Management of panfacial fractures.

Panfacial injuries are those that involve trauma to the lower, middle, and upper facial bones. They often are accompanied by multisystem- or polytrauma and require a team approach for management. Although stabilization of the overall status of the patient takes precedence, early and total restoration of facial form and function should be the goal of the maxillofacial surgeon.

Common cycling injuries. Management and prevention.

The increasing participation in the athletic forms of bicycling warrants expanded physician attention to the traumatic and overuse injuries experienced by cyclists. The modern bicycle consists of a frame with various components, including handlebars, brakes, wheels, pedals, and gears, in various configurations for the various modes of cycling. For high performance cycling the proper fit of the bicycle is critical. The most efficient method to provide an accurate fit is the Fitkit, but proper frame selection and adjustment can be made by following simple guidelines for frame size, seat height,

[Traumatic sinus pericranii. Case report].

The patient was involved in a traffic accident at the age of 1 and the left parieto-occipital scalp was contused without skull fracture. At the age of 5, an extracranial scalp mass was first noticed just beneath the multiple scalp scars. Angiography through the mass revealed that the extracranial mass cavity was connected to the superior sagittal sinus through the emissary veins. The mass located in the subgaleal, epiperiosteal space was totally resected and the connection with the intracranial sinus was closed with bone wax. Histologically, there were many capillaries and some large blood cav

Axial loading injuries to the middle cervical spine segment. An analysis and classification of twenty-five cases.

Injuries to the cervical spine at the C3-C4 level involving the bony elements, intervertebral disks, and ligamentous structures are rare. We present 25 cases of traumatic C3-C4 injuries sustained by young athletes and documented by the National Football Head and Neck Injury Registry. Review of the cases reveals that the response of energy inputs at the C3-C4 level differ from those involving the upper (C1-C2) and lower (C4-C5-C6-C7) cervical segments. Specifically, the C3-C4 lesions appear to be unique with regard to the infrequency of bony fracture, difficulty in effecting and maintaining red

Three-wheeled vehicle injuries in children.

Injuries associated with nonmotorized three-wheeled vehicles were prospectively evaluated through a standard questionnaire during the summer months at the emergency department of the Children's Hospital of Philadelphia, Pa. Forty-four children, including 32 boys, with a mean age of 3.77 years, reported such injuries. Most fell (36.4%), but a second vehicle was often involved. Most children (33 of 44) were on low-slung Big Wheels. Head, neck, and dental injuries accounted for the majority. Most injuries were not serious. Two patients were admitted; both had fractures, one of them had a concussi

Femoral capital physeal fractures in 25 foals.

The medical records of 25 horses 1 year of age or younger affected with femoral head and neck fractures during an 18 year period were reviewed. Each fracture involved the capital physis. The foals were 11 days to 12 months of age (mean, 5 months). No femoral capital physeal fractures occurred in horses older than 1 year of age during the same period. The history in each case included acute onset of severe unilateral hindlimb lameness, 3 hours to 2 months (mean, 12 days) before presentation. Injuries observed were violent falls, struggles, and kicks. Crepitation, swelling, pain with manipulatio

Acquired maxillofacial defects from motor vehicle accidents: statistics and prosthodontic considerations.

Trauma from motor vehicle accidents is one of the leading causes of death in the United States; moreover, the costs related to personal harm are only second to cancer. Head and neck injuries predominate and involve contact with the vehicle's interior (80%), contact with the vehicle's exterior (12%), or noncontact with the vehicle (8%). The patient with maxillofacial defects resulting from motor vehicle accident will have numerous soft tissue and hard tissue injuries ranging from neurologic involvement to fractures and/or avulsions of the temporomandibular joint, maxillae, mandible, teeth, and

An Unusual Fracture in Arm Wrestling.

In brief: Previous reports of arm wrestling fractures have involved the distal third of the humerus. What makes this case unique is that the fracture occurred at the radial neck. The mechanism of injury probably involved extreme pronation where the radial head is locked into the capitellum-an area of potential weakness. Treatment was conservative, consisting of keeping the elbow flexed, 90° in a sling.

[Clinical and therapeutic concepts in ischemic femur head necrosis].

Bone ischaemia can result from four mechanisms: (1) interruption of arterial inflow (e.g. after femoral neck fracture), (2) occlusion of venous outflow (possibly due to capsular distension), (3) intravascular arteriolar occlusion (as in sickle cell disease); (4) extravascular sinusoidal tamponade (e.g. in Gaucher's disease). Osteonecrosis following high-dosage corticosteroid administration or alcohol abuse could, theoretically, be due to either intravascular fat embolism or sinusoidal tamponade resulting from the marked fat deposition in the marrow. It is proposed here that, except in traumati

[Juvenile epiphyseolysis of the femoral head--coxa vara development and treatment].

