Is a certain degree of osteomalacia involved in femoral neck fractures of the elderly? Histological approach to the problem and practical applications.

A study was made in Geneva of 14 patients with femoral neck fractures (7 subcapital and 7 trochanteric) to determine whether a certain degree of osteomalacia might be involved (group I). No patients with risks factors for osteomalacia were used. For ethical reasons, double tetracycline labelling was not possible and osteomalacia criteria were based on osteoid tissue values determined in a histomorphometric study directly on cancellous bone samples of the femoral head. A comparison was made with femoral head bone tissue taken from 12 patients with hip osteoarthritis at a distance from the remod

Computerized tomography of the temporal bone.

Computerized tomography is acquiring an increasingly important role in the radiographic assessment of the temporal bone. In the latest generation of CT scanners, spacial resolution has been reduced to 0.1 cu mm and slice thickness to 1.5 mm. The main advantages of computerized tomography in comparison to multidirectional tomography are the improved contrast of the images and above all the clear demonstration of soft tissue structures and masses within or adjacent to the temporal bone. The following three applications of computerized tomography will be discussed: 1. Evaluation of intratemporal

Transmastoid decompression of the facial nerve in temporal bone fracture.

Among 41 cases of facial palsy caused by closed heart injury, temporal bone fractures were surgically confirmed in 36 cases, of which there were two mixed fractures and 34 longitudinal fractures. The fracture involved the geniculate ganglion area in 20 cases (55%). In 15 cases, decompression of the facial nerve was carried out using the transmastoid supralabyrinthine approach with disarticulation of the incus; in only five cases was the middle fossa approach used. The technique avoids the craniotomy for the middle fossa approach and is a reliable method of treatment in the majority of patients

Surgical management of metastatic disease of bone at the hip.

Concordant advances in cancer chemotherapy, radiotherapy, and surgical technique have been of apparent benefit to many cancer patients suffering from skeletal involvement at the hip. Consideration of the effects of underlying disease and prior treatment must be evaluated prior to developing a surgical plan. Acetabular involvement can be treated by either conventional or augmented total hip arthroplasty using protrusio rings and acetabular meshes, depending upon minor or major degrees of tumor destruction. Girdlestone resection arthroplasty for massive degrees of acetabular involvement yields o

Fractures of the mandible: a review of 580 cases.

The anatomic distribution of fractures in this study showed more fractures of the condylar and parasymphysis/symphysis regions, with correspondingly fewer body and angle fractures, than in other reported studies. The site of fracture is related to the type of trauma involved. Altercations, in which most force is directed in a single blow to the lateral aspect of the jaw, tend to result more frequently in angle and body fractures and less often in condylar, symphysis, and alveolar fractures. Automobile accidents, which more frequently involve trauma to the anterior mandible, result in more frac

Fractures of the radial head and neck in children.

In thirty-three children with fractures of the radial head or neck, and with a minimum follow-up of two years, fractures of the radial neck were most common. The proximal radial epiphysis was usually closed by the age of fifteen years. The older children in the series had the worst prognosis, as did children with other associated upper-extremity injuries, usually on the medial aspect of the elbow. The results were best if treatment was initiated early, and closed reduction usually gave the most satisfactory results. Roentgenograms of the involved elbow were often abnormal even with a good clin

Facial nerve paralysis: it is always Bell's palsy?

Most patients with facial nerve paralysis have idiopathic, or Bell's palsy. Nevertheless, other more serious causes should be carefully excluded before Bell's palsy is diagnosed. Several tests can help the physician determine the site of the lesion and the physiologic extent of involvement of the facial nerve. The type of treatment depends on the cause of the paralysis and whether it is complete or partial. Familiarity with facial nerve anatomy and the various causes of facial paralysis should facilitate early diagnosis and appropriate treatment.

Anisocoria with orbital floor fractures: worry and wait.

We present a new complication: transient pupillary dilatation in an eye that sustained a posterior orbital floor blow-out fracture. By reviewing the neural pathways involved with pupillary motor function, we surmise the causes of this complication to include trauma to the ciliary ganglion, short ciliary nerves, or the parasympathetic motor root. This type of injury has only recently been reported in the ophthalmologic literature, and has not, prior to this report, been included in the otolaryngologic literature.

Recent advances in paranasal sinus surgery.

Recent advances in paranasal sinus surgery have occurred in the treatment of trauma and neoplasia, rather than in the more traditional management of inflammatory disease. Cranialization of the frontal sinus in severe penetrating trauma was first described in 1978. By removal of the sinus posterior wall, excision of the mucosa, and preservation of the anterior wall fragments, the forehead contour can be preserved and the cavity ablated by the forward expansion of the frontal lobes. Craniofacial surgery for malignancies of the paranasal sinuses has vastly improved the survival rate for these pat

Systemic prophylactic antibiotics: need the 'cost' be so high?

To assess compliance with accepted principles, the use of prophylaxis with systemic antibiotics in selected specialty procedures was examined. The operations reviewed were aortofemoral bypass, pulmonary resections, open-hip procedures, and head-neck cancer procedures that involve the oropharyngeal cavity. Of all patients, 74% received antibiotics preoperatively and 79% received prophylaxis with antibiotics longer than 24 hours postoperatively. Evaluated against the criteria of preoperative initiation and limited postoperative administration (less than 24 hours) only 10% of the patients receive

Treatment of nasofrontal ethmoidal complex fractures.

