Pathological fracture of the mandible, secondary to eosinophilic granuloma in a medically compromised patient.

A case of eosinophilic granuloma of the mandible with submandibular lymph node involvement in a medically compromised patient is presented. The lesion caused the pathologic fracture of the. mandible. It was treated radically with the resection of the mandible between the angle posteriorly and the midline anteriorly and immediate reconstruction with a 3DBDB AO reconstruction plate. This was followed by telecobalt therapy of 2000 cGys in 10 fractions over 20 days. The combined surgical and radiation therapy should be reserved for patients having: (1) unremitting pain following currettage (2) Limi

Sclerosing mucoepidermoid carcinoma with eosinophilia of the thyroid: report of two patients, one with distant metastasis, and review of the literature.

Sclerosing mucoepidermoid carcinoma with eosinophilia (SMECE) is a recently recognized malignant neoplasm of the thyroid gland. Two additional cases of this condition which occurred in a 70-year-old woman and a 69-year-old woman are presented. The case of the 70-year-old woman (patient 1) is the first report of distant metastasis, besides lymph node metastasis, for this type of tumor. The patient initially presented with a thyroid mass, and the thyroid gland with surrounding cervical lymph nodes was removed. Because of focal keratin "pearl" formation, the tumor was misinterpreted as a metastat

Condylar injuries in growing patients.

Although the mandibular condyle is one of the most common sites of injury of the facial skeleton, it is also the most overlooked and least diagnosed site of trauma in the head and neck region. The condyle forms the very cornerstone of mandibular form and function and therefore injuries to the mandibular condyle in growing children may adversely affect growth and development of the jaws and the occlusion. The aim of this article is to present an overview of condylar injuries in growing patients for the purpose of increasing the awareness of all dental practitioners involved in the treatment of

Differences in the incidence of injury between rugby league forwards and backs.

Evidence with regard to the incidence of injury to forwards and backs in the game of rugby league is extremely limited. A four year prospective study of all the injuries from one professional Rugby League club was conducted. All injuries that were received during match play were recorded, and those for forwards and backs compared. Forwards had a higher overall rates of injury than backs (139.4 [124.2-154.6] vs. 92.7 [80.9-104.6] per 1000 player hours, P < 0.00006). Forwards had a higher rate of injuries to all body sites with the exception of the ankle and the 'others' category of injury. They

Experimental impact injury to the cervical spine: relating motion of the head and the mechanism of injury.

The purpose of this study was to analyze, with use of an impact model, the relationships among motion of the head, local deformations of the cervical spine, and the mechanisms of injury; the model consisted of the head and neck of a cadaver. Traditionally, the mechanisms of injury to the cervical spine have been associated with flexion and extension motions of the head and neck. However, the classification of the mechanisms is not always in agreement with the patient's account of the injury or with lacerations and contusions of the scalp, which indicate the site of the impact of the head. Descending necrotizing mediastinitis: a complication of dental implant surgery.

BACKGROUND: The placement of osseointegrated dental implants is considered a minimally invasive procedure with a low complication rate. Reported complications include local trauma to neurovascular structures, mandible fractures, sinusitis, and localized gingivitis. Major life-threatening complications are extremely rare. Severe infection has not been reported in a review of the English literature. METHOD: Case study. RESULTS: We present a case of life-threatening deep neck space infection resulting in descending necrotizing mediastinitis following osseointegrated dental implant placement. Tr

Fatal light aircraft accidents in Ontario: a five year study.

Fatal civil aviation crashes in Ontario from 1985 to 1989 were studied. Data regarding accident circumstances, injury patterns and medical factors (disease, alcohol/drugs) which could have contributed to accident causation was obtained from a review of the files of the Chief Coroner for Ontario in Toronto and the aviation occurrence reports of the Transportation Safety Board of Canada. Forty-seven crashes involving mainly general aviation type aircraft but also 2 gyroplanes, 2 ultralights and a glider were reviewed. About half occurred during the cruise phase of the flight. Seventy (40 pilots;

Priorities in the management of penetrating maxillofacial trauma in the pediatric patient.

