Cervico-ocular coordination during neck rotation is distorted in people with whiplash-associated disorders.

People with whiplash-associated disorders (WAD) not only suffer from neck/head pain, but commonly report deficits in eye movement control. Recent work has highlighted a strong relationship between eye and neck muscle activation in pain-free subjects. It is possible that WAD may disrupt the intricate coordination between eye and neck movement. Electromyographic activity (EMG) of muscles that rotate the cervical spine to the right (left sternocleidomastoid, right obliquus capitis inferior (OI), right splenius capitis (SC) and right multifidus (MF)) was recorded in nine people with chronic WAD. C

Masson's disease in hand surgery: a clinicopathologic study of four cases.

Intravascular papillary endothelial hyperplasia (IPEH), also known as Masson's tumour, is an uncommon benign vascular lesion characterized by small multiple, endothelial-lined, papillary structures with hyaline stalks. It appears to be a reactive condition involving excessive proliferation of endothelial cells in normal blood vessels or in vascular malformations, perhaps in response to blood vessel injury or thrombosis. The lesions are small, superficial, reddish-blue nodules, usually in the head, neck or hand. Distinction from pyogenic granuloma and angiosarcoma usually requires pathological

Effect of a novel anatomically shaped endotracheal tube on intubation-related injury.

OBJECTIVES: To develop an anatomically shaped endotracheal tube (ETT) and to compare the degree of induced laryngeal injury of this ETT with that of a standard ETT using an animal model. DESIGN: Randomized controlled animal study. SUBJECTS: Eight Sus scrofa piglets (15-20 kg) randomly intubated with either a standard or a modified uncuffed ETT. INTERVENTIONS: The modified ETT was handcrafted by gluing and then trimming dry polyvinyl acetate foam circumferentially to the distal end of a standard uncuffed ETT. After intubation, the foam quickly self-expanded as it absorbed the secretions of t

Histologic characterization of human scarred vocal folds.

Vocal fold scarring remains a significant problem. Although several animal models have been developed to improve our understanding of the histopathology, the histologic features of scarred human vocal folds have rarely been reported. The present case studies aimed to define the histologic changes of scarred human vocal folds caused by cordectomy or cordotomy. Ten patients with the scarred vocal folds were involved in this study. Nine patients with early glottic cancer underwent endoscopic cordectomy, and one patient underwent superficial cordotomy for idiopathic scar. The postcordectomy or cor

Free anterolateral thigh flap for reconstruction of major craniofacial defects.

Free-tissue transfer has revolutionized skull-base surgery by expanding the ability to perform cranial base resection and by improving the quality of reconstruction. The anterolateral thigh flap has come recently into use in the field of head and neck reconstruction. Its role in craniofacial and midface reconstruction has not been specifically defined. This study involved a total of 18 patients who were treated over a 5-year period from 1998 to 2003. Seventeen patients had locally advanced head and neck cancer, requiring craniofacial resection, and one patient had a complicated gun shot wound

Local excision procedure for Warthin's tumor of the parotid gland.

The aim of this work is to establish a local excision procedure (LEP) and indications of this procedure for Warthin's tumor. Seventy-three patients (82 sides) with Warthin's tumor were studied. Point I was located 1 cm from the intertragal notch in the direction indicated by the notch. Point S was located 5 mm superior to the inferior end of the mandibular angle. The trunk of the facial nerve and the marginal mandibular branch run at the points I and S, respectively. In surgical maneuvers below the I-S line, the marginal mandibular and colli branches may exist within the surgical field, but th

Secondary end-to-end repair of extensive facial nerve defects: surgical technique and postoperative functional results.

BACKGROUND: Repair of the transected facial nerve is imperative for restoration of muscle function, including the ability to produce appropriate facial expressions. Injury might involve the main trunk and its several branches. Restoration of function presupposes meticulous repair of all injured nerve branches. METHODS: Here we report three cases of secondary tension-free end-to-end coaptation of a transected trunk and branches of the facial nerve by removal of the superficial part of the parotid gland. RESULTS: Facial tone and symmetry at rest and motion were achieved. In two patients, a sli

Glutamatergic clock output stimulates melatonin synthesis at night.

