Extreme conformal prediction: Reliable intervals for high-impact events.

BACKGROUND: Prehospital (ambulance) care can reduce morbidity and mortality from trauma. Yet, there is a dearth of effective evidence-based interventions and implementation strategies. Emergency Medical Services Traumatic Shock Care (EMS-TruShoC) is a novel bundle of five core evidence-based trauma care interventions. High-Efficiency EMS Training (HEET) is an innovative training and sensitization program conducted during clinical shifts in ambulances. We assess the feasibility of implementing EMS-TruShoC using the HEET strategy, and feasibility of assessing implementation and clinical outcomes

Distal radial artery cannulation in the Hegu (LI4) region for perioperative invasive arterial blood pressure monitoring: two case reports.

BACKGROUND: Invasive arterial blood pressure monitoring is essential for perioperative hemodynamic management. While proximal radial artery cannulation is widely used due to its ease and low complication rate, it is unsuitable for patients with upper-limb trauma, wrist/forearm surgery, local soft-tissue injury, or prior vascular injury from puncture. Distal radial artery access at the Hegu (LI4) region-originally developed for cardiovascular procedures-offers a viable alternative: it lies superficially, is less affected by wrist motion, and benefits from collateral perfusion via the ulnar arte

MRI and US of Normal and Abnormal Digital Nerves.

Digital nerves provide essential sensory innervation to the fingers and toes, playing a crucial role in tactile sensation and proprioception. Injury, inflammation, or entrapment of these nerves can result in sensory deficits, paresthesia, and neuropathic pain. Imaging the digital nerves is technically challenging due to their small caliber, adjacent and similarly appearing vascular bundles, and distal location in the extremities. Accurate evaluation requires detailed anatomic knowledge and optimized protocols using high-frequency US or MRI to ensure proper nerve visualization. Common pathologi

Polydactyly and syndactyly in a Chinese family with Floating-Harbor syndrome: an expansion of the clinical phenotype.

Floating-Harbor syndrome (FLHS) is a rare neurodevelopmental and skeletal disorder caused by truncating variants in exons 33 and 34 of the SRCAP gene. It is characterized by distinctive facial features, delayed bone age, short stature, and moderate intellectual disability. While digital anomalies have been reported in approximately half of the more than 100 known cases, the phenotypic spectrum continues to expand. Here, we describe a family in which two individuals were identified with FLHS. Both the proband and her mother presented with typical manifestations, including classic facial dysmorp

ANALYSIS OF THE TYPES AND PATTERNS OF LIMB AMPUTATIONS RELATED TO DIABETIC FOOT CONDITIONS IN THE QASSIM REGION: A RETROSPECTIVE STUDY.

BACKGROUND: Diabetic foot ulcers (DFUs) are a significant complication of diabetes, frequently leading to limb amputations. This study analyzes the patterns and types of amputations associated with DFUs in the Qassim region, Saudi Arabia, addressing a gap in regional data. METHODS: A retrospective study was conducted at the Sheikh Fahad Alawidah Diabetic Foot Center, Buraidah, Qassim, Saudi Arabia, including all patients diagnosed with DFUs who underwent amputations between January 2020 and September 2024. Data on demographics, diabetes-related characteristics, and amputation details were col

Clinical outcomes of tibial cortex transverse transport versus antibiotic-loaded bone cement for Wagner grade 3-4 diabetic foot ulcers: a real-world retrospective cohort study.

BACKGROUND: Diabetic foot ulcers (DFUs) are a primary cause of non-traumatic lower-extremity amputations. Wagner grade 3-4 ulcers, which are often complicated by deep infection and impaired perfusion, present significant management challenges. Antibiotic-loaded bone cement (ALBC) establishes a localized high-concentration antimicrobial environment and facilitates dead-space management, while tibial cortex transverse transport (TTT) is designed to enhance pedal perfusion via distraction-induced angiogenesis, thereby promoting wound healing. Rather than being interchangeable procedures, ALBC and

Combined intervention: hydroxysafflor yellow A acts synergistically with aerobic exercise to alleviate cerebral ischemia-reperfusion injury via the HIF-1α/CREB pathway.

