Extradigital Glomus Tumor of the Ankle Associated With Tibial Neuropathy: A Case Report.

We report the case of a 26-year-old man with a 14-year history of severe, highly localized pain and allodynia in the right medial retromalleolar region. He had undergone multiple previous medical evaluations and had repeatedly been treated for presumed tendinopathy without meaningful improvement. Magnetic resonance imaging (MRI) of the right ankle demonstrated multiple soft-tissue lesions involving superficial and deep tissue planes around the ankle and hindfoot. Electroneuromyography revealed severe focal axonal neuropathy of the right tibial nerve and its plantar branches. Surgical explorati

The Composite Deformity Score (CDS): Development and Preliminary Assessment of a Radiographic Scoring System for Assessing Osteochondral Lesion Severity of the Talus.

BACKGROUND: Osteochondral lesions of the talus (OLTs) vary widely in severity; lower-limb biomechanical alignment may contribute to this variation, but no composite radiographic score exists to capture it. We developed and preliminarily assessed the Composite Deformity Score (CDS), a five-parameter radiographic score designed a priori to summarize multiple alignment domains, with prospective stratification by lesion location. METHODS: A prospective cohort of 80 patients with MRI-confirmed OLTs was enrolled (June 2024-June 2025; power analysis minimum n=74). CDS summed five prespecified weight

Weight-bearing computed tomography volumetric analysis for diagnosis of syndesmotic instability: A new standard of care?

Assessing the severity and clinical impact of syndesmotic injury, especially when subtle, remains among the most diagnostically challenging conditions in foot and ankle orthopaedics. The distal tibiofibular syndesmosis is a complex, three-dimensional (3D) joint critical to ankle stability, and substantial injury to its ligament complex-especially if undetected or undermanaged-can result in long-term functional impairment. Unfortunately, routine clinical and conventional imaging evaluations are often inconclusive, falling short of capturing the dynamic and subtle nature of syndesmotic instabili

Prevalence, Incidence, Severity, and Anatomical Location of Overuse Injuries in Male and Female Ballet Dancers: A Systematic Review and Meta-Analysis.

Background: Ballet dancers are deemed at a great risk of overuse injuries due to high training loads, biomechanical requirements related to technique, maturation, hypermobility, and injury history. Sex differences are known from previous studies, which have mainly focussed on all injury types particularly on the foot and ankle complex. Aims: To critically analyse the literature for differences between female and male professional and pre-professional ballet dancers in terms of prevalence, incidence, severity, and anatomical location of overuse injuries. Methods: The search for published origin

The Role of the Individual Bundles of the Deltoid Ligament in Multidirectional Stability and Articular Contact Pressure of the Ankle Joint: A Finite Element Analysis.

The deltoid ligament (DL) is the primary stabilizer of the medial ankle; however, a limited understanding of the functional roles of its various bundles hinders rational surgical decision-making. This study aims to investigate the roles of individual DL bundles in maintaining ankle stability and articular contact pressure and thus seeks to guide decisions on whether reconstruction is required for specific injuries. A validated finite element foot model was used to simulate isolated and multiple deficiencies in the DL bundle. The articular displacements, rotations, and peak talar cartilage cont

Missed Medial Swivel Type of Midtarsal Injury Presenting Late with Deformity - A Case Report.

INTRODUCTION: Midtarsal injuries, especially pure dislocations and subluxations, are commonly missed injuries, often misdiagnosed as sprains, due to the occult nature of radiographic findings. The authors present one such case of missed midtarsal and subtalar subluxation with extra-articular calcaneum fracture and 2nd metatarsal shaft fracture, which ended up in equinus, forefoot adduction, midfoot inversion, and missed due to prolonged immobilization. CASE REPORT: We present the findings in a 24-year-old active male patient, who sustained a fall from his 2 wheeler and was treated for Right E

Quantification of Retained Donor Nuclei in ECM-Based Nerve Wraps: A Histological Assessment of Decellularization and Biocompatibility.

BACKGROUND: Peripheral nerve injuries present a significant clinical challenge, often resulting in chronic pain, sensory loss, and motor dysfunction. Biologically derived nerve wraps are frequently used in surgical nerve repair to reduce adhesion, promote regeneration, and minimize fibrosis. These extracellular matrix (ECM)-based biomaterials vary widely in composition and processing, including the extent of donor decellularization. Retained donor nuclei and cellular remnants have been implicated in triggering adverse immune responses, including chronic inflammation and fibrotic encapsulation.

Diagnostic criteria and severity assessment for syndesmosis injury using magnetic resonance imaging: A systematic review.