INTRODUCTION: Otomycosis is a fungus-causing superficial infection of the external auditory canal that is common in otolaryngology practice. Though it is a worldwide infection, but is more prevalent in warm and humid regions. There has been rise in the occurrence of otomycosis in the last few years because of the extensive usage of antibiotic eardrops. Other influencing reasons for the occurrence of otomycosis is swimming, an immunocompromised host (e.g.: DM, AIDs) pregnancy, post-canal wall down mastoidectomy, tympanic membrane perforation, hearing aids and self-inflicted injuries. Influence of testosterone on the electrical properties of scrotal nerves at the cutaneous and spinal levels in the male rat.

Purpose of current study was to find out the incidence of displacement of facial nerve trunk from its normal anatomical position in relation to size and site of tumour in superficial lobe of parotid gland and its surgical significance. In the literature, the importance was given to the anatomical variations in branching pattern of facial nerve but not deviation. The knowledge of this variation is very important to prevent its injuries during superficial parotidectomy. The present study included 60 patients who underwent superficial parotidectomy for benign tumour involving the superficial lobe

Burn injury secondary to air bag deployment.

The efficacy of air bags has been proven in diminishing the rate of fatalities and severity of injury in motor vehicle crashes. Unfortunately, as with any developing technology, new problems have been encountered that are directly attributable to the deployment of the air bag itself. Most air bag-related injuries are minor and, surprisingly, more than 7% are burns typically involving the upper extremity or head or neck. Fortunately, these are superficial burns that usually require only expectant therapy, but a high degree of suspicion in these circumstances is needed to make the proper diagnos

A new technique of scarless expanded forehead flap for reconstructive surgery.

The forehead flap is an ideal flap for reconstructive surgery, especially for that involving reconstruction of the face and neck. However, it is usually limited to use in nasal reconstruction, even when performed in conjunction with tissue expansion, because of the severe visible morbidity of the donor site. In this article, the author discusses his development of a new technique of forehead flap, performed in conjunction with tissue expansion, for reconstructive surgery without visible scarring at the donor site. The technique involved positioning a tissue expander in the forehead pocket unde

Parotid selective lymphadenectomy in malignant melanoma.

Malignant melanoma of the head and neck can metastasize to lymph nodes within the parotid gland. Selective lymphadenectomy is the modern method of staging regional lymph node basins in clinically localized melanoma. This procedure involves intraoperative lymphatic mapping and directed, selective removal of the first draining nodes or sentinel lymph nodes (SLNs). Historically, the assessment of parotid lymph nodes would involve a superficial parotidectomy with facial nerve dissection. Since 1993, 28 patients with localized melanoma of the head and neck have demonstrated lymphatic drainage to pa

Cervicothoracic lesions in infants and children.

Cervicothoracic lesions are not uncommon in children. All cervicothoracic lesions except superficial lesions extend from the neck to the thorax through the thoracic inlet. Evaluation of this area involves multiple imaging modalities: plain radiography, ultrasonography, nuclear medicine, computed tomography, and magnetic resonance (MR) imaging. However, MR imaging is the method of choice for assessing the full extents of cervicothoracic lesions and their relationships to neurovascular structures. Cervicothoracic lesions can be classified as congenital lesions, inflammatory lesions, benign tumor

Cervicofacial and mediastinal emphysema as the result of a dental procedure.

Cases of cervicofacial subcutaneous emphysema occurring during dental treatment often result from the use of air-water cooled dental drills during tooth extraction. A case is presented in which a compressed air syringe, used to dry the field, caused diffuse cervicofacial emphysema with retropharyngeal and mediastinal extension. The point of entry appeared to be a 4 mm superficial laceration of the buccal mucosa. Despite the size of the wound, a significant amount of air was able to enter the tissues and spread quite distantly. Though many cases of subcutaneous emphysema go unnoticed, diffuse e
