Treatment evaluation of hindfoot injuries caused by pressure activated explosive devices in the war and peace time.

Injuries caused by pressure activated explosive devices are very complex medical and social problem in Bosnia and Herzegovina. Hindfoot injuries are also a big challenge for orthopedic surgeons concerning all forthcoming physical assessment of the patients and the fact that the forefoot and midfoot remain preserved. The study included 60 patients with amputation that followed such an injury, and 60 patients treated with reconstructive procedures. The gait of the patients was assessed in the period of 4 years. In 20 patients, destroyed tissues were sent referred for pathohistologic examination,

A comparison of soft-tissue anchors in tendo achilles reattachment.

This prospective study evaluated four soft-tissue fixation modalities, used in seven different combinations, to reattach the tendo Achilles in 34 cadaveric specimens. Ultimate loads, elastic moduli, and modes of failure were evaluated by loading the specimen in a cantilevered fashion on an Instron. Mann-Whitney U tests were performed to compare the failure load data for statistical significance. Although the use of two Mitek SuperAnchors showed better load resistance than one anchor (p < .01), there was no significant improvement between using two or three anchors (one anchor 116 +/- 24 N, two

Outcome of subtalar arthrodesis after calcaneal fracture.

Between 1983 and 1995, we used subtalar arthrodesis to treat 16 consecutive patients for continued pain after an intra-articular calcaneal fracture. Average time to union was 3 months (2 to 4 months). Complications were minor in 4 patients, and major in 4 others. Length of follow-up in 14 patients was 55 months (range, 12 to 112 months). Hindfoot scores (clinical rating system of the American Orthopaedic Foot and Ankle Society) improved from 38 (range, 28 to 62) to 67 (range, 39 to 94). Results of medical outcome surveys indicate that patients had low scores in areas related to physical condit

Adjuncts to preparing wounds for closure: hyperbaric oxygen, growth factors, skin substitutes, negative pressure wound therapy (vacuum-assisted closure).

Achieving closure in a chronic wound requires provision of adequate oxygen delivery to the tissue, adequate protein and other nutritional factors, a moist environment, an appropriate inflammatory milieu, dèbridement, and correction of contributing medical diagnoses. In some patients, these conditions are achieved easily, whereas in others, greater effort is required. Adjunctive treatments, including HBO2, growth factors, skin substitutes, and negative-pressure wound therapy (e.g., V.A.C.) can provide the proper conditions for healing in appropriately selected patients.

Glabrous skin grafts for plantar defects.

The principle of reconstructing like to like has been a long-standing and useful concept for plastic surgeons. One arena in which this concept has not been put to its full use is that of reconstructing soft tissue deficits of the sole of the foot. Most commonly, plantar defects that are to be skin grafted are reconstructed with split- or full-thickness, nonglabrous skin grafts. Nonglabrous skin grafts have significant disadvantages when used for reconstruction of plantar defects. These include painful hyperkeratotic build up at the periphery of the skin grafts, craters, contractures, and tight

Soft tissue coverage of the extremely traumatized foot and ankle.

The development of microsurgery and the expansion of plastic surgery techniques have led to a significant increase of surgical options for the salvage of the lower extremity. The traditional methods still have a role, but many authors have demonstrated a superiority of free and sophisticated regional flaps. This article gives an overview of treatment algorithms and surgical options. A therapeutic goal for the surgeon is to select the appropriate procedure with respect to the patient's medical condition and rehabilitation potential, the defect, and the surgeon's technical skills to achieve dura

Débridement. The key initial first step in wound healing.

Before addressing a wound, whether it is chronic or acute, clinicians must thoroughly assess the wound and the patient. An acute wound in a patient with normal blood flow and good medical and nutritional condition should go on to heal if appropriate care is given. This means that the wound has to be débrided adequately, dressed, and closed when appropriate. Getting back to healthy tissue is the key. In chronic wounds, healing is more difficult because the etiology of the wound is harder to determine, and the measures to reverse the medical abnormalities are often complex. When these have been

Method for manual reduction of displaced intra-articular fracture of the calcaneus: technique, indications and limitations.

