Trends in hospitalization for firearm-related injury in Finland from 1990 to 2003.

BACKGROUND: The firearm-related mortality in Finland is one of the highest in Europe. The study objective was to describe the incidence trends and nature of firearm-related injury hospitalizations in Finland between 1990 and 2003. METHODS: We included all firearm-related injury hospitalizations between 1990 and 2003. The data were obtained from the Finnish National Hospital Discharge Register. Only events with traumatic physical injuries were included. RESULTS: The overall incidence of firearm-related injury hospitalization was 5.1 (95% confidence interval [CI]: 34.5-5.7) per 100,000 person-

Managing the blue man: multiple traumas involving a paint-carrying truck.

INTRODUCTION: This report describes the difficulty in evaluating a patient with multiple traumas because he was covered with paint poured from a truck in a car accident. Cleansing with paint thinner and isotonic saline solution was necessary. CASE REPORT: A 29-year-old male patient was admitted to the emergency department following the collision of his car with a paint-carrying truck. His head, face, neck and hands were covered with a cyan-blue oil paint, and bloody "paint mud" covered all frontal and occipital areas of the scalp. Abdominal guarding was identified. A rapid cleansing with norm

Clinical results of an open prospective study of a bis-GMA composite in percutaneous vertebral augmentation.

In this open prospective trial, 53 patients with acute pain from osteoporotic vertebral fracture related to osteoporosis or malignancy underwent vertebral augmentation with a new bisphenol-a-glycidyl dimethacrylate (bis-GMA) resin (Cortoss, Orthovita, Malvern, Pa, USA). Treatment consisted of up to 8 ml of Cortoss injected into a given vertebra. The procedure encompassed single and multiple injections (including the contralateral hemivertebra, to a maximum of 3 vertebral levels). Follow-up was at 4 and 8 days and at 1, 3, and 6 months. The primary efficacy end point was patient-rated pain usin

Open reduction of pediatric atlantoaxial rotatory fixation: long-term outcome study with functional measurements.

OBJECT: Atlantoaxial rotatory fixation (AARF) is an uncommon disorder of childhood in which resolution usually occurs spontaneously or after traction therapy. In a minority of children irreducible or chronic fixation develop, and the natural history then usually involves restriction of head on neck movement, abnormal head position, and progressive facial asymmetry. The conventional management in these cases has been a posterior fusion. METHODS: The authors performed an open reduction via the extreme-lateral approach without adjunctive fixation surgery in 13 children who ranged in age from 4 t

The effects of protocol and test situation on maximal vs. submaximal cervical motion: medicolegal implications.

The objectives of this study were to examine the influence of the measurement protocol on the range and consistency of cervical motion (CM) in maximal vs. feigned limitation of CM, to explore some cognitive aspects of the feigning performance and to assess the effect of imagined pain vs. financial gain as a stimulus for the submaximal performance. The directions of flexion, extension, right and left rotation and right and left lateral flexion were measured in 26 normal subjects. Four protocols were compared: performance of CM with eyes open vs. eyes closed and testing at either a repetitive (w

Avidity progression of dietary antibodies in healthy and coeliac children.

BACKGROUND: Klebsiella pneumoniae is a Gram-negative bacterial species well known for its capacity to cause infections in humans, and to carry and spread a wide variety of resistance genes including extended-spectrum beta-lactamase genes, carbapenem resistance genes, and colistin resistance genes. Recently, our real-time laboratory-based surveillance system MARSS (the Marseille Antibiotic Resistance Surveillance System) allowed us to observe a intringing dramatic decrease in the beta-lactam resistance level of the K. pneumoniae strains routinely isolated from patients hospitalized in our setti

Management of complications of simultaneous kidney-pancreas transplantation with temporary venting jejunostomy.

BACKGROUND: Ganoderma lucidum (Lingzhi; LZ) and San-Miao-San (SMS) are Chinese medicines (CMs) used to treat inflammatory ailments and numbing syndrome/arthralgia syndrome (Bi Zheng), respectively. Given that the main symptoms of systemic lupus erythematosus (SLE) include inflammation of the joints, joint pain, edema and palpitations of the heart because of problems associated with Bi Zheng, it was envisaged that LZ and SMS could be used as potential treatments for this autoimmune disease. This study aims to investigate the anti-inflammatory activity of a combination formulation containing LZ

[Shoulder hemi-arthroplasty in proximal humeral fractures].