BACKGROUND: Clinically amyopathic dermatomyositis (CADM) is a rare subtype of idiopathic inflammatory myositis often linked with the presence of autoantibodies targeting melanoma differentiation-associated protein 5 (MDA5). Patients with CADM are at increased risk of developing rapidly progressing interstitial lung disease, which significantly increases both morbidity and mortality compared to other forms of inflammatory myopathies. While there is no standardized treatment regimen, current therapeutic strategies are generally focused on combination immunosuppressive therapies. Despite early di

Nutritional risk versus established malnutrition in older adults: metabolic considerations for the association with depression.

UNLABELLED: In human cells TORC1 mTOR (target of rapamycin) protein kinase complex renders heat shock transcription factor 1 (Hsf1) competent for stress activation. In such cells, as well as in yeast, the selective TORC1 inhibitor rapamycin blocks this activation in contrast to Hsp90 inhibitors which potently activate Hsf1. Potentially therefore rapamycin could prevent the Hsf1 activation that frequently compromises the efficiency of Hsp90 inhibitor cancer drugs. Little synergy was found between the effects of rapamycin and the Hsp90 inhibitor radicicol on yeast growth. However certain rapamyc

Long-term outcomes of abatacept in individuals at risk of developing rheumatoid arthritis (ALTO): a randomised, double-blind, placebo-controlled trial.

BACKGROUND: Clinical trials aimed at preventing rheumatoid arthritis in individuals at risk have had variable results. The long-term outcomes of disease interception, however, are not known. We aimed to examine the long-term effect of therapeutic intervention, with emphasis on efficacy and safety. METHODS: The Arthritis Prevention In the Preclinical Phase of Rheumatoid arthritis with Abatacept (APIPPRA) phase 2b, randomised controlled trial recruited 213 anti-citrullinated protein antibody (ACPA) positive individuals with arthralgia in 28 hospital-based early arthritis clinics in the UK and t

Atraumatic Compartment Syndrome in Patients on Extracorporeal Membrane Oxygenation: A Case Series.

INTRODUCTION: Extracorporeal membrane oxygenation (ECMO) is a lifesaving procedure used to treat cardiopulmonary failure. Atraumatic extremity compartment syndrome is a known complication of limb ischemia related to thrombi from ECMO cannulation, prolonged hypotension, and pressor use. The purpose of this study is to describe a series of patients at a single institution who developed atraumatic compartment syndrome on ECMO. METHODS: A retrospective review was done at a single academic center between January 1, 2005, and October 30, 2023. The inclusion criteria were clinical diagnosis of compa

Successful Cases of Combined Treatment for WIfI Clinical Stage 4 Chronic Limb-Threatening Ischemia Using Popliteal-to-Distal Bypass, Minor Amputation, Debridement, and Rheocarna Therapy in Hemodialysis Patients: A Case Series.

Chronic limb-threatening ischemia (CLTI) is a severe form of lower extremity artery disease. CLTI is an umbrella term for conditions in which the lower limb is threatened by ischemia, tissue defects, neural disorders, or infection. In Japan, CLTI primarily affects patients with diabetes and those on hemodialysis. The severity of CLTI is assessed using the Wound, Ischemia, and foot Infection (WIfI) classification. CLTI needs multidisciplinary therapy. Rheocarna is a novel direct hemoperfusion device and can adsorb low-density lipoprotein (LDL) cholesterol and fibrinogen, thereby improving hemor

Psychological outcomes following digital replantation and revascularization: A narrative review.

PURPOSE: Literature on digital replantation has historically prioritized primary outcomes such as viability rates, return-to-work timelines, and restoration of range of motion and grip strength. Psychological and psychosocial sequelae, including post-traumatic stress disorder (PTSD), depression, anxiety, sleep disturbance, adjustment disorder, and body image disruption, are reported inconsistently, infrequently, or not at all. This narrative review examines the scope and quality of psychological outcome reporting in the digital replantation and revascularization literature from 2015 to 2025.

