Femoral neck fractures: a cohort comparison of nonunion and complication rates after ballistic versus blunt mechanism fractures.

PURPOSE: This study aims to review complications and union rates of ballistic femoral neck fractures when compared to their blunt counterparts. We hypothesize that gunshot injuries to the femoral neck may have higher rates of complications (nonunion, avascular necrosis [AVN], and implant failure) as a result of increasing comminution and disruption to the already tenuous blood supply through the femoral neck. METHODS: Following the Institutional Review Board approval, a retrospective chart review was performed identifying femoral neck fractures at a single level 1 trauma center from 2018 to 2

Internal Fixation With Cannulated Screws for Stable Femoral Neck Fractures in High-Risk Patients: Good Clinical Outcomes and Evaluation of Postoperative Complications.

This study aimed to investigate the clinical outcome and the postoperative complications after internal fixation with cannulated screws of stable femoral neck fractures (FNFs) in high-risk patients. A total of 76 patients (mean age 70.11 ± 9.83 years) with stable FNFs participated in the study. All patients underwent fixation with two or three cannulated screws (percutaneous). Postoperatively, they were evaluated with the Harris Hip Score (HHS), while parameters regarding other possible comorbidities or delayed complications were also assessed. Regarding the HHS, more than 75% of patients pres

Global incidence of osteonecrosis of the femoral head after femoral neck fracture surgery in adolescents: a meta-analysis.

BACKGROUND: Osteonecrosis of the femoral head (ONFH) is a significant postoperative complication following femoral neck fractures (FNFs) in adolescents, that has garnered considerable attention from researchers. Despite this interest, the incidence of ONFH in adolescents post-FNF surgery has not been extensively evaluated. To contribute to the body of research, we performed a meta-analysis utilizing articles sourced from multiple databases, with an emphasis on the rate of ONFH occurrence in young adolescents following FNF surgery. OBJECTIVE: The purpose of this study was to investigate the in

Characteristics of Lumbar Pars Interarticularis and Pedicle Stress Injuries by Sport in 902 Pediatric and Adolescent Athletes: A Retrospective Study.

BACKGROUND: Recent studies utilizing magnetic resonance imaging (MRI) for the evaluation of symptomatic lumbar spondylolysis in pediatric and adolescent athletes have indicated that upper level lumbar involvement has a higher incidence than previously reported. There has been a paucity of literature evaluating sport-specific patterns of lumbar spondylolysis, specifically upper versus lower level involvement. PURPOSE: To assess the potential risk factors for upper level stress injuries of the lumbar spine in pediatric and adolescent athletes. STUDY DESIGN: Cross-sectional study; Level of evid

Massive bone destruction in a human proximal femur from ancient Nubia.

Massive bone destruction in a human right proximal femur is described and differentially discussed in this case study. The individual was an older adult female buried at the ancient Nubian site of Tombos (modern-day Sudan) dating to the early Napatan/Third Intermediate Period (c. 1069-750 BCE). The right femur displayed a pathologic fracture with extensive lytic destruction and resorption of the entire femoral neck, most of the femoral head, and trochanters. Macroscopic and radiographic analyses revealed cortical thinning of the proximal diaphysis with new bone formation enclosing the medullar

Tumour-induced osteomalacia.

Tumour-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by increased production of fibroblast growth factor 23 (FGF-23). A 45-year-old man presented to us with progressive weakness over 2 years along with recurrent fractures with minimal trauma. His FGF-23 was found to be above the normal range and DOTATATE positron emission tomography (PET) scan showed a well-defined enhancing soft tissue density involving the left posterior ethmoid with extension to the spheno-ethmoidal recess and sphenoid sinus ostium. He underwent endoscopic sinus surgery and excision of the tumour. Hist

Nasopharyngeal Migration of Coils Following Embolization of a Traumatic Pseudoaneurysm of the Internal Carotid Artery: A Case Report.

Post-traumatic pseudoaneurysms of the internal carotid are a rare but potentially fatal cause of epistaxis; they are associated with fractures of the base of the skull with involvement of the carotid canal. Endovascular management is the preferred therapeutic strategy, with optimal long-term results and low complication rates. Complications may include thromboembolic events, infarction of perforating arteries, and rupture of the pseudoaneurysm. We present a case of a 28-year-old male with a post-traumatic pseudoaneurysm of the internal carotid who was managed with endovascular therapy. A late

Single Nasal Bones: A Report of Two Cases.

The nasal bones are important bony parts of the external nose and maxillofacial scaffold. Generally, the nasal bone is a small quadrangular plate paired and attached to the frontonasal suture superiorly and the nasal septum internally. The nasal septum is symmetrically located and comprises the septal cartilage, the perpendicular plate of the ethmoid bone, and the vomer. Herein, we report the anatomy of single nasal bones (SNB) identified in two human skulls. Two adult human skulls were found to have SNB. In one, the nasal septum was bifid and hooked as it joined with the nasal bone superiorly

Bikini Incision Modification of the Direct Anterior Approach.

