From 1974 to 1989, we treated 32 patients with a femoral head fracture: 28 were associated with a traumatic posterosuperior dislocation of the hip (6 Pipkin type I, 7 type II, 4 type III and 11 type IV), 1 with a posteroinferior and 3 with an anterior displacement of the femoral head. Twenty-four patients had been involved in a traffic accident, 23 had associated injuries, and the average polytrauma score (PTS) was 21. We analyzed failures of surgical treatment of femoral neck fractures due to osteonecrosis in a consecutive series of 26 patients with an average age of 31 (range, 10 to 49 years). These patients were referred for treatment of osteonecrosis that developed after internal fixation of a fracture of the femoral neck. The average Harris hip score at the time of referral was 59. A review of 156 patients with burn injuries admitted over a 5-year period (January 1988-December 1992) at Wesley Guild Hospital, Ilesha, was undertaken. The male:female ratio was 1.6:1. About 70% of the cases were between 1 and 10 years of age (mean 9 years). Scalds were responsible for 50.6% of the cases while naked flames, including kerosene explosion and burns at road traffic accidents, were responsible for 43.5%. Eleven ipsilateral neck and shaft fractures of the femur in 11 patients (4 females, 7 males) were treated between May 1987 and December 1990 at the Department of Orthopedic Surgery. Injuries to the cervical spine at the C3-C4 level involving the bony elements, intervertebral disks, and ligamentous structures are rare. Open reduction and internal fixation was employed in the treatment of 25 severely displaced fractures and fracture-dislocations of the proximal humerus. Our aims were accurate reduction and stable fixation to allow early mobilisation and to achieve full functional recovery. As part of a transcultural investigation of violent behavior in Denmark and South America, the lesions from accidents involving deliberate violence registered in three Danish emergency wards during a 1-year period were studied. A quantity of 2211 lesions were diagnosed in 1316 patients (953 male and 363 female patients). Sixty-five percent of the lesions were in the head/neck region, 13% in truncus, 18% in the upper extremities and 5% in the lower extremities. As part of a transcultural investigation of violent behavior in Argentina and Denmark, the lesions from accidents involving deliberate violence registered in the emergency wards of Hospital Fernandez and Hospital Pinero in Capital Federal Buenos Aires over a period of 13 and 17 months, respectively, were studied. A retrospective review is presented of 72 patients who sustained frontal sinus fractures (FSF) and were subsequently treated by the department of otolaryngology/head and neck surgery between the years of 1974 and 1986. Eighty-four percent of FSF occurred in males and 71% were a result of motor vehicle accidents. A very rare diagnosis before a terminal hemorrhagic accident, aorto-esophageal fistula (FAO) is almost always fatal. Three cases of this exceptional lesion are reported. Fractures of the scapula occur mainly from direct trauma involving considerable violence and associated injuries of the shoulder and thorax are common. In most cases early functional treatment gives good or excellent results.