One hundred and two cases of displaced intra-articular fractures of the calcaneus were treated using this method for manual reduction. All but three were treated within three days of injury. The average follow-up was seven years and three months (range three to 14 years). Patients' ages ranged from 21 to 76 years. Results were assessed by clinical outcome as well as roentgenographic appearance. Successful reductions were obtained in 92 cases. In the remaining 10 cases, the reduction was felt to be unsatisfactory and either percutaneous pinning or open reduction internal fixation was performed.

Limited open repair of ruptured Achilles tendons with Bunnel-type sutures.

A new method of repairing Achilles tendons, which uses a small medial skin incision with a Bunnel-type suture, was performed on 14 patients, and the clinical results were found satisfying. In order to restore the Achilles tendon to its original length, restore tensile strength in the suture site, and minimize injury to the sural nerve, an operative procedure has been devised which uses a limited open repair with a Bunnel-type suture. We followed 14 patients who had undergone the new procedure for a mean interval of 26 months after the injury. They returned to work in six weeks and to playing s

Soft tissue coverage options for ankle wounds.

Soft tissue deficiencies of the ankle are caused by several mechanisms, such as trauma, tumor, and infection. Compounding the reconstructive problems is that soft tissue problems often present in patients who have underlying diseases such as peripheral vascular disease, diabetes or both. For example, a 65-year-old person with diabetes who smokes two packs of cigarettes per day sustains an ankle fracture. After undergoing open reduction and internal fixation of the fracture, there is subsequent wound behiscence over the patient's fibular plate. The wound edges cannot be reapproximated, and ther

[Overuse injury syndromes of the calf and foot] ].

The lower leg, foot and ankle form a functional unit of the locomotor system with an important static and dynamic function. Injury or loss of function of any muscle group of the lower leg and the foot influences normal gait or impedes standing. Increased or frequent loads on the muscle group may lead to pain syndromes known as overuse syndromes or overuse injuries. Athletes and certain professions involving standing or walking for a long time are especially susceptible to increased muscle strains. This article describes and clarifies the causes and the development of the pain syndromes in the

Functional instability of the ankle and the role of neuromuscular control: a comprehensive review.

A review of current knowledge of the clinical syndrome of functional ankle instability is presented. Recent evidence has demonstrated that the majority of patients with functional instability of the ankle do not have mechanical hypermobility of the ankle joint. Functional instability of the ankle results from a loss of neuromuscular control. Components of neuromuscular control include proprioception, muscle strength, muscle reaction time, and postural control. Proprioceptive deficits lead to a delay in peroneal reaction time, which appears to be a peripheral reflex. Proprioception and eversion

Focused rigidity casting: a prospective randomised study.

Focused rigidity casting was compared with standard casting in a randomised prospective study. Two hundred consecutive patients attending a fracture clinic were assigned to have either a standard cast consisting of synthetic or plaster of paris, or a focused rigidity cast of synthetic material. Patients were assessed using a specially developed scoring system termed the Bradford Plaster Index. In patients with fractures of the base of fifth metatarsal, focused rigidity casting proved superior to traditional techniques for ability score (p=0.0001), satisfaction score (p=0.0023), overall impairm

Surgical anatomy of the sural and superficial fibular nerves with an emphasis on the approach to the lateral malleolus.

The aim of this study was to investigate the risk and to analyse the significance of laceration of the sural and superficial fibular nerves during the surgical approach to the lateral malleolus. The sural and the superficial fibular nerves, and their branches were dissected under x 3 magnifying lenses in 68 embalmed leg-ankle-foot specimens. The specimens were measured, drawn and photographed. In 35% of specimens the superficial fibular nerve branched before piercing the crural fascia, and in all these specimens the medial dorsal cutaneous nerve of the foot was located in the anterior compartm

Surgical management of severe deformity resulting from posterior tibial tendon dysfunction.

A retrospective case study format was used to evaluate the results of surgical treatment of severe deformities due to long-standing tibialis posterior tendon dysfunction. To be included in the study, the peri-talar (talonavicular) subluxation on a weight-bearing AP X-ray should have been at least 75%. This criterion limited the field to patients with long-standing, rigid, severe deformities. Surgical correction was obtained through a two-incision approach, dorsomedial and dorsolateral. Significant shortening of the medial column and extensive removal of adaptive bone formation was necessary to
