Frequency of Injuries in a Youth Soccer Tournament.

In brief: This study documents the types and frequency of injuries sustained in the 1984 Norway Cup-the largest soccer tournament in the world. During a total of 35,154 player-hours, 411 injuries and hyperventilatory conditions were recorded for the 1,016 boys' teams and 332 girls' teams participating. The injury rate was 8.9 and 17.6 per 1,000 player-hours for boys and girls, respectively. The overall injury rate for both sexes was 11.7 per 1,000 player-hours. Of total injuries 47% were contusions, 22% sprains, 18% lacerations, and 6% fractures. More than 60% of the injuries involved the lowe

Calvarial bone grafting of facial defects.

Bony defects of the face continue to challenge the reconstructive surgeon. Traditional sources of autologous bone from the rib or pelvis have been associated with the limitations of pain at the donor site, the need for a second operative field, variable "take" of the graft with poor eventual survival and, ultimately, less than optimal reconstruction. Calvarial bone grafting provides a readily accessible source of bone from an inconspicuous donor site, usually within the same operative field. The morbidity is minimal. The calvarial bone graft is especially useful in repair of the frontal sinus,

Magnetic resonance imaging of osteonecrosis.

Because the available preliminary data strongly indicate that MRI is accurate in diagnosing osteonecrosis, MRI of the hips is recommended in patients suspected of having osteonecrosis, especially if other diagnostic studies are equivocal. MRI also may be useful in following patients who have strong risk factors for osteonecrosis, such as corticosteroid therapy, femoral neck fracture, traumatic femoral head dislocation (Fig. 13), slipped capital femoral epiphysis (Fig. 14), and congenital hip dislocation. In diseases such as systemic lupus erythematosus, both hips eventually may become involved

Injury pattern analysis of helicopter wire strike accidents (-Gz load).

Injury patterns in rotary wing aircraft wire strike accidents were reviewed to determine mechanisms of injury. It was found that U.S. Army Safety Center data showed that between 1 January 1974 and 31 August 1981 there were 167 wire strikes involving Army helicopters which resulted in 60 injuries and 34 fatalities at a cost of $12,809,100. Updated data on all military rotary wing aircraft accidents investigated between 1978 and 1982 were screened by the Division of Aerospace Pathology to determine the mechanisms of injury to flight deck personnel. From 13 December 1978 to 23 June 1982, three ty

Experiences in the use of eight cutaneous and osteocutaneous superficial and deep circumflex iliac free flaps.

Eight patients underwent free tissue transfers involving groin cutaneous or osteocutaneous flaps using the superficial and deep circumflex iliac arteries and veins. Four were used for head and neck reconstruction, two for severe neck scar contractures, and two for composite skin and mandibular reconstruction. Three were used in lower extremity reconstructions, all in situations requiring bone and skin reconstruction. One was used for skin cover alone in an upper extremity reconstruction for wrist cover. One of the head and neck cases sustained vascular thrombosis requiring a second successful

Cochlear hearing loss in tympanoplasty.

In tympanoplasty, trauma to the cochlea is usually caused by the transmission of vibration induced during the removal of cholesteatoma, granulation tissue, and tympanosclerosis surrounding the ossicular chain. In 100 sequential cases 20 showed evidence of cochlear trauma. Hearing was recovered in 10 of these cases during the immediate postoperative period. In nine the hearing was partially recovered but remained below the preoperative bone-conduction level, and in one there was a total cochlear hearing loss. When the stapes footplate was fractured or dislocated and a perilymphatic fistula deve

Osteopoikilosis: a radiological and pathological study.

Anatomico-pathological and radiological studies of osteopoikilosis were performed in two cases, one involving a femoral head, excised after a fracture of a femoral neck, in an elderly man and the other following biopsy of an iliac crest in a young woman. In both patients wide-spread radiological evidence of the disorder was present as an incidental finding. The radiological appearance of rounded and linear densities corresponded to old and inactive remodelling of spongy trabeculae in epiphyseal and metaphyseal locations. The distribution and appearance of these osteopoikilotic densities sugges

Three-dimensional response of a lumped parameter head-neck model due to impact and impulsive loading.

A numerical procedure developed previously for predicting sagittal-plane motion of the human head-neck system due to impact and impulsive loading has been extended to three dimensions. In both situations, a lumped parameter approach is employed, but the current model lumps the mechanical response of each intervertebral joint into a single force-deformation relation evaluated from mechanical properties assembled by various investigators. Computations were performed to obtain the response of the model to a two-dimensional case of flexion whiplash, to one three-dimensional case of side impact to

The management of dural tear resulting from mid-facial fracture.

This study examines the relative risks and benefits of conservative and surgical management of dural tear secondary to middle third facial fracture and ascertains the type of skull and facial injury most often associated with the development of posttraumatic meningitis. Two projects were undertaken. First, the histories of 247 cases of major middle third facial fracture were reviewed with a recent follow-up of those patients who also sustained a dural tear. Second, 280 cases of bacterial meningitis were reviewed and particular attention was given to cases of posttraumatic meningitis. Of the 24

Elective ilioinguinal lymph node irradiation.

