Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Vascular venous anatomy of the calcaneal fillet flap for tibial stump reconstruction: Anatomical study focused on reduction of vascular complications.

INTRODUCTION: Traumatic lower-limb amputations and severe leg injuries may necessitate below-knee amputation when replantation or reconstruction is not feasible. In these situations, tibial bone and soft tissue may be insufficient to preserve the knee. The use of tissue from the amputated part, particularly the foot fillet flap, can allow knee salvage with significant functional benefit. The foot fillet flap is a composite flap that can be designed as pedicled or free, depending on injury characteristics. However, venous insufficiency remains a potential complication. This study investigated t

Contemporary management and time to revascularization in upper extremity arterial injury.

INTRODUCTION: Upper extremity arterial injury is associated with significant morbidity and mortality for trauma patients, but there is a paucity of data to guide the clinician in the management of these injuries. The goals of this review were to characterize the demographics, presentation, clinical management, and outcomes, and to evaluate how time to intervention associates with outcomes in trauma patients with upper extremity vascular injuries. METHODS: The National Trauma Data Bank (NTDB) Research Data Set for the years 2007-2016 was queried in order to identify adult patients (age ≥ 18) w

Development of overuse musculoskeletal conditions after combat-related upper limb amputation: a retrospective cohort study.

STUDY DESIGN: Retrospective cohort study INTRODUCTION: Service members who have sustained traumatic amputations are typically young and otherwise healthy. Beyond standard care, these individuals desire long, highly active, and relatively pain-free lifestyle, whether that is returning to active duty or transitioning to civilian life. Development of overuse musculoskeletal conditions could have a significant influence on quality of life for Service members with traumatic upper limb amputation. PURPOSE OF THE STUDY: Compare one-year incidence of overuse musculoskeletal injuries in Service member

Outcome Assessments of Patients with Posttraumatic "Ultra-Time Vascular Injuries" of the Extremities.

The management of posttraumatic vascular injury that presents after 8 h, or "ultra-time vascular injury", is daunting, and inciting recognition of this injury is vital. We retrospectively analyzed 29 patients with ultra-time vascular injuries to determine the patients' demographic characteristics and identify the determinants for amputation and disability. The age distribution of the high-risk population was from 18 years to 40 years, which indicated that these patients had plenty of productive life remaining. Injuries to the lower limbs (79.31%) were over four times more common than injuries

Pelvic, spinal and extremity wounds among combat-specific personnel serving in Iraq and Afghanistan (2003-2011): A new paradigm in military musculoskeletal medicine.

INTRODUCTION: Previous studies regarding musculoskeletal injuries sustained during war have been limited by a lack of specificity regarding wounds incurred by combat-specific personnel. This investigation endeavoured to develop a comprehensive catalogue of the extent of musculoskeletal trauma, as well as the frequency of distinct injuries, among soldiers possessing a single combat-specific specialty. METHODS: The Department of Defense Trauma Registry (DoDTR) and the Armed Forces Medical Examiner System (AFMES) were queried for all individuals with the combat-specific designation of cavalry sc

Amputee mobility predictor-bilateral: a performance-based measure of mobility for people with bilateral lower-limb loss.

The Amputee Mobility Predictor (AMP) is an outcome measure designed to assess mobility and functional capabilities for people with unilateral lower-limb amputation. No comparable measure exists for those with bilateral lower-limb amputation (BLLA). The purpose of this study was to examine the utility of the AMP-Bilateral (AMP-B) to measure the ability to perform functional tasks related to participation in advanced skill activities in those with BLLA and to determine whether AMP-B scores correlated with 6-minute walk test (6MWT) performance. Twenty-six male servicemembers (SMs) completed the s

Rehabilitation in a patient with replantation of amputated distal leg.

BACKGROUND: Many studies have been reported on replantation of an upper extremity after traumatic amputation, but there are only a few case reports on replantation of the lower extremities rather than amputation surgery. This is due to preoperative factors, complications and the possibility of good prosthetic substitution. We describe here the dynamic evaluation necessary for a continuous rehabilitation treatment in a subject with replanted left foot after traumatic amputation. CASE REPORT: A 58-year-old male person with amputation of the left leg at the level of the left talocrural joint, wit

Department of Veterans Affairs compensation and medical care benefits accorded to veterans with major limb loss.

Veterans injured in theaters of combat operations are eligible for benefits, including medical care and compensation. This article describes veterans with service-connected disability for major lower- and/or upper-limb loss resulting from combat-field-associated injuries sustained in the Vietnam war, Operation Desert Shield/Operation Desert Storm, and Operation Iraqi Freedom/Operation Enduring Freedom (OIF/OEF). Using the Department of Veterans Affairs (VA) Compensation and Pension Mini-Master file, we identified 2,690 veterans who in August 2007 received compensation for loss of one or more l

Estimating the prevalence of limb loss in the United States: 2005 to 2050.

OBJECTIVE: To estimate the current prevalence of limb loss in the United States and project the future prevalence to the year 2050. DESIGN: Estimates were constructed using age-, sex-, and race-specific incidence rates for amputation combined with age-, sex-, and race-specific assumptions about mortality. Incidence rates were derived from the 1988 to 1999 Nationwide Inpatient Sample of the Healthcare Cost and Utilization Project, corrected for the likelihood of reamputation among those undergoing amputation for vascular disease. Incidence rates were assumed to remain constant over time and ap

Dissociation of phantom limb phenomena from stump tactile spatial acuity and sensory thresholds.

Most amputees experience phantom limb sensations and/or phantom limb pain as well as residual limb (stump) pain that are resistant to treatment. Phantom phenomena are not homogeneous; each patient presents with a unique combination of spontaneous or evoked sensations, pain, and/or awareness. In an effort to understand the underlying mechanisms, postamputation pain has been subclassified based on the perceived sensory qualities reported by the individual. However, little is known about the relationship between subjective phantom phenomena and sensory function of the residual stump. The aim of t

[Traumatic vasculo-orthopedic combined lesions: 18-years retrospective evaluation of epidemiology and risks factors for amputation].

This study was undertaken to assess retrospectively the epidemiology and risk factors for amputation of combined vasculo-orthopaedic traumatic lesions, during a 18-year period, from March 1987 to May 2005, comprising the review of the clinical charts of 149 patients. The series includes a predominance of male patients (84%) with an average age of 34 years. Eighty-five per cent of the lesions resulted from traffic accidents (49% velocipedes, 39% automobiles, 12% trampling), 10% were consequence of falls and 5% resulted from agriculture activities. Orthopaedic lesions include 83 fractures of upp

Management of the multiple limb amputee.

ISSUE: Multiple limb amputations involving at least one upper extremity are very uncommon. The amputation of both an upper and lower limb is even more uncommon. Due to the rarity of these amputations therapists are uncertain regarding the most appropriate treatment methods. While the majority of the protocols used for single limb amputations are appropriate for these multiple limb amputees, there are differences. Loss of multiple limbs creates a problem of overheating for the individual. Loss of an arm and leg results in difficulty donning the prostheses and difficulty using crutches and paral

Mutilating shredder/chipper hand injuries.

Outdoor recreational and vocational hand injuries occur with a predictable seasonal fluctuation and regional variation. An increasing demand in less urban areas for efficient yard and garden waste removal, combined with the availability of more affordable shredder/chipper machines, has increased their utilization. Potential hazards have been well recognized, but, not surprisingly, this behavior pattern has concomitantly introduced another characteristic syndrome of self-inflicted operator injury most prevalent in the fall and spring months. The seriousness of the possible upper extremity mutil
