The development of a computational stress analysis of the femoral head. Mapping tensile, compressive, and shear stress for the varus and valgus positions.

Using a computer-based, two-dimensional finite-element analysis which has wide application to problems involving the hip joint, alterations in the distribution of stress in the femoral head consequent to varus and valgus osteotomy were studied. The mathematical model used in this analysis incorporated experimentally measured spatial variations in the stiffness of the bone of the femoral head and neck. These variations led to patterns of load transmission that were strikingly different from those in a homogeneous material. Because of their lower stiffness, the central region of the head and the

Treatment of radiation-induced tissue injury by hyperbaric oxygen.

Hyperbaric oxygen therapy appears to be a beneficial adjunctive treatment modality in the management of radionecrosis of bone and soft tissue in the head and neck. The mechanism of such wound healing enhancement appears to be related to oxygen stimulation of fibroblastic activity and neovasculation. In this pilot study, involving 13 cases of refractory mandibular radionecrosis treated with hyperbaric oxygen, complete healing of soft tissue disease with covering of exposed bone has been noted in seven cases 11 to 27 months posttreatment; transient healing was seen in three cases; moderate to ma

225 skateboard injuries in children.

Among 225 skateboard accidents treated at the Children's Hospital, Sheffield, in 8 months from August 1977, there were 92 fractures (40.9 per cent), of which 19 required manipulation under general anesthetic. Most involved the upper limb. Next most frequent were lacerations, bruises, abrasions, and injuries of joints and soft tissues. Four children had concussions (one with a fractured skull), 8 others had minor head injury, and 23 had soft tissue injuries of the head and neck. As for the reasons for the accidents, 20 per cent were due to hitting a stone, 10 per cent to a hole or bump in the p

Carotid-cavernous sinus fistula after trauma to the head.

The similarities between the various conditions described are remarkable. The orbital or frontal bruit and pulsating exophthalmos are common only to the carotid-cavernous fistula. A recent history of dental treatment, of facial infection with an elevated white blood cell count, and leukocytosis suggests cavernous sinus thrombosis. The superior orbital fissure syndrome and orbital apex syndrome occur most frequently in conjuction with Le Fort II and III fractures, which can involve the optic foramen and superior orbital fissure, therefore disrupting the sensory and motor innervation to the orbi

Spinal injuries in belt-wearing car occupants killed by head-on collisions.

In 34 post-mortem examinations of car occupants wearing seat belts and killed in straight or oblique head-on collisions, a thorough investigation of the spine was performed. The autopsy results were correlated with the findings in the cars in order to reconstruct the events when the occupant's body struck the interior of the car. In 2 cases the victims had worn lap belts, in 15 cases shoulder belts and in 17 cases combined shoulder-lap belts (three-point belts). In victims involved in head-on collisions while wearing lap belts, fractures of the neural arch of the axis were found which were pro

Injuries of the cervical spine in football.

The neck is a vulnerable area in contact sports in general and in football in particular. The type of injuries encountered often vary with the age and development of the players. Five well-established mechanisms of injury have been identified. Most injuries appear to occur during the act of tackling when the well protected head sustains violent trauma which is transferred directly to the neck. The most dangerous single mechanism is that of flexion, but lateral deviation, extension and impaction also have been identified as mechanisms of injury. The pattern of injuries would suggest that severa

Cervicocranium and the aviator's protective helmet.

An analogy based on the likenesses of the APH-6 aviator's protective helmet and the hangman's noose has some interesting relationships to aircraft accident investigation and to the associated traumatic pathology. A superficial examination of the similarities might propose that the inferior edge of the helmet, when considered part of the continuous circle completed by the nape strap and the chin strap, forms a loop which can be likened to a hangman's noose. The analogy might be extended to the comparison of lesions made about the neck by the straps or the edge of the helmet compared with abrasi

The pathology of Legg-Perthes disease and its comparison with aseptic necrosis.

A review of pathological material obtained in cases of Legg-Perthes disease and in aseptic necrosis in the adult shows some similarities in distribution of the involved segment of the femoral head. X-ray signs of separation of the central avascular segment from the superior lateral and medial viable segments of the femoral head are recognizable. Crushing in of the weight-bearing segment then occurs and is recognized by flattening of the femoral head outline. At this point the child is in danger of suffering lateral displacement of the viable lateral segment leading to an incongruous extruded f

Shall not refer in detail to fractures of the spinous or transverse processes.(2) Mechanics of injury to vertebrae. Two variables: (1) the nature of the bones; (2) the qualities of the force. Spinal injury usually caused by indirect violence.(3) The different results of injuries applied to the head; may break skull, failing that, the neck. Atlas fracture. Difference in qualities of the force causing atlas fracture and low cervical dislocation.(4) The compound nature of the verte
