation is done by the fingers of the examinator or by a s

Subungual exostosis.

This report is intended to focus attention on the often occurring, but seldom reported, subungual exostosis. Familiarity with its clinical characteristics should enable the physician to make a preoperative diagnosis or at least include it in the differential diagnosis of any subungual growth. Establishing the practice of x-raying all subungual tumors may save the physician's ego from the trauma of a missed diagnosis. More importantly, however, it may save the patient from unnecessary and at times even unnecessarily drastic surgery.

Structure and position of receptors within scales bordering the toes of gekkonids.

In gekkonids, the scales bordering the toes or the adjacent tissue possess suberpidermal and intraepithelial receptors in addition to setae-bearing organs. The position of subepidermal lamellated corpuscles seems to be correlated with the size of the species. The larger the adult animal the more frequently is this type of receptor found laterally in the toe. This can be explained in connection with the vibration-sensitive function of lamellated receptors. Intraepithelial axon terminals were found close to the setae-bearing sensilla in one species only. They are surrounded by numerous tonofibri

Occlusion cuff for routine measurement of digital blood pressure and blood flow.

A miniaturized blood pressure cuff made of plastic material and applicable to fingers and toes is described. The cuff was compared to rubber cuffs and to bladder-free cuffs. It was found to be more reliable than the former type and much easier to use than the latter type. It is recommended for use in conjunction with a mercury-in-Silastic strain gauge for routine measurement of digital blood pressure and blood flow in patients with arterial disease.

Experimental evaluation of microsurgical techniques in small artery anastomoses.

A number of factors that influence the success of microvascular anastomosis have been studied, including adventitial stripping, choice of suture material and needles, suturing technique, and perfusion of the distal lumen. It is apparent that only minimal stripping of the adventitia is indicated in order to prevent increased necrosis of the vessel ends at the anastomosis site. The use of 10-0 monofilament nylon suture material with needles 75 microns or less in diameter achieves the best results in small vessel anastomoses. Optimal anastomosis of 1 mm. vessels requires interrupted full thicknes

Essential facets of radiological diagnosis of extremity trauma.

Trauma to the extremities is sometimes dismissed as relatively unimportant in certain radiologic circles. In many instances, radiologic responsibility is denied and relegated to the orthopedist. Although the radiologic manifestations of much trauma to the extremities is clear cut and does not require sophisticated techniques or interpretation, there are many diagnoses which require innovative techniques and a knowledge of mechanisms of injury as well as the subtle oseous and soft tissue manifestations which may be confronted radiologically. The above factors will be stressed along with the nee
