Salvage of a foot by free transplants from the contralateral leg in a 5-year-old.

A 5-year-old girl had a traumatic amputation of her left thigh and on the right side loss of the posterior part of the talus and parts of the posterior tibial nerve and artery. These defects were treated with free transplantation of the corresponding part of the talus from the amputated left leg and a vascularized graft from the left posterior tibial nerve and artery. 2 years later the girl was able to walk without crutches, the ankle mobility was good, and so was the sensibility in the sole of the foot.

The sural nerve in the foot and ankle: an anatomic study with clinical and surgical implications.

Operative procedures and traumatic injuries of the lateral foot and ankle place the sural nerve and its branches at risk. Anatomic studies quantifying the course of this nerve are sparse. In this study, 17 cadaver specimens were dissected to clarify its course and branching patterns. Despite multiple variations of the nerve and its branches, description of the course of a "typical" nerve trunk was possible. The nerve was found to lie in close proximity to the Achilles tendon at a level of 7 cm above the tip of the lateral malleolus. In the hindfoot, the nerve coursed 14 mm posterior and 14 mm

The relationships of skeletal injuries with trauma score, injury severity score, length of hospital stay, hospital charges, and mortality in children admitted to a regional pediatric trauma center.

Thirty-four-hundred and seventy-two children were consecutively admitted for acute traumatic injuries over a 34 month period to the Children's National Medical Center. The study comprised 805 patients who sustained 953 fractures and dislocations. The male to female ratio was 2:1. Age at the time of admission was evenly distributed over 16 years, with a mean of 8.3 years. Pedestrian accidents and falls each accounted for 34% of the musculoskeletal injuries, whereas motor vehicle accidents accounted for an additional 13%. The femur was the most commonly fractured bone, representing 22% of all fr

[Foot problems in the elderly].

Foot problems in the geriatric patient are multiple and sometimes complex. The choices in treatment are frequently restricted because of contraindications for surgery. Common foot problems are common in the elderly patient, too, but in addition there is a specific pathology of this group of patients. Inexpensive, but institutionalized routine foot care is an essential medical need of the elderly to improve the quality of life.

First metatarsophalangeal fusion in the salvage of failed hallux abducto valgus operations.

The surgical technique of fusing the first metatarsophalangeal joint with multiple Herbert screws has been described. When properly performed, it can achieve a very high fusional rate and a relatively low incidence of complications. It is particularly useful in salvaging failed hallux abducto valgus operations, such as postoperative infection, angular and rotational deformities of the first metatarsophalangeal joint, complications of distal first metatarsal osteotomies, failure of the first metatarsophalangeal joint fusion, and iatrogenic destruction of the first metatarsophalangeal joint.

Localized multiple neurilemmomas of the lower extremity.

A neurilemmoma usually presents as a solitary subcutaneous tumor adherent to a peripheral nerve. Multiple localized neurilemmomas may occur as an isolated cutaneous finding or as part of the syndrome of neurilemmomatosis. We report a case of multiple neurilemmomas appearing on an extremity after trauma. The multiplicity and localization of the tumors are rare and illustrate that neurilemmomas are not invariably solitary tumors and that their distribution can be regional. We review the previously reported cases of multiple neurilemmomas and review the characteristics that help the clinician to

Concurrent compartment syndromes of the foot and leg.

We reviewed the case summaries of eight patients who developed concurrent compartment syndromes of the foot and leg after trauma. Patients with multiple fractures of the lower extremities, particularly the tibia, femur, and foot, may develop this disorder. The obvious causes of the concurrent syndromes, concurrent fractures and generalized limb ischemia, were seen only in two patients. The communication between the compartments of the foot and leg may be an additional causative factor. Delayed fasciotomy resulted in muscle necrosis and/or tibial nerve dysfunction. Suspected cases should have t

Magnetic resonance imaging of the foot and ankle.

Magnetic resonance imaging (MRI) of the foot and ankle is useful for the diagnosis and staging as well as the surgical planning of multiple disorders: first, for osseous injury, particularly stress fractures and occult fractures, which appear like bone bruises but behave more like fractures. Then, for the Achilles tendon, MR is useful for staging the chronicity and extent of damage, as well as whether the tendon can be surgically repaired or reconstructed. For the diagnosis of posterior tibialis tendon tears and injuries, MR is an important tool in surgical planning. Often, MR is helpful for t

[Indications and incidence of spongiosa transplantation in open fractures: analysis of 470 open fractures].

From a prospective series of 651 open fractures, treated at the institution of the authors in the years 1984-1989, 124 cancellous bone grafts in 470 open long bone fractures--651 open fractures excluding amputation after Type III open fractures, excluding traumatic amputations and open fractures of the ankle joint and foot--were analyzed using approximately 155 variables per individual fracture to deduct the influencing factors for indication and frequency of bone grafts. As a result of this analysis it could be shown that bone grafting depends significantly on variables describing the bone in

Tear of the posterior tibial tendon causing asymmetric flatfoot: radiologic findings.