High ankle sprains involving syndesmosis injury present challenges in both diagnosis and severity assessment. Magnetic resonance imaging is widely regarded as the preferred modality for evaluating syndesmosis injury and related structural damage. This systematic review primarily examined the diagnostic utility of magnetic resonance imaging. Secondarily, it explores grading and prognostics of syndesmosis injuries with magnetic resonance imaging and identified possible imaging parameters predictive of injury severity. A comprehensive search of MEDLINE, Embase, CINAHL Complete, and Scopus was per

Weightbearing CT Reveals No Anatomical Features Associated With Ligamentous Lisfranc Injuries: A Matched Case-Control Study.

BACKGROUND: Several radiographic and cadaveric studies have suggested that specific anatomical features of the Lisfranc joint complex, including a shallower medial mortise of the second metatarsal (M2), shorter M2 length, or pes cavus alignment, may be associated with ligamentous Lisfranc injuries. These associations have largely been derived from 2-dimensional imaging or post-injury assessments. Weightbearing computed tomography (WBCT) enables 3-dimensional evaluation of midfoot anatomy under physiologic load and allows a more robust reassessment of these proposed anatomical associations. Exercise-primed Trpv4+ progenitors sense mechanical to regulate graded transition in enthesis regeneration.

AIMS: To identify the molecular mechanisms of mechanotransduction that drive functional regeneration rather than scar formation at the tendon-bone interface, addressing the high clinical failure rates of enthesis injuries. METHODS: Single-cell RNA sequencing was performed on enthesis tissues at multiple post-injury timepoints. A mouse tendon-bone insertion injury model was established with treadmill exercise versus sedentary controls. Transient receptor potential vanilloid 4 (TRPV4)+ cells were isolated by fluorescence-activated cell sorting for functional assays. Mechanistic studies included

The effect of proximal cuff design on limb loading and mechanics during gait with carbon fiber custom dynamic orthoses, in unimpaired individuals.

BACKGROUND: Limb trauma often results in long-term disability. Carbon fiber custom dynamic orthoses (CDOs) include a proximal cuff, posterior strut, and footplate, and improve outcomes following limb trauma, but the effects of cuff design are not well understood. RESEARCH QUESTION: Does CDO cuff design influence limb mechanics, foot loading, or CDO comfort? METHODS: Fifteen unimpaired individuals completed testing without an orthosis (NoCDO) and in a CDO with four interchangeable cuffs: anterior shell with straps, patellar tendon bearing shell with ratcheting system, patellar tendon bearing

Reimagining Orthopedic Practical Examinations: The Expanding Role of Objective Structured Clinical Examination.

INTRODUCTION: Assessment of orthopedic clinical competence has traditionally relied on long-case examinations, short-case discussions, and viva voce assessments. Although these methods evaluate theoretical knowledge and bedside skills, they are often limited by examiner bias, variability in case complexity, and lack of standardization. The Objective Structured Clinical Examination (OSCE) has emerged as a structured and objective method for assessing multiple domains of orthopedic competence within a uniform examination framework. OBJECTIVE: The objective of the study is to discuss the expandi

From alignment to helical tension: A biotensegrity-based multiscale conceptual framework for foot and ankle mechanics.

Foot and ankle biomechanics have traditionally been described using alignment-based and joint-centric models. While these approaches have contributed substantially to clinical and biomechanical understanding, they may not fully explain the dynamic, three-dimensional, and force-driven nature of foot function. This conceptual review synthesizes published biomechanical, kinematic, and plantar pressure studies together with biotensegrity principles to propose an integrative framework for foot and ankle mechanics. Based on this concept, we propose a multiscale double-helix model of the foot and ank

MRI Analysis of the Axial Plane Orientation of the Achilles Tendon: Implications for Level-Specific Percutaneous Repair Trajectories.

BACKGROUND: Percutaneous Achilles tendon repair has become increasingly favored because of its lower rate of wound complications compared with open techniques. However, its success rates are limited and rely on precise tendon capture, which can be challenging because of limited intraoperative visualization. Previous clinical and cadaveric studies have reported frequent cases of incomplete or eccentric suture placement with this approach. Although not the only factor in improving the results of percutaneous Achilles repair, understanding the positional anatomy of the Achilles could be helpful i

Meta-analysis of surgical approaches to lateral ankle instability: Open Broström versus arthroscopic Broström versus lasso-loop repair.

INTRODUCTION: Anterior talofibular ligament (ATFL) injury is a common cause of chronic lateral ankle instability and is more frequent among those who are active and play sports. There are multiple techniques to repair the ATFL, including the historically gold-standard Open Bröstrom (OB) technique, the Arthroscopic Bröstrom (AB) technique, and the more novel arthroscopic Lasso-Loop (LL) technique. This meta-analysis seeks to compare outcome statistics of these techniques for ATFL repair. METHODS: A literature search was conducted on PubMed and Embase for comparison studies or randomized contro
