Fractures of the frontal sinus.

It is not possible to establish rigid guidelines for the treatment of frontal sinus fractures. However, this series demonstrates that aggressive treatment of the sinus with damage to the frontal sinus drainage system will produce a "safe" sinus. The individual surgeon must base his or her treatment philosophy on the type and severity of injuries being treated, the patient population involved, and his or her experience with each of the treatment options. All patients must be made to realize that they have a life-long risk for delayed complications following any type of reconstructive procedure

[Femoral osteonecrosis in patients treated with radiochemotherapy of Hodgkin's lymphoma].

Therapeutic progress has much improved the prognosis of Hodgkin's disease; therefore long-term complications of the treatment have become a major problem. Among these, the authors focused on osteonecrosis of the femoral head, and conducted a retrospective study on 182 patients treated with inverted-Y-field radiation therapy combined to MOPP chemotherapy in 129 cases. Femoral osteonecrosis was found in 6 patients (3.3%), 5 males and 1 female; 4 of them received combined modality treatment (inverted-Y radiation therapy + MOPP), and 2 radiation therapy alone. The interval between the end of treat

Immediate reconstruction of contaminated central craniofacial injuries with free autogenous grafts.

Free autogenous osseous and soft tissue grafts were used for the immediate, one-stage reconstruction of central craniofacial injuries involving the frontal sinus in 95 patients with wounds contaminated by either skin or nasal bacteria. Graft removal and delayed reconstruction were necessary in only one patient who suffered an infection in the first postoperative week. To date, no delayed complications are known to have occurred in any patient. As anticipated, long-term follow-up has been erratic (6 weeks to 5 years) and only suggestions rather than definite guidelines for the management of the

Gorham's disease affecting the maxillofacial skeleton.

Twenty-one cases of Gorham's disease in the maxillofacial region are described in the literature. This case is an additional one. The disease has a slight male predominance. All cases involved either the mandible alone or in association with the maxilla or other bones of the head and neck. The most common symptoms were pain, tooth mobility, mandibular pathological fractures, and facial deformity. The mean duration of symptoms prior to diagnosis was 6.4 years. Treatment included surgery, radiotherapy, and various medications, alone or combined. Four patients died of their disease. The diagnosis

Injuries due to deliberate violence in areas of Denmark. III. Lesions. The Copenhagen Study Group.

As part of a transcultural investigation of violent behavior in Denmark and South America, the lesions from accidents involving deliberate violence registered in three Danish emergency wards during a 1-year period were studied. A quantity of 2211 lesions were diagnosed in 1316 patients (953 male and 363 female patients). Sixty-five percent of the lesions were in the head/neck region, 13% in truncus, 18% in the upper extremities and 5% in the lower extremities. The most frequent diagnosis was an open wound in the head/neck region. Adding contusions and fractures in the same region this amounted

Frontal sinus fractures: some problems and some solutions.

Head trauma can result in serious injuries leading to life-threatening situations. Often accompanying these injuries are fractures involving the frontal sinus. Some fractures are subtle and difficult to diagnose, while others are massive and difficult to treat. The experience at this institution in the management of frontal sinus fractures is examined. Particular emphasis is placed on the problematic cases. These include fractures that were nearly overlooked, those that were associated with massive CSF leaks, and those which were extensively comminuted. In treating some of the most severe comp

[Conservative treatment of pediatric elbow fractures].

Between 1978 and 1982, 146 children with elbow fractures were treated at the Department of Traumatology of the Freiburg University Hospital. Forty-two children received conservative treatment and in 104 an operation was performed. The possibilities for conservative treatment of the elbow joint in children are discussed together with our results. Out of 47 supracondylar fractures of the humerus, 25 were treated conservatively (28%, dislocation grade 1; 48%, grade 2). Good or very good results were achieved in more than 76%. Unstable grade 3 fractures often involve rotation of the elbow; in 35%

Subcranial management of 395 combined frontobasal-midface fractures.

In severe craniofacial injuries, the involvement of the skull base with concomitant major dural tears is significantly high. Our methods and treatment plan are controversial compared with the conventional procedure: primary urgent neurosurgical exploration and repair with deferral of maxillofacial reconstruction. To avoid the disadvantages of the transfrontal intracranial management of the skull base, we modified the transethmoidal approach so as to enable the subcranial exposure of all the anterior fossa planes, including the sellar-sphenoidal region. The advantages rendered by this method ar

Rigid internal fixation for fractures involving tooth-bearing maxillary segments.

Fracture dislocations of the middle third of the face usually involve a complex combination of the three types of fractures initially described by LeFort. Treatment of these injuries requires a six- to eight-week period of intermaxillary fixation, unless rigid internal fixation devices (plates and screws) are used to stabilize the fractures. However, rigid fixation carries the risk of producing a malunion and serious malocclusion if not performed correctly. A review of 22 patients with complex LeFort fractures treated with rigid fixation revealed that the only absolute contraindication to its

Osteogenesis imperfecta type I with unusual dental abnormalities.

A large kindred with dominantly inherited osteogenesis imperfecta was evaluated. Affected individuals had bone fractures, blue sclerae, and hearing loss. In addition, all had dental abnormalities distinct from those previously described in other families with this syndrome. Deciduous teeth were normal in color or blue-grey. On radiographs of an early developing deciduous dentition, pulps were larger than normal. In patients with mixed dentitions, pulp chambers of deciduous teeth were partially obliterated. Increased constriction at the junctions of the crowns and roots was found in some decidu

Frontal sinus fractures: a review of 72 cases.