Peripheral Neuropathy: A Review.

IMPORTANCE: Peripheral neuropathy, defined as damage to peripheral nerves, affects approximately 1% of adults worldwide. More than 200 causes of peripheral neuropathy exist, with symptoms ranging in severity from mild toe numbness to debilitating symptoms that can require a wheelchair. Diabetes is the most common cause of neuropathy, affecting approximately 206 million people worldwide. OBSERVATIONS: Peripheral neuropathy is typically length-dependent, which means that symptoms appear in the longest nerve axons (toes) and progress proximally over time. Peripheral neuropathy is typically symme

Penile Replantation in Severe Polytrauma: A Rare Case With Surgical Insights.

Penile amputation is a rare urological emergency that requires prompt surgical intervention to restore both anatomical integrity and functional outcomes. Most reported cases are associated with self-inflicted injury, psychiatric disorders, or sharp trauma, whereas high-energy blunt mechanisms are exceedingly uncommon. We present the case of a 29-year-old male who sustained a motor vehicle accident resulting in traumatic right upper extremity amputation and complete penile transection with testicular exposure. Initial life-saving management was performed within the first two hours after injury,

Millipede Dermatitis Mimicking Pediatric Snake Envenomation: A Case Report.

Millipede dermatitis is an uncommon clinical presentation caused by exposure to defensive secretions released by millipedes, often resulting in striking cutaneous discoloration that may mimic more serious conditions such as envenomation, infection, vasculitis, or tissue necrosis. We present the case of a previously healthy four-year-old male who developed acute black discoloration and mild pain involving the plantar surfaces of the right toes after playing barefoot outdoors. Physical examination demonstrated sharply demarcated black discoloration with surrounding erythema and mild edema while

18F-FDG PET/CT in polyarteritis nodosa with multisystem involvement: utility in defining disease extent.

Polyarteritis nodosa (PAN) is a rare systemic necrotizing vasculitis. It affects the medium-sized arteries. It attacks many parts of the body in very different ways. Therefore, it is difficult to diagnose and stage. 18F-fluorodeoxyglucose (18F-FDG) PET/CT combines anatomic and metabolic activity. This can show the complete extent of the disease in PAN very well. We report a 35-year-old man with fever, generalized myalgia, black stools and dry gangrene at the tip of his fingers. Laboratory tests revealed high C-reactive protein and high erythrocyte sedimentation rate. However, the anti-neutroph

Papillary thyroid carcinoma presenting as paraneoplastic acral vascular syndrome in a 37-year-old female: A case report.

BACKGROUND: Paraneoplastic acral vascular syndrome is a rare condition associated with various malignancies, most commonly adenocarcinomas. It manifests as digital ischemia, including Raynaud's phenomenon, acrocyanosis, and digital gangrene. Reported cases typically show improvement after treatment of the underlying cancer. We describe the first known case of paraneoplastic acral vascular syndrome secondary to papillary thyroid carcinoma (PTC), with complete resolution following thyroidectomy. The CUBED Versus ABCDE Recognition Tools in the Detection of Malignant Melanomas Arising in the Foot: An Analysis of Four Recent Case Studies in a Single NHS Community Trust.

BACKGROUND: Melanoma of the foot and nail unit carries a worse prognosis than cutaneous melanoma elsewhere because lesions are frequently hidden, misdiagnosed, and therefore diagnosed at more advanced stages. Traditional ABCDE criteria are often insufficient for foot and nail presentations; the CUBED recognition tool was developed to improve detection of atypical plantar and subungual lesions. AIM: To evaluate four consecutive cases of foot melanoma seen in a single NHS community podiatry department, compare case characteristics against ABCDE and CUBED recognition tools, and describe service
