Patient Satisfaction With Surgery of Basal Cell Carcinoma: Keep It Safe and Simple.

BACKGROUND: Patient-reported outcomes are important quality indicators in dermatologic surgery. OBJECTIVES: We performed a prospective cohort study to assess patient satisfaction with surgery of basal cell carcinoma (BCC) and identify patient-, tumor-, and treatment-related influencing factors. METHODS: Patients who underwent BCC surgery at the Vivantes Skin Cancer Center in Berlin, Germany, rated global, cosmetic, and functional satisfaction on a visual analog scale from 0 (very dissatisfied) to 10 (very satisfied) at suture removal (T0) and three months after surgery (T1). Group difference

Oral Niacin and Topical Nitroglycerin to Prevent Nicotine-Related Complications Following Facelift Surgery.

Objective To prove through our experience that the prophylactic use of oral niacin and topical nitroglycerin results in a reduction in nicotine-related skin flap complications following facelift surgery.  Methods This is a retrospective, observational study. The study population consisted of 40 surgical patients (27 patients who used nicotine products and 13 non-nicotine users with signs of flap ischemia) seen over 18 months. Follow-up ranged from six months to 1.5 years. The main outcome measure was the incidence of flap ischemia in nicotine users who were placed on oral niacin and topical ni

Total hip arthroplasty for head and neck of femur fractures secondary to civilian gunshot injuries.

BACKGROUND: The optimal treatment for intracapsular neck of femur fractures secondary to civilian gunshot injuries (GSI) remains a challenge. Surgical fixation is associated with a high failure rate due to avascular necrosis and non-union. This manuscript reports on the largest series of patients who underwent total hip arthroplasty (THA) for civilian GSI involving the hip joint. The objectives are to assess clinical outcomes and to report on complications as well as associated injuries. METHODS: All patients who had undergone THA for a civilian GSI to the hip joint at a single Level 1 Trauma

Association Between Surgical Timing and Clinical Outcomes in Elbow Fracture Management.

Background Elbow fractures, including distal humerus, radial head and neck, and olecranon injuries, pose significant surgical challenges due to complex anatomy and the high risk of postoperative stiffness. Early fixation has been advocated to restore alignment and enable early mobilization, but concerns regarding soft tissue readiness and patient optimization often delay intervention. The 48-hour threshold for surgical timing was selected based on biological reasoning-intervening during the early inflammatory phase to minimize periarticular fibrosis-and logistical feasibility within tertiary-c

[On-Table Reconstruction of Radial Head Fractures].

PURPOSE OF THE STUDY: Fractures of the proximal radius constitute a significant proportion of elbow joint injuries, representing approximately one-third of all such fractures and 75% of proximal forearm fractures. In adults, they account roughly for 4% of all fractures. Treatment of these fractures varies depending on their severity. No uniform guidelines have been published to date for the treatment of severely comminuted fractures. MATERIAL AND METHODS: The study included 16 patients who underwent on-table reconstruction, with a mean age of 49 years. Half of the cases involved Mason type II

On a Countable Sequence of Homoclinic Orbits Arising Near a Saddle-Center Point.

BACKGROUND: Treatment failures and relapses are not uncommon in onychomycosis. Therefore, it is worthwhile to consider the combination of systemic and topical antifungals to improve the cure rates further and to reduce the duration of systemic treatment. OBJECTIVES: To evaluate and compare itraconazole pulse therapy combined with amorolfine with itraconazole alone in the treatment of Candida fingernail onychomycosis. METHODS: Ninety patients with moderate to severe Candida fingernail onychomycosis were randomized into two treatment groups of 45 subjects each. Group 1 received itraconazole pu

Pipkin type III fractures: a narrative review of literature and comparative opinion on ORIF vs. arthroplasty.

Pipkin Type III is extremely rare and is associated with worse prognosis and several complications. There is still no consensus on the management of injuries and whether these fractures should be treated surgically with ORIF or arthroplasty. Pipkin type III involves a combination of ipsilateral femoral head and neck fractures. Physioanatomical complexity of this fracture is often challenging, with a poorer prognosis than any other subgroups. The most common complications after management of Pipkin Type III are avascular necrosis of the femoral head and post-traumatic osteoarthritis. The treatm

Comparative success of different graft types in pediatric laryngotracheal reconstruction: a systematic review and meta-analysis.

BACKGROUND: Laryngotracheal stenosis (LTS) results from abnormal airway narrowing, primarily caused by iatrogenic injuries like prolonged intubation. It leads to respiratory distress and vocal complications, requiring interventions ranging from endoscopic procedures to open surgeries like laryngotracheal reconstruction (LTR) with autologous grafts. Pediatric patients pose unique challenges due to smaller airways but may have better healing outcomes. This review aims to synthesize evidence on the success and operative parameters of different grafts in pediatric LTR to guide clinical decision-ma

Patterns and Outcomes of Craniomaxillofacial Trauma in Puerto Rico: Insights From a Five-Year Retrospective Study.

Background Craniomaxillofacial (CMF) trauma is a significant problem in the United States, with estimated costs of nearly one billion dollars annually. Facial fractures occur based on factors such as facial structure, the direction and intensity of the impact, and the mechanism of injury. The most frequent facial fractures include the nose, orbits, zygomatic complex, mandible, maxilla, and frontal bone. Additionally, demographic, social, cultural, and environmental factors can contribute to particular trauma mechanisms like falls and motor vehicle accidents (MVAs), leading to different CMF inj

Application of Special Cross-midline Blood Supply Internal Mammary Artery Perforator Flap for Facial Reconstruction.

Severe burn injuries may limit head and neck mobility while distorting facial features; they also place significant psychological strain on patients. The anterior thoracic area, similar in color and texture to the adjacent face-neck regions, often uses preexpanded flaps for reconstruction. Preexpanded internal mammary artery perforator (IMAP) flaps can further minimize donor area damage, provide a relatively thin flap for repairing large skin defects, and do not require an additional graft flap to close the donor site. In this article, we presented a case involving prolonged tissue expansion l