A retrospective review is presented of 72 patients who sustained frontal sinus fractures (FSF) and were subsequently treated by the department of otolaryngology/head and neck surgery between the years of 1974 and 1986. Eighty-four percent of FSF occurred in males and 71% were a result of motor vehicle accidents. Only 24% remained conscious at the time of trauma, and in only one third of the cases was there no other fracture. Seventy-six percent of FSF involved both the anterior and posterior walls--a figure that possibly reflects the referral patterns to a hospital that is a major regional tra

Spinal injury at the level of the third and fourth cervical vertebrae resulting from the axial loading mechanism: an analysis and classification.

The traumatic C3-C4 level injuries sustained by young athletes and documented by the National Football Head and Neck Injury Registry are discussed. Twenty-eight of 885 (2.8 per cent) injuries involved the C3-C4 vertebrae. Review of these cases reveals that the response to energy inputs at the C3-C4 level differ from that of those involving the upper (C1-C2) and lower (C5-C6) cervical segments. Specifically, these lesions appear unique with regard to infrequency of bony fracture, difficulty in effecting and maintaining reduction, and their more favorable response to early aggressive treatment.

The National Football Head and Neck Injury Registry: 14-year report on cervical quadriplegia (1971-1984).

The specter of catastrophic cervical neurotrauma resulting from athletic participation, although infrequent, has been consistently associated with football, water sports, gymnastics, rugby, and ice hockey. Injury involving intracranial hemorrhage can result in death or permanent neurologic impairment, whereas certain fractures and dislocations of the cervical spine are associated with quadriplegia. Athletic injuries to both the central nervous system and spinal cord demand our attention as an active area of clinical and basic injury. A review of the available literature reveals changing injury

The architecture of the proximal femur: an imaging analysis.

The proximal femur is a dynamic region whose internal architecture reflects the biomechanical demands of weight-bearing. It is composed of two major trabecular systems arranged along the lines of compressive and tensile stresses. Adaptive alterations in these trabeculae occur in a variety of disorders, including osteoporosis, osteoarthritis, and ischemic necrosis. Plain film radiography and computed tomography (CT) have been utilized to study these changes and provide insight into the pathophysiology of these disorders. Analysis of the order of resorption of trabeculae in osteoporosis, the pro

The free greater omental flap for treatment of mandibular osteoradionecrosis.

Osteoradionecrosis can involve the mandible following radical irradiation for treatment of oral cavity cancer. The radionecrosis of the mandible is often associated with severe intractable pain, local or extensive deformity, including pathologic fracture, orocutaneous fistula formation, and frequent loss of function. Treatment has ranged from analgesia and antibiotics to hyperbaric oxygen treatments to local or extensive sequestrectomies with partial or total mandibulectomy and restoration of tissue losses with unirradiated tissue. To our knowledge, this is the first report of the successful u

[Whiplash trauma of the cervical vertebrae in traffic: biomechanical and expert-opinion aspects].

The indirect injury mechanisms of the cervical spine should be subdivided into "non-contact injury" of the cervical spine, without head impact, by definition (hyperflexion or hyperextension), sometimes also called whiplash injury mechanism, and bending mechanism due to head contact, i.e. "contact injury" (hyperflexion, hyperextension, lateral bending, axial compression). The term "whiplash" is, in strict biomechanical terms, very often incorrect since it implies a biphasic motion (hyperextension and hyperflexion, or vice versa); in traffic collisions, however, its second phase is insignificant

Traumatic intratemporal facial nerve injury: management rationale for preservation of function.

A retrospective review of 29 cases of intratemporal facial nerve injuries included 18 temporal bone fractures, 7 gunshot wounds, and 4 iatrogenic complications. Surgical exploration confirmed involvement of the fallopian canal in the perigeniculate region in 14 longitudinal and 3 transverse or mixed fractures of the petrous pyramid. Gunshot and iatrogenic injuries usually occurred within the tympanic and vertical segments of the facial canal and at the stylomastoid foramen. When hearing is salvageable, the middle fossa approach provides the best access to the perigeniculate region of the facia

Nasal spine suspension for reduction and immobilization of mandibular fractures.

Successful treatment of mandibular fractures involves proper fracture reduction and immobilization for an adequate length of time. A simple wire fixation technique that can be used in many situations involves suspension wiring from the base of the anterior nasal spine to a pair of circummandibular wires. The technique may be used alone or adjunctively with other methods of fixation. It offers several advantages over other methods, particularly in the treatment of pediatric mandibular fractures.

Obturator hip dislocation with ipsilateral fractures of the femoral head and femoral neck. A case report.

A 28-year-old female pedestrians, struck by an automobile, sustained an obturator hip dislocation with concomitant ipsilateral fractures of both the femoral head and femoral neck. This unusual hip injury seems not to have been previously reported. The femoral neck fracture was widely separated from the shaft, and unfortunately, a 6-mm deep indentation fracture involved the anterosuperior portion of the articulating surface of the femoral head. This complex injury was treated with a press-fit bipolar prosthesis.

Obliteration of mastoid and middle ear for severe trauma to the temporal bone.

A review of four cases of severe trauma to the temporal bone involving fracture of the external auditory canal indicates that adipose obliteration of the mastoid and middle-ear cleft with closure of the external auditory canal is the optimal method of repair. In two of four patients, one-stage repair was successful using this technique, and in the other two patients, obliteration was required as a revision operation after cholesteatoma developed from previous procedures.