BACKGROUND: Although the direct anterior approach (DAA) represents an intermuscular and internervous approach to total hip arthroplasty (THA), it did not reach global acceptance until its adoption by large teaching centers. Today, >50% of primary THA procedures in Switzerland are performed via the DAA. Besides being truly minimally invasive, a key advantage of the DAA is the inherent stability that it provides. A shortcoming has been that the traditional longitudinal skin incision does not follow the skin tension lines1 and therefore can result in wound-healing problems, poor scar cosmesis, an

Innovative Categorization and Operative Management of Auditory Ossicle Disruption Following Trauma: Therapeutic Efficacy and Interventional Paradigms.

Currently, there is no consensus on the treatment protocol for ossicular chain trauma. This study aims to investigate the classification and treatment strategies for traumatic ossicular chain dislocation. We retrospectively analyzed 15 patients. Traumatic ossicular chain dislocations were categorized based on the location of trauma identified during surgery: Type I-ossicular trauma without stapediovestibular dislocation; Type II-stapediovestibular dislocation (with or without associated incus dislocation). Of the 10 patients with Type I trauma, 9 experienced head trauma, and 1 had a penetratin

Correlation of Radiological Parameters with Functional Outcomes in Proximal Humeral Fractures Treated with PHILOS Plates: A Retrospective Analysis.

INTRODUCTION: Proximal humerus fractures are prevalent, especially among the elderly, and pose significant challenges in treatment, particularly for displaced fractures. Despite the availability of various surgical techniques, outcomes remain inconsistent, often due to poor anatomical reduction and fixation. This study examines the correlation between specific radiographic parameters and functional outcomes, aiming to identify key predictors of recovery. By focusing on these parameters, we aim to refine surgical approaches and improve follow-up strategies, ultimately enhancing patient outcomes

Traumatic Aneurysm Involving the Posterior Communicating Artery.

Traumatic intracranial aneurysms (TICAs) are rare, accounting for less than 1% of all intracranial aneurysms. However, they are associated with a mortality rate of over 50%. The case presented herein focuses on a posterior communicating artery TICA caused by violent aggression. A 41-year-old man with massive subarachnoid hemorrhage (SAH), on admission to hospital, had a CT angiography that showed a ruptured left posterior communicating artery aneurysm with continuous blood loss and underwent neurosurgical cooling. The CT scan also showed fractures of the mandible, mastoid and left styloid proc

Compact Arterial Monitoring Device Use in Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA): A Simple Validation Study in Swine.

Introduction Hemorrhage is the leading cause of preventable death in trauma in both the military and civilian settings worldwide. Medical studies from Operation Enduring Freedom (OEF) and Operation Iraqi Freedom (OIF) informed change in military prehospital medicine by influencing widespread tourniquet distribution and training on their use to stop life-threatening extremity hemorrhage. In the military setting, there has been a significant reduction in preventable death due to extremity exsanguination since the widespread implementation of tourniquets within the Department of Defense. However,

Traumatic Brain Injury in Patients With Frontal Sinus Fractures.

Traumatic brain injury (TBI) is an insult to the brain from an external mechanical force that may lead to short or long-term impairment. Traumatic brain injury has been reported in up to 83% of craniofacial fractures involving the frontal sinus. However, the risk factors for TBI at presentation and persistent neurological sequelae in patients with frontal sinus fractures remain largely unstudied. The authors aim to evaluate the prevalence and risk factors associated with TBI on presentation and neurological sequelae in these patients. The authors retrospectively reviewed patients who presented

Far Posterior Approach for Rib Fracture Fixation: Surgical Technique and Tips.

BACKGROUND: The present video article describes the far posterior or paraspinal approach to posterior rib fractures. This approach is utilized to optimize visualization intraoperatively in cases of far-posterior rib fractures. This technique is also muscle-sparing, and muscle-sparing posterolateral, axillary, and anterior approaches have been shown to return up to 95% of periscapular strength by 6 months postoperatively1. DESCRIPTION: Like most fractures, the skin incision depends on the fracture position. The vertical incision is made either just medial to a line equidistant between the palp

Identification of Neuropsychiatric Symptoms in Preclinical and Prodromal Stages in Dementias: Development of a Screening Test for Mild Behavioral Impairment.

UNLABELLED: The study investigates the roles of bacterial virulence factors in the development of mycotic aneurysms and explores the potential of silver nanoparticles (AgNPs) as a therapeutic intervention. Mycotic aneurysms are different from degenerative ones in that they result from bacterial infections, and eradication or control of their virulence factors is influenced by bacteria. An early diagnosis with prompt treatment is needed in order to save lives since this type of aneurysm carries a high mortality rate associated with it. Imaging techniques include computed tomography (CT), positr
