Hyperbaric Oxygen Therapy in Surgical Wound Healing and Tissue Salvage: A Structured Narrative Review.

Surgical wound complications remain a major source of postoperative morbidity, particularly in the setting of tissue hypoxia, ischemia, and impaired perfusion. Hyperbaric oxygen therapy (HBOT) has emerged as an adjunctive modality aimed at enhancing tissue oxygen delivery and optimizing wound healing dynamics across diverse surgical contexts. A structured narrative review of the literature was conducted using PubMed/MEDLINE to identify clinical studies, systematic reviews, and translational investigations evaluating the role of HBOT in surgical wound healing and tissue salvage. The search incl

Anterior peri-iliopsoas muscle space block for analgesia in lower-limb orthopedic surgery: A randomized controlled trial.

BACKGROUND: A combined lumbar and sacral plexus block may offer unilateral analgesia for the entire lower limb. However, the lumbar plexus block presents challenges due to the plexus's deep location and proximity to vital structures. Moreover, this technique often requires repositioning the patient, which can be painful or risky, especially in cases of multiple or spinal fractures. This study evaluated a novel anterior peri-iliopsoas muscle space block using a single skin puncture for perioperative analgesia in lower limb orthopedic surgery. Perfusion index was assessed as a supportive physiol

Bleomycin-Electrosclerotherapy in the treatment of superficial slow-flow vascular malformations of head and neck.

INTRODUCTION: Recently, Bleomycin-Electrosclerotherapy (BEST) has emerged as a novel alternative in the treatment of slow-flow vascular malformations. Regarding the commonly affected head and neck area, this procedure has to be safe and controllable due to the close anatomical proximity of critical structures. The aim of this study is to evaluate the safety and clinical outcome of BEST for slow-flow vascular malformations of the head and neck region. METHODS: Twenty-five patients with symptomatic predominantly superficial SFVMs were treated by 41 BEST sessions between 11/21 and 09/24. Treatme

Autopsy-Based Analysis of Landmine Blast Fatalities Associated With Left-Wing Extremism.

Background The utilization of landmine detonations, frequently facilitated via improvised explosive devices (IEDs), continues to be a favored strategy employed by Maoist insurgents in regions of India afflicted by left-wing extremism (LWE). Personnel tasked with anti-insurgency operations face an elevated probability of incurring fatal blast-related injuries. Conducting a forensic analysis of such fatalities is crucial to establishing consistent patterns of injury, enhancing protective measures, and guiding medico-legal protocols. Methodology A retrospective analysis was conducted at the Depar

Exploring the Correlation Between Surgical Depth and Postoperative Voice Rest Duration in a Rabbit Phonomicrosurgery Model.

OBJECTIVE: The optimal duration of voice rest after phonomicrosurgery remains a subject of debate, as clinical studies on its effect on vocal fold wound healing have yielded conflicting results, and a controlled animal model for investigating voice rest is currently unavailable. This study aimed to determine the optimal postoperative voice rest duration using a controlled rabbit model. METHODS: Building on our established rabbit model for graded voice rest, we created two distinct phonomicrosurgery injury models: one involving the superficial lamina propria (SLP) and another extending to the

From Minor Knee Laceration to Multisystem Catastrophe: A Five-Year-Old Boy's Battle With Protein C Deficiency and Purpura Fulminans.

Purpura fulminans (PF) is a rapidly progressive thrombotic emergency. Trauma-induced acquired protein C deficiency is exceptionally rare in pediatric patients. This case adds to the limited literature describing PF triggered by minor traumatic injury, complicated by delayed recognition and resource limitations. A previously healthy five-year-old boy developed rapidly expanding purpura and skin necrosis after a knee laceration from flying debris in Gaza. Lesions progressed to involve the feet, groin, thighs, back, and upper arm, ultimately resulting in 14.5% total body surface area (TBSA) necro

Operative management of self-inflicted upper extremity amputations: A retrospective case series.

INTRODUCTION: Self-inflicted upper extremity amputation is a rare injury which leaves surgeons with unique operative considerations. Although psychiatric decompensation is traditionally considered a contraindication to replantation, available evidence is limited to isolated case reports and small series. Hence, we sought to describe the injury characteristics, psychiatric comorbidities, operative decision-making, and clinical outcomes of patients with self-inflicted upper extremity amputations. METHODS: A retrospective review of traumatic upper extremity amputations and replantations was perf

Distal nerve blocks at the elbow for traumatic fingertip semi-amputation repair - A case report.

Regional anesthesia has been proven useful in hand surgery after trauma, most commonly via brachial plexus blocks (BPB), with widely established benefits. Distal nerve blocks at the elbow are used much less frequently. A 15-year-old boy was admitted because of traumatic amputation of the 4th digit on his right hand. Difficult intubation was suspected. History was indicative of obstructive sleep apnea, and nil per os status was confirmed, with no apparent risk factors for regurgitation. Ultrasound-guided blocks of the median, radial, and ulnar nerves at the elbow were performed with 2% Lidocain

Emergency Free Flap Reconstruction in Traumatic Extremity Injuries: Clinical Indications and Framework.

BACKGROUND: Emergency free flap reconstruction (EFFR) involves flap coverage performed during initial operative intervention or within 24 h after traumatic injury. Although EFFR offers notable advantages, standardized criteria for its application are not well established. This manuscript proposes a treatment framework outlining three primary indications for EFFR, including the use of viable tissue from spare parts. METHODS: We retrospectively analyzed 14 patients (15 flaps) with traumatic limb-threatening injuries treated with EFFR across two academic centers from 2016 to 2024. Data collected