Children's coxa vara is based on epiphyseolysis of the proximal femoral epiphysis due to relative overload of a primarily healthy metaphysis, or to a "normal" load on diseased structures. These give passively way in the direction of varus and retroversion of the head in the presence of fatigue fractures. Etiologically the idiopathic C. vara infantum can be differentiated from the symptomatic forms with known causes. Broadening of the epiphyseal line, spur formation and cystic bone resorption of the medial-caudad portions of the metaphysis are early x-ray signs. When the process involves the wh

Sequelae and reconstruction after septic arthritis of the hip in infants.

We evaluated the residual deformity and late treatment of thirty-four hips of thirty-one children who had had septic arthritis when they were less than one year old. The hips were classified into four groups on the basis of radiographic changes. Type-I deformity (five hips) involved transient ischemia of the epiphysis, with or without mild coxa magna, and these hips did not need reconstruction. Type-II deformity (eleven hips) included deformity of the epiphysis, physis, and metaphysis, and these hips needed an operation to prevent subluxation; the goals of the operation included improvement in

Three-dimensional x-ray computed tomography of the temporal bone as an aid to surgical planning.

Three-dimensional x-ray computed tomography (3DCT) has been useful in planning surgical procedures involving craniofacial reconstruction, the pelvis, hip, spine, knee and shoulder. The clinical use of 3DCT in temporal bone surgery has not been evaluated. We used 3DCT to assess temporal bone anatomy in 9 patients. These cases evaluated two encephaloceles, two postinflammatory dehiscences, two temporal bone fractures, one glomus jugulare tumor, one acoustic neuroma, and one meningioma. We found 3DCT useful in these temporal bone applications as a supplement to standard two-dimensional CT scannin

General anesthesia for horses with specific problems.

We have discussed anesthetic techniques, special considerations, and expected complications involved in anesthetizing horses for abdominal, orthopedic, and head and neck surgery, and myelography and have described expected physiologic dysfunction that may require changes in anesthetic technique or supportive measures. The objective is high-quality patient care and reduction in anesthesia-related morbidity and death.

Open reduction and fixation of proximal humeral fractures and fracture-dislocations.

Open reduction and internal fixation was employed in the treatment of 25 severely displaced fractures and fracture-dislocations of the proximal humerus. Our aims were accurate reduction and stable fixation to allow early mobilisation and to achieve full functional recovery. In 15 fractures an AO T-plate was used and in 10 a bent semitubular plate was employed as a blade plate. Excellent or satisfactory results were obtained in all six patients with two-part fractures involving the surgical neck; in four of the five patients with three-part fractures involving the surgical neck and tuberosities

Pauwels osteotomy for nonunions of the femoral neck.

The concept of the Pauwels osteotomy seems to be a valuable method of treating nonunions of the femoral neck. If there is concomitant avascular necrosis, the involved area should be small and the patient younger than 60 years old.

[Femur head necrosis in childhood].

The natural history of avascular necrosis of the femoral head in children and adolescents shows significant differences compared to the course of this disease in adults. Depending on the remaining growth potential, even extensive damage of the femoral head can regenerate, as long as the epiphyseal growth plate is not involved in the process. Otherwise, the deformity will increase with time. Current knowledge on the etiology, natural history, diagnosis and treatment is described on Perthes' disease and other possible causes of avascular necrosis, such as CDH, epiphyseolysis of the femoral head,

Cellular enzymatic activities in the articular cartilage of osteoarthritic and osteoporotic hip joints of humans: a quantitative cytochemical study.

Quantitative cytochemical methods were applied to measure the activity of several oxidative enzymes in human articular cartilage of the femoral head obtained from osteoarthritic patients (OA) and from patients with fractured femoral neck (OP) due to primary osteoporosis. In both conditions, lactate dehydrogenase (LDH) was found to be the most active one followed by two additional cytosolic enzymes: glyceraldehyde 3-phosphate dehydrogenase (GAPD) and glucose 6-phosphate dehydrogenase (G6PD). On the other hand, the activity of mitochondrial enzymes such as succinate dehydrogenase (SDH) and beta-

Floating petrous bone fracture.

Floating petrous bone fracture appears to occur when the elastic pediatric skull is subjected to large compressive forces. Unlike a longitudinal fracture, a floating petrous bone fracture does not involve the ear canal and squamous cortex. Rather, the fracture line runs through the middle ear, separating the petrous apex from its lateral and inferior bony attachments. Characteristic clinical findings include immediate onset of conductive hearing loss, with ipsilateral sixth and seventh cranial nerve paralysis. This article details the third known occurrence of this type of fracture, and includ

[Osteosynthesis using the "palm-tree" nail technic in fractures of the surgical neck of the humerus].

Presented in 1974, this technique consists of internal fixation of the humeral neck by means of three divergent K-wires. These are introduced through a proximal entry point located on the lateral aspect of the humeral diaphysis at the lower end of the "V" shaped insertion of the deltoid which is one of the three subcutaneous areas of this bone. The two principles of this technique are: anchorage of the epiphysis by three divergent pins driven into the cancellous bone of the head and introduction of the pins from a very proximal point that allows easy access across the fracture without the disa
