pREBOA Abroad: Leveraging Partial Aortic Occlusion for Stabilization and Transport in Ukraine.

The war in Ukraine highlights the challenges in providing combat casualty care when both nations are near-peer adversaries. Providing timely definitive hemorrhage control within one hour of injury, as the US military and its allies have grown accustomed, is unrealistic. Stabilization points (SP) in Ukraine are forced to provide prolonged field care to exsanguinating patients with little resources while waiting extended periods of time for patient transport. pREBOA-PRO™ (Prytime Medical, Boerne, TX)has been suggested as a bridge to this problem as it can provide temporary hemorrhage control, st

Feasibility of computer-aided aortic occlusion catheter in non-traumatic cardiac arrest.

BACKGROUND: The persistently high mortality rate following cardiac arrest over the past decades underscores the urgent need for advancements in resuscitation strategies. Animal studies have shown increased coronary and cerebral perfusion during cardiopulmonary resuscitation following endovascular balloon occlusion of the aorta. METHODS: This single-arm study investigated the feasibility and safety of a novel computer-aided automated aortic occlusion catheter (NEURESCUE® Catheter & NEURESCUE® Assistant) as an adjunct to advanced cardiac life support in patients with non-traumatic cardiac arres

Fourteen-year outcome of unilateral leg replantation after bilateral lower leg and unilateral upper extremity amputation following traumatic injury.

In cases of traumatic major extremity amputations, particularly in the lower extremity, every stage-ranging from decision-making to implementation and outcomes-remains a topic of debate. Managing major amputation cases with high mortality rates is extremely challenging both during the injury and treatment processes. We present a rare and severe case of a 30-year-old patient who sustained injuries from a concrete machine, resulting in bilateral lower extremity amputations and a unilateral proximal arm amputation. We successfully performed a replantation at the proximal ankle level on one side.

Global Overview of Acquired Upper Limb Amputation Epidemiology: A Systematic Review of Prevalence, Incidence, Level, and Etiology.

Upper limb amputation leads to significant physical and psychosocial burden; however, it remains underrepresented in global epidemiological and rehabilitation studies. This study aimed to systematically review the global epidemiological data on upper limb amputation, focusing on its prevalence, incidence, anatomical levels, and etiology. According to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, a comprehensive literature search was conducted using PubMed, Embase, Cochrane Library, and Regional Information Sharing Systems (up to May 2023). Associations Between Fracture Characteristics and Surgical Outcomes in Distal Digit Replantation of Tamai Zones 1-2.

BACKGROUND: Although numerous risk factors were reported for the failure of digit replantation, there was a lack of studies investigating the association between replantation outcomes and fracture characteristics. The purpose of this study was to evaluate the survival rate and functional outcomes of distal digit replantation based on the fracture patterns. METHODS: We retrospectively reviewed patients with Tamai zones I-II amputated digits who underwent replantation between 2018 and 2022. The fracture characteristics of the distal phalanx were compared to outcomes, including the survival rate

A multidisciplinary emergency protocol reduces revascularization time in major upper and lower limb replantations.

BACKGROUND: Major limb amputation salvage procedures exhibit an increased risk of failure when revascularization is delayed beyond 360 min. Institutional delays persist as critical barriers, even with advancements in surgical techniques. METHODS: Retrospective cohort study (November 2022- December 2024) at Level I Trauma Center. We implemented a systematized emergency protocol featuring: Prehospital activation → Green channel → OR-direct transport Parallel processing → revascularization. PRIMARY OUTCOME: Revascularization time (limb arrival → arterial flow). RESULTS: 30 consecutive amputees

Brief Ectopic Banking and Immediate Plate Fixation With Free Omental Flap Addresses Multiple Fundamental Problems for Single-Stage Replant of Transhumeral Amputation.

High-velocity traumatic amputations of the proximal upper extremity are devastating to the patient and represent an extreme surgical challenge to the treatment team. The hand surgeon must simultaneously battle devascularization with timely microvascular anastomosis, gross contamination with meticulous debridement, and amputation with stable fixation. In restoring a functional extremity, many of these goals are in contention with each other. This case describes a novel management strategy for a left transhumeral amputation sustained by a 23-year-old man in a motorcycle accident. The amputated a

Concurrent validity study of QuickDASH with respect to DASH in patients with traumatic upper extremity amputation.

BACKGROUND: The Disability of the Arm, Shoulder and Hand Outcome Measure (DASH) is a validated patient-reported outcome measure (PROM) for many upper extremity musculoskeletal disorders. In patients with severe traumatic conditions, limited evidence exists regarding the equivalence between DASH and its shortened version, QuickDASH, which is more feasible in clinical practice. The rationale of this study was to analyze the concurrent validity of QuickDASH with respect to DASH in patients with traumatic upper extremity amputation. METHODS: This study is based on a consecutive cohort of traumati

The failed adult traumatic brachial plexus reconstruction.

Traumatic adult brachial plexus injuries typically cause immediate loss of upper limb function. Timely multidisciplinary treatment in specialized centres often results in a useful helper arm. Both the patient and the surgical team can benefit from an open discussion to set realistic expectations. Surgical reconstruction is customized for each patient, considering their injury factors and functional objectives. Optimizing pain control, adherence to procedure indications and using meticulous surgical techniques help minimize the risk of failing to meet the patient's goals. The need for potential

Muscle recruitment during gait in individuals with unilateral transfemoral amputation due to trauma compared to able-bodied controls.

Individuals with transfemoral lower limb amputations walk with adapted gait. These kinetic and kinematic compensatory strategies will manifest as differences in muscle recruitment patterns. It is important to characterize these differences to understand the reduced endurance, reduced functionality, and progression of co-morbidities in this population. This study aims to characterize muscle recruitment during gait of highly functional individuals with traumatic transfemoral amputations donning state-of-the-art prosthetics compared to able-bodied controls. Comparison of Hand Function after Single-Digit Replantation in Elderly and Younger Patients.

Background: With ageing of the population, there is an increasing likelihood that elderly people will seek employment, with a resultant increase in the incidence of hand injuries, including digital amputations. However, the surgical indication for replantation of an amputated digit in elderly patients is controversial. The purpose of this study was to compare functional outcomes after single-digit replantation of the thumb to long finger in older (≥65 years old) and younger (<65 years old) patients to assess the feasibility of digit replantation in the elderly population. Methods: A retrospect

Heterodigital flap as a solution for a thumb defect: a case report.

Thumb traumatic injuries are incredibly common in hand injuries. The thumb is essential to hand function in order to do daily tasks like gripping, holding, opposing, circumducting, and movements. As a result, compared to injuries to other fingers, a thumb injury significantly impairs hand function. Traumas can cause soft tissue loss linked to vascular injuries that require revascularization. Replantation is the surgical treatment most frequently suggested to patients who have had their thumbs amputated in an attempt to restore function and attractiveness. There are alternative reconstructive t

[MEP-11] Replantation of Traumatic Limb Loss: A Case Report of Multidisciplinary Approach with Cardiopulmonary Bypass Technique for Ischemic Perfusion.

Reimplanting a traumatically amputated limb requires minimal ischemia duration with adequate perfusion at low temperatures. Cardiopulmonary bypass (CPB) techniques play a significant role in the limb salvage algorithm in reducing ischemia time, allowing planning, and preventing reperfusion injury. A 23-year-old female was admitted to the emergency department due to a traffic accident. The right arm at the shoulder and the left arm at the elbow were amputated. The patient was initially taken into surgery due to injuries to the liver and spleen. During this process, a CPB circuit was used to per