Most radiologists accept that modest doses of irradiation (4500-5000 rad/4 1/2-5 weeks) can control subclinical regional lymph node metastases from squamous cell carcinomas of the head and neck and adenocarcinomas of the breast. There have been few reports concerning elective irradiation of the ilioinguinal region. Between October 1964 and March 1980, 91 patients whose primary cancers placed the ilioinguinal lymph nodes at risk received elective irradiation at the University of Florida. Included are patients with cancers of the vulva, penis, urethra, anus and lower anal canal, and cervix or va

Fractures of the femoral neck in patients between the ages of twelve and forty-nine years.

Femoral neck fractures in young adults have a poor prognosis because the incidence of non-union and aseptic necrosis is high. We reviewed the results in twenty-seven consecutive patients with a femoral neck fracture who were younger than fifty years and who were treated with a standard protocol at Harborview Medical Center from 1975 to 1981. There were twenty-two male and five female patients, and they ranged in age from twelve to forty-nine years (mean, 32.4 years). Twenty patients were involved in high-velocity trauma and twelve of them had significant injuries to other organ systems. One of

The operative treatment of scapular fractures.

Fractures of the scapula occur mainly from direct trauma involving considerable violence and associated injuries of the shoulder and thorax are common. In most cases early functional treatment gives good or excellent results. Operative treatment may, however, be indicated, especially with displaced intra-articular fractures, fractures of the glenoid rim associated with humeral head subluxation, or unstable fractures of the scapular neck. Between 1967 and 1981, we treated 37 such fractures by open reduction and stable osteosynthesis. We were able to follow up 33 cases (89%), of which 21 (64%) h

The management of fractures of the mandible.

In some countries there is a move away from the traditional use of predominantly closed or semi-closed procedures in the management of facial fractures to widespread use of aggressive surgical techniques. Observations are made on the effect that this trend may have on the trainee Oral Surgeon. An analysis of a series of 156 consecutive cases of mandibular fracture bears out the impression that simple jaw support, when there is patient co-operation, is adequate for the treatment of the majority of mandibular fractures and that when fixation is indicated, the eyelet wiring technique gives excell

Radiation exposure and associated risks to operating-room personnel during use of fluoroscopic guidance for selected orthopaedic surgical procedures.

Because of the increased use of fluoroscopic guidance techniques in certain orthopaedic surgical procedures, surgeons and other operating-room personnel who are involved in these procedures are voicing growing concern over possible associated radiation health hazards. Using thin-layer lithium fluoride chips for thermoluminescence dosimetry, we directly measured the radiation exposure encountered by the primary surgeon during seven operative procedures that were done utilizing fluoroscopic guidance techniques. Dosimetry studies were also carried out using a tissue-equivalent phantom model to de

Is a certain degree of osteomalacia involved in femoral neck fractures of the elderly? Histological approach to the problem and practical applications.

A study was made in Geneva of 14 patients with femoral neck fractures (7 subcapital and 7 trochanteric) to determine whether a certain degree of osteomalacia might be involved (group I). No patients with risks factors for osteomalacia were used. For ethical reasons, double tetracycline labelling was not possible and osteomalacia criteria were based on osteoid tissue values determined in a histomorphometric study directly on cancellous bone samples of the femoral head. A comparison was made with femoral head bone tissue taken from 12 patients with hip osteoarthritis at a distance from the remod

Computerized tomography of the temporal bone.

Computerized tomography is acquiring an increasingly important role in the radiographic assessment of the temporal bone. In the latest generation of CT scanners, spacial resolution has been reduced to 0.1 cu mm and slice thickness to 1.5 mm. The main advantages of computerized tomography in comparison to multidirectional tomography are the improved contrast of the images and above all the clear demonstration of soft tissue structures and masses within or adjacent to the temporal bone. The following three applications of computerized tomography will be discussed: 1. Evaluation of intratemporal

Transmastoid decompression of the facial nerve in temporal bone fracture.

Among 41 cases of facial palsy caused by closed heart injury, temporal bone fractures were surgically confirmed in 36 cases, of which there were two mixed fractures and 34 longitudinal fractures. The fracture involved the geniculate ganglion area in 20 cases (55%). In 15 cases, decompression of the facial nerve was carried out using the transmastoid supralabyrinthine approach with disarticulation of the incus; in only five cases was the middle fossa approach used. The technique avoids the craniotomy for the middle fossa approach and is a reliable method of treatment in the majority of patients

Surgical management of metastatic disease of bone at the hip.

Concordant advances in cancer chemotherapy, radiotherapy, and surgical technique have been of apparent benefit to many cancer patients suffering from skeletal involvement at the hip. Consideration of the effects of underlying disease and prior treatment must be evaluated prior to developing a surgical plan. Acetabular involvement can be treated by either conventional or augmented total hip arthroplasty using protrusio rings and acetabular meshes, depending upon minor or major degrees of tumor destruction. Girdlestone resection arthroplasty for massive degrees of acetabular involvement yields o

Median nerve anatomy and entrapment syndromes: a review.

Clinicians commonly observe upper extremity signs and symptoms which result from median nerve entrapment and can develop at multiple sites along this nerve. Median nerve entrapment may occur at the distal humerus when the rarely present ligament of Struthers connects an anomalous bony spur of the humeral shaft to its medial epicondyle. The pronator syndrome refers to compromise of the median nerve in the proximal forearm region. This may result from entrapment between the 2 heads of the pronator teres, between the pronator teres and the flexor digitorum sublimis, or by the lacertus fibrosus ex