Successful reduction of severely telescoped nasofrontal ethmoidal fractures is difficult to achieve. In an effort to improve the results, a technique involving open reduction and external fixation was performed on three patients. These cases, along with ten other nasofrontal ethmoidal fractures treated between 1971 and 1980, are presented, with special emphasis in methods of reduction, stabilization, and results. The method of open reduction using an osteoplastic frontal sinus flap with external fixation gave the best overall results.

Giant cell tumor and aneurysmal bone cyst of the talus: clinicopathological review and two case reports.

Two cases of giant cell tumor and aneurysmal bone cyst of the talus were analyzed with a review of eight cases from the literature. This anatomic setting represents a distinct clinical and roentgenographic entity with a less aggressive natural history than the more common sites. All lesions presented a characteristic radiographic involvement of the head and neck of the talus. Eight of ten lesions were treated by curettage, with or without bone grafting, and healed. Two underwent partial en bloc resection. None developed late fracture, recurrence, avascular necrosis, or metastases. We concluded

Fractures of the adult femur excluding the femoral head and neck: a review and evaluation of current therapy.

A review and appraisal of current methods of treatment of fractures of the femur including operative and nonoperative methods as well as complications or limitations of various methods, reveals that a specific appliance or procedure is of value only when used with proper indication and proper technique. When internal fixation is performed, load-sharing between appliance and bone is mandatory, because of the large mechanical stresses involved. Intramedullary fixation permits load-sharing more often than extramedullary fixation and should be used on femoral fractures whenever possible. Conservat

Hip injuries in children and adolescents.

Hip injuries in children present a wide spectrum of problems. Frequently because of the severity of the trauma involved, other injuries may take precedence and may require modification of the usual approaches to treatment. However, certain precepts are essential to the successful treatment of these injuries and should be kept in mind regardless of the milieu in which they are found. In the child with a hip dislocation, the potential presence of an acetabular, femoral head, femoral shaft, patellar or tibial plateau fracture must always be considered. Specialized x-ray views are necessary for th

The National Football Head and Neck Injury Registry. Report and conclusions 1978.

The National Football Head and Neck Injury Registry has documented 1,129 injuries since 1971 that involved hospitalization for more than 72 hours, surgical intervention, fracture-dislocation, permanent paralysis, or death. Of this group of injuries, 550 were fracture-dislocations of the cervical spine, of which 176 were associated with permanent quadriplegia. It appears that during the last two decades, there has been a decrease in the incidence of direct fatalities, head injuries associated with intracranial hemorrhage, and injuries associated with death. Conversely, cervical spine injuries w

Temporal bone fractures in children.

Fifty children with temporal bone fractures were treated during a 40-month period. The diagnosis and management of temporal bone fractures in children is reviewed. The author's cases are classified as to longitudinal and transverse fractures, and the figures are presented for age distribution, sex, cause, loss of consciousness, CSF otorrhea, hearing loss, permanent tympanic membrane perforations, and facial nerve involvement. The author has followed a conservative method of management. Permanent impairment was not frequent, although all the patients with transverse fractures suffered total los

Limb fractures in a defined population. II. Orthopedic treatment and utilization of health care.

This study describes the orthopedic treatment and utilization of health care obtained by 2,333 patients in the population of Rochester, Minnesota, who suffered 2,519 limb fractures during the period 1969 through 1971. Overall, 24% of fracture occurrences required patient hospitalization, the remainder involving care on an ambulatory basis only. The mean number of physician visits was 4.5 per fracture, with the visits occurring during an interval of 103 days from the time of first evaluation. Fifteen percent of limb fractures were subject to at least one surgical operative procedure as part of

Unilateral lateral mass compression fractures of the axis.

Lateral mass compression fractures of the axis involve compression of the pedicle and displacement of the overlying facet, with or without an accompanying fracture of the odontoid. These unilateral fractures are not rare. Correlation is made between clinical cases and cadaver material with analysis of microradiographs. Criteria for recognition of the resultant healed fracture deformity include: (1) Tilting of the odontoid; (2) Loss of the odontoid shoulder on the affected side; (3) Increased downward slope of the affected facet; (4) Lateral compression, possibly with buckling; (5) Asymmetrical

A clinical-radiographic classification of skull base lesions.

Lesions involving the base of the skull can be divided into primary and secondary disease processes. The primary group consists of bone lesions more often seen elsewhere in the skeletal system while the secondary group consist of those lesions extending to the base of the skull by direct extension from adjacent intracranial or extracranial sites. The purpose of this paper will be to arrive at a classification of skull base lesions based on the roentgen localization of the disease process. The base of the skull because of its anatomically variable curvature can be difficult to evaluate on good

Epidemiology of traumatic and nontraumatic osteonecrosis.

In reviewing the epidemiology of traumatic and nontraumatic osteonecrosis the only common denominator appears to be that of circulatory compromise. Causative mechanisms seem to be quite distinct. A number of factors may predispose to postfracture hip necrosis, i.e. the type of fracture, the quality of reduction and union of the fracture. For postdislocation necrosis, causative factors include the type of dislocation as well as the timing and nature of the reduction. For nontraumatic osteonecrosis, an array of consistently associated systemic disorders appears to be largely linked by frequent l