Penetrating facial trauma is uncommon in children; a large series published by Cooper et al revealed that only 1% to 2% of the total population of infants and children admitted for trauma during their study period had a diagnosis of penetrating trauma to the head or neck. Little has been published specifically addressing these injuries in the pediatric population. The records of 20 patients treated for penetrating facial injuries at Kosair-Children's Hospital in Louisville, Kentucky from January 1991 through December 1994 were reviewed. The location, mechanism and extent of injury, as well as

Nonunions of the surgical neck of the humerus: surgical treatment with an intramedullary bone peg, internal fixation, and cancellous bone grafting.

Twenty patients with pseudarthrosis of the upper humerus underwent surgery with the intramedullary bone peg technique. A 6 to 10 cm corticocancellous autogenous bone graft (11 iliac crest, 6 anterior tibial crest, 3 middle-third of the fibula) was pegged] into the humerus and bridged the pseudarthrosis. Stability of the fracture site was obtained by plate osteosynthesis; an additional peripheral cancellous graft was performed. Our patient series included 15 women and five men with an average age at operation of 58 years; the dominant side was involved in 12 cases. Eleven had undergone 22 previ

Delayed diagnosis in a rural trauma center.

BACKGROUND: The rapid and accurate diagnosis of all injuries is critical in trauma surgery. Injuries not diagnosed after the secondary survey are not without serious consequences. Therefore, in an effort to decrease this problem a policy was initiated to perform an ongoing serial exam during the entire course of each patient's involvement with the trauma team at Saint Francis Medical Center. METHODS: Prospective identification and evaluation of patients admitted to a single trauma service with delayed diagnosis was done from July 1, 1993, to October 31, 1995. RESULTS: Sixty-eight delayed dia

[Surgical treatment of radiation-induced lesions of the hip in adults].

The authors report their experience in the surgical management of lesions of the adult hip following radiotherapy. The diagnosis of a hip problem occurring after pelvic radiotherapy for malignant tumors is made by clinical and radiological examination. The problems include femoral head necrosis, necrosis and/or fracture of the acetabulum, or involvement of the entire hip joint (radiation coxopathy). Fractures of the femoral neck have been described in the literature, but are now very rare following the routine use of external shields as protection during irradiation. Post-irradiation lesions a

Triradiate cartilage fracture associated with a transepiphyseal separation of the femoral head: a case report.

Fracture-separation of the capital femoral epiphysis is an extremely rare lesion, representing approximately 8% of femoral neck fractures according to the Delbet and Colonna classification. The prognosis is generally quite poor when it is associated with hip dislocation. Traumatic disruption of the triradiate cartilage is an uncommon lesion and may be associated with fusion of the physis with the possibility of progressive subluxation of the hip. We report an exceptional case combining a capital femoral transepiphyseal fracture and Salter-Harris type 2 triradiate cartilage injury that required

[Femur head dislocation fractures. Long-term outcome of conservative and surgical therapy].

From 1974 to 1989, we treated 32 patients with a femoral head fracture: 28 were associated with a traumatic posterosuperior dislocation of the hip (6 Pipkin type I, 7 type II, 4 type III and 11 type IV), 1 with a posteroinferior and 3 with an anterior displacement of the femoral head. Twenty-four patients had been involved in a traffic accident, 23 had associated injuries, and the average polytrauma score (PTS) was 21. All dislocations primarily treated at our hospital were reduced by closed methods within 4 h (mean 105 min). Eleven patients received no further operative treatment, 21 were tre

Quantitative bone scanning of the hip. Comparison between the perfusion and static phases.

Quantitative bone scanning of the hip was carried out in 50 patients, 45 with unilateral disorders, and 5 with no symptoms and normal radiographs. The uptake ratio of the involved femoral head was calculated in the perfusion and static phases. In the patients with no symptoms the difference in the uptake ratio between the right and left hips averaged less than 0.02 in both phases. In 5 patients with a unilateral endoprosthesis the average was 0.52 in the perfusion and 0.34 in the static phase. In 10 patients with intertrochanteric fractures, 9 with undisplaced femoral neck fractures and 7 with