OBJECTIVE: The purpose of this study was to describe the preoperative radiographic appearance of the acquired asymmetric flatfoot caused by a tear of the posterior tibial tendon. These radiographic changes reflect the loss of tendon function and the development of flatfoot deformity. SUBJECTS AND METHODS: Preoperative radiographs of 30 patients (mean age, 48 years) with surgically proved complete tears of the posterior tibial tendon were evaluated. Erect anteroposterior and lateral views of the feet were obtained in all patients and were frequently supplemented with anteroposterior views of t

Isolated popliteal artery occlusion in the young.

Three young patients with an isolated popliteal artery occlusion are presented, two with severe claudication and the third with a critical ischaemic foot. The work-up of these patients leads to certain aetiologic possibilities: microtrauma, smoking and contraceptive pills. Two of our patients underwent thromboembolectomy, the third managed conservatively. The follow-up was between six months and seven years and up to now all three patients remain well.

Powered metaphyseal stapler: an effective and simplified internal fixation system used in multiple types of podiatric surgery.

The multiplicity of types of usage of the powered stapler system in podiatric surgery were explored. The stapler system reduces surgical time and trauma, especially with complex reconstructive and repair procedures. The expedited surgical procedures along with minimized surgical trauma and comfortably implanted staples improved healing rate and less postoperative pain. The important criteria and indications for the usage of powered stapler fixation were reviewed. This relatively new system offers an effective alternative for internal fixation in the foot and ankle.

A composite neuro-teno-cutaneous forearm flap in the one-stage reconstruction of a large defect of the soft tissue around the ankle.

The vascular anatomy of the radial forearm flap, incorporating the brachioradialis and palmaris tendons together with the superficial radial nerve in 20 fresh cadavers, is described. The radial artery in the cadaveric forearm was irrigated and injected with blue latex. The number and distribution of the cutaneous branches supplying the skin, brachioradialis tendon, palmaris tendon, and superficial radial nerve, were then documented in relation to the distance from the radiocarpal joint. The radial artery was found to provide adequate blood supply to the above structures. This flap has been use

Comparative biomechanical effects of the standard method of ankle taping and a taping method designed to enhance subtalar stability.

The purpose of this study was to compare the restrictive effects of the widely recognized standard method of ankle taping with those of a modified ankle taping method that incorporates an additional component, the subtalar sling. Both ankles of 30 college football players were measured for maximal passive motion before taping, immediately after taping, and after a 2 to 3 hour football practice session. Downward and inward motions of the foot within the sagittal and frontal planes were induced separately and in combination with each other to allow quantification of four ankle motions. The data

Functional evaluation of complete sciatic, peroneal, and posterior tibial nerve lesions in the rat.

Quantification of peripheral nerve regeneration in animal studies of nerve injury and repair by histologic, morphologic, and electrophysiologic parameters has been controversial because such studies may not necessarily correlate with actual nerve function. This study modifies the previously described sciatic functional index (SFI), tibial functional index (TFI), and peroneal functional index (PFI) based on multiple linear regression analysis of factors derived from measurements of walking tracks in rats with defined nerve injuries. The factors that contributed to these formulas were print-leng

Chronic peroneus brevis tendon lesions.

Changes can occur in the peroneus brevis tendon following ankle injuries or sprains. A series of 14 tendon lesions is reported in the ankles of 13 patients. The duration of symptoms ranged from 8 months to 20 years. The predominant symptom in 12 ankles was lateral pain. In 11 ankles, lateral ankle instability was treated by a reconstruction with the split peroneus brevis graft, and in one ankle, by direct repair. The defects were found during harvest of the graft. One patient had previous fractures with bony impingement and one had a chronic tear of the tibialis posterior tendon with pes planu

Metabolic bone disease presenting as multiple recurrent metatarsal fractures: a case report.

A case of adult hypophosphatasia involving a patient with recurrent bilateral metatarsal fractures is presented. This disorder, although relatively uncommon, is often associated with multiple pseudofractures and complete pathologic fractures of the lower extremity. It can be detected with routine laboratory analysis. Metabolic bone disease should be considered as a possible causative factor in patients with multiple "stress" fractures of the foot.

Mechanical considerations for the syndesmosis screw. A cadaver study.

The purpose of this study was to examine the mechanical necessity of using a syndesmosis screw to supplement rigid internal fixation of the fibula and medial malleolus in the treatment of pronation-external rotation fractures. The legs of thirty embalmed and five fresh cadavera were dissected and mounted through the tibia to a frame so that multiple radiographs could be made with a constant relationship between the specimen and the x-ray apparatus. A standardized pronation-external rotation load was applied to the foot, and widening of the syndesmosis was studied on mortise radiographs that we

Crush injuries of the foot with compartment syndrome: immediate one-stage management.

Severe crush injuries with compartment syndrome were treated in five patients by an immediate one-stage procedure. This procedure included the assessment of skin flap viability with accurate debridement of devascularized tissues. It was performed according to the split-thickness skin excision technique. Compartment pressures were measured and the fasciotomies were performed through open wounds or separate medial and lateral incisions. The medial incision was extended to release the tarsal tunnel. Fractures were reduced and internally fixed and exposed bones were covered with locally transposed
