Idiopathic inflammation and rupture of the posterior tibial tendon (PTT) has received much attention in the recent literature. In this report of the presentation of PTT dysfunction as a manifestation of seronegative inflammatory disease, we describe the clinical and laboratory features of 76 patients with inflammation and/or rupture of the PTT. Problems in the diagnosis, reduction, and maintenance of reduction of dislocations of the tarsometatarsal joints are discussed. Many different patterns may present due to the anatomy of the foot and ankle and the different mechanisms of injury. Closed reduction and maintenance of reduction may be difficult to achieve, and open reduction may be required. Fixation with multiple K wires is useful in maintaining reduction. A prospective study was performed to establish definitive guidelines for selective use of radiography in the assessment of inversion ankle injuries. Five hundred patients were included, representing 3.2% of the workload of the department during the study period. There were 379 soft tissue injuries, 56 malleolar fractures, 40 avulsion fractures, 21 fractures at the base of the fifth metatarsal, and four calcaneal fractures. In children valgus deformities of the ankle joint are usually due to neurological conditions (spina bifida, poliomyelitis, spasticity). The deformity is characterized by a shortened fibula, wedging of distal tibial epiphysis and valgus tilt of the talus. The usual cross-leg flap procedures to cover composite wounds involving exposed ankle joint or bone require awkward positioning of the extremities with resultant complications.