Most fractures of the proximal humerus with significant displacement are best treated surgically. The range of surgical treatment varies from closed reduction and pinning to hemiarthroplasty depending on the degree of displacement, age of the patient, and bone quality. Determining whether or not the individual fractured bone segments are displaced to a significant degree requires high quality x-rays which can be difficult to obtain from acutely injured patients. Indications for replacement of the humeral head in acute fractures include: head splitting fractures in elderly patients, Neer 4-part

Fractures of the proximal radial head and neck in children with emphasis on those that involve the articular cartilage.

This is a review of 116 children who had a fracture of the proximal radial head or neck over a 15-year period. Of 33 teenagers with closed physes, 17 (52%) had intraarticular involvement. Of 83 younger children with an open proximal radial physis, six (7%) had an intraarticular fracture (Salter-Harris type III or IV). Of the 17 patients with closed physes and intraarticular fracture, 13 had adequate follow-up. There were eight excellent, three good, one fair, and one poor results. Of the six children with open physes and intraarticular fracture, there were one good and five poor results. This

External laryngeal trauma.

External laryngeal trauma, blunt or penetrating, is a rare but potentially life-threatening injury. This is frequently seen in multiple-trauma patients and can go unrecognized in the absence of astute clinical awareness. Injuries may range from small endolaryngeal hematomas or lacerations to complete laryngotracheal separation. Proper airway management is of utmost importance and is one of the most controversial aspects of treatment of laryngeal trauma. Flexible fiberoptic laryngoscopy and high resolution computed tomography scanning of the larynx has greatly enhanced the evaluation of these i

Post-traumatic heterotopic ossification about the elbow.

To evaluate the effect of surgical timing on the formation of heterotopic ossification about the elbow, 71 consecutive patients with elbow trauma requiring operative management were evaluated. Fourteen patients were excluded because they suffered from head injury, burns, or severe open injuries requiring surgery on two or more occasions. Sixteen patients were lost to follow-up, leaving a group of 41 patients. The average age of patients was 35 years. The fractures involved the olecranon in 19, distal humerus in 12, and radial head/neck in 10 patients. Six of these fractures were accompanied by

Traumatic chronic TMJ dislocation: report of an unusual case and discussion of management.

Dislocation of the temporomandibular joint (TMJ) is most often spontaneous, but it can be associated with direct or indirect head and neck trauma. The disorder may be treated in general by simple closed techniques, if managed acutely. If the dislocation becomes chronic, however, open reduction is usually required. The article discusses various methods for an open surgical approach described in the literature. A case of a prolonged TMJ dislocation is presented, involving a severe bilateral mandibular dislocation with the condyles displaced into the infratemporal fossa and the lateral poles of t

Management of frontal sinus fractures with posterior table involvement: a retrospective study.

Management of frontal sinus fracture has been a matter of debate. Combined fractures of the anterior and posterior walls have been managed by various techniques, including observation, open reduction and internal fixation, ablation, obliteration, and, most recently, cranialization. The earlier techniques have met with frequent complications, including sinusitis, mucopyocele, cerebrospinal fluid leak, meningitis, and brain abscess, along with various chronic pain symptoms. Cranialization of the frontal sinus was first introduced for injuries of both walls with intracranial penetration. From 199

Aerobic and anaerobic microbiology of infections after trauma in children.

OBJECTIVE: To review the recovery of aerobic and anaerobic bacteria from infections after trauma in children over a 20 year period. METHODS: Only specimens that were studied for both aerobic and anaerobic bacteria were included in the analysis. They were collected from seven separate centres in which the microbiology laboratories only accepted specimens that were properly collected without contamination and were submitted in appropriate transport media. Anaerobes and aerobic bacteria were cultured and identified using standard techniques. Clinical records were reviewed to identify post-trauma
