Immediate reverse shoulder arthroplasty demonstrates better outcomes than salvage reverse shoulder arthroplasty for proximal humerus fractures in the elderly: a meta-analysis.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is a promising option for proximal humerus fractures (PHFs) and also used as a salvage procedure for failed treatment. Whether the timing of RSA would influence the clinical outcomes is controversial. This meta-analysis compared outcomes between immediate RSA (iRSA) and salvage RSA (sRSA) in geriatric patients with PHFs. METHODS: PubMed, Embase, Web of Science and Cochrane Library databases were searched up to 1st April 2024. Comparative studies reporting outcomes of iRSA and sRSA in geriatric patients with PHFs were included. A random-effects m

Epidemiology of Lower Leg Fractures at a Colorado Ski Resort.

Lower extremity fractures are relatively common in winter sports. This retrospective descriptive study evaluated the demographics, injury mechanisms, and environmental factors associated with lower leg fractures (below the knee, excluding tibial plateau and fibular head fractures) among patients treated at a Colorado clinic during the 2012/13 to 2016/17 ski seasons. The chart review confirmed diagnoses and assessed associated factors. A total of 346 lower leg, ankle, and/or foot fractures were identified, representing 5.4% of all visits. The average patient age was 33.1 years (range 4-74), and

Nano-Immunomodulators for Directing Macrophage Fate to Enhance Peripheral Nerve Repair: Advances, Challenges, and Translational Perspectives.

Peripheral nerve injury remains a major clinical challenge due to inefficient axonal regeneration and limited therapeutic options. As central regulators of the repair process, macrophages dynamically transition from a pro-inflammatory to a pro-regenerative phenotype, coordinating debris clearance, axonal regrowth, remyelination, and functional restoration. However, disruptions in macrophage polarization-particularly in aging or chronic injury contexts-compromise regenerative outcomes. Here, we present a comprehensive overview of nano-immunomodulatory strategies designed to precisely direct mac

Stabilising the syndesmosis in ankle fractures - It's not just about flexible or rigid.

Ankle syndesmotic injuries remain a controversial topic, with few high-level studies and conflicting evidence. Consequently, practice varies widely. We aim to provide a narrative review, based on an extensive analysis of the current literature to inform the authors' management of syndesmosis injuries, in the context of ankle fractures. Management options include indirect stabilisation of the syndesmosis through fixation of malleolar and/or avulsion fractures or, direct trans-syndesmotic stabilisation of the syndesmosis with rigid or flexible implants. The choice between these two types of devi

Zadek Osteotomy.

The Zadek osteotomy, also known as a dorsal closing wedge calcaneal osteotomy, is a surgical procedure used to treat insertional Achilles tendinopathy and Haglund's deformity when conservative management fails. This procedure aims to reduce the bony prominence at the heel and to relieve Achilles tendon impingement to improve function and decrease pain. Recent innovations in minimally invasive/percutaneous approaches have enhanced outcomes while minimizing complications. Clinical and radiographic evaluation aids in diagnosis and surgical planning. Literature consistently demonstrates improved p

Mechanical equilibrium of the ankle joint: Implications for Achilles tendon injury.

BACKGROUND: A longer Achilles tendon (AT) moment arm (MA) relative to the foot length requires less muscle force for a given plantarflexion moment, thereby lowering mechanical load on the tendon. We examined whether the AT and foot MA lengths in healthy (N = 28) differ from height and body mass matched individuals with AT tendinopathy (N = 28) and rupture (N = 29). We hypothesized that a longer AT MA has a protective effect on tendon loading with the patient groups having smaller MA compared to asymptomatic individuals. METHODS: MAs were assessed using 2-D image-based method. AT MA was define

Effects of transcranial magnetic stimulation on cortical network modulation and functional recovery in patients with chronic ankle injury following surgery: study protocol for randomised controlled trial.

INTRODUCTION: Single-modality motor rehabilitation is often insufficient to address the complex functional restoration needs of patients with chronic ankle injuries following surgery. Research indicates that non-invasive neuromodulation techniques, such as intermittent theta-burst stimulation (iTBS), may enhance motor recovery by increasing cortical excitability and facilitating neural network reorganisation. Compared with exercise therapy alone, this study aims to explore the rehabilitation efficacy of combining iTBS with exercise therapy in patients with chronic ankle injuries following surg

Optimizing antibiotic prophylaxis for type III open fractures in China: a cost-effectiveness analysis.

BACKGROUND: Surgical site infections (SSI) remain a significant concern following Type III open fractures. Recent studies have suggested that narrow-spectrum antibiotics like cefazolin (CEZ) may offer comparable efficacy to broader-spectrum combinations in preventing SSI, with potential cost benefits. However, the cost-effectiveness of different prophylactic regimens in Type III open fractures in China has not been comprehensively evaluated. METHODS: A decision-tree model was constructed for a hypothetical cohort of 1,000 adult patients with Type III open fractures in the Chinese healthcare se

A study protocol for a randomised trial of bracing after ankle fracture.

INTRODUCTION: Ankle fractures are common and debilitating injuries usually treated with immobilisation using a foot-ankle brace (walker). Emerging evidence suggests that a less restrictive ankle stirrup may accelerate recovery without increasing complications, and patients tend to prefer ankle stirrups. However, evidence supporting their non-inferiority is inconclusive. The aim of this study is to assess whether ankle stirrups are non-inferior to walkers in improving function and pain measured by the Manchester-Oxford Foot Questionnaire three months after ankle fracture. METHODS: The trial wi
