Functional and subjective outcomes after surgical management of complex elbow dislocations: a retrospective study.

INTRODUCTION: Dislocations of the elbow are the second most frequent upper-body injury after shoulder dislocations, comprising 11-28% of all elbow injuries. Complex elbow dislocations pose challenging management due to the involvement of critical stabilizing structures. This study aimed to investigate functional and subjective outcomes (MEPS, DASH, Oxford score) in 44 patients with complex elbow dislocations who underwent surgery between 2018 and 2020, with subgroup analysis focusing on gender and age differences. MATERIAL AND METHODS: A retrospective analysis was conducted on patients treate

Generation of soluble, immunoreactive recombinant JEV E protein through a simplified inclusion body extraction and refolding approach in vitro.

Japanese encephalitis (JE) is a mosquito-borne infectious disease caused by the Japanese encephalitis virus (JEV). There is currently no effective treatment for JE, and all approved Japanese encephalitis vaccine products originated from the JEV genotype III (GIII). In recent years, JEV genotype I (GI) has gradually replaced GIII as the dominant genotype, and a new symptom of peripheral nerve injury (PNI) caused by JEV NX1889 strain has attracted wide attention, in which JEV envelope (E) protein may be involved in early peripheral nerve injury. Here, the envelope (E) gene of JEV GIb, NX1889 str

An Overview of Recent Advances in the Neuroprotective Potentials of Fisetin against Diverse Insults in Neurological Diseases and the Underlying Signaling Pathways.

The nervous system plays a leading role in the regulation of physiological functions and activities in the body. However, a variety of diseases related to the nervous system have a serious impact on human health. It is increasingly clear that neurological diseases are multifactorial pathological processes involving multiple cellular systems, and the onset of these diseases usually involves a diverse array of molecular mechanisms. Unfortunately, no effective therapy exists to slow down the progression or prevent the development of diseases only through the regulation of a single factor. To this

Acetaminophen and pregabalin attenuate central sensitization in rodent models of nociplastic widespread pain.

The "nociplastic pain," a recently proposed novel mechanistic pain descriptor, is defined as pain occurring through altered nociception without nociceptor activation and nerve injury. Nociplastic pain is often characterized by widespread pain sensitization (WSP) in multiple body regions (Fitzcharles et al., 2021). As many patients with primary chronic pain would have nociplastic backgrounds, developing appropriate methods to evaluate drug effects against nociplastic pain in animal model is in great demand. Using two rat models with the WSP involving central amygdala (CeA) activation by orofaci

RXRα Blocks Nerve Regeneration after Spinal Cord Injury by Targeting p66shc.

Nerve regeneration after spinal cord injury is regulated by many factors. Studies have found that the expression of retinoid X receptor α (RXRα) does not change significantly after spinal cord injury but that the distribution of RXRα in cells changes significantly. In undamaged tissues, RXRα is distributed in motor neurons and the cytoplasm of glial cells. RXRα migrates to the nucleus of surviving neurons after injury, indicating that RXRα is involved in the regulation of gene expression after spinal cord injury. p66shc is an important protein that regulates cell senescence and oxidative stres

Osteochondral Autograft Transplantation for Capitellar Osteochondritis Dissecans.

BACKGROUND: The goal of the osteochondral autograft transplantation (OAT) procedure is to replace both the bone and cartilage that have been compromised by osteonecrosis of the capitellum, a condition known as osteochondritis dissecans (OCD). In children, the vascularity of the capitellum is limited compared with that in adults because the physis acts as a physical barrier to vascular ingrowth from the metaphysis to the epiphysis. The necrotic subchondral bone cannot keep up with the weight-bearing demands of certain high-level athletes such as gymnasts, accumulating microfractures and eventua

Bilateral Brachial Plexus Injury After MiraDry Procedure for Axillary Hyperhidrosis.

BACKGROUND: Multiple treatments are available for primary axillary hyperhidrosis including noninvasive, microwave-based thermal treatments designed to destroy sweat glands in the axilla. Often these procedures involve local anesthetic injection to the axilla, followed by placement of the microwave emitter onto the skin and applying the heat treatment to varying depths of the subcutaneous tissues. CASE DESCRIPTION: A 49-year-old, thin, active woman (body mass index 19.6) underwent microwave-based treatment to the bilateral axillary regions. She experienced an electric sensation into the ulnar

Changing presentation of knee dislocation and vascular injury from high-energy trauma to low-energy falls in the morbidly obese.

OBJECTIVE: Reports in the literature of low-energy (LE) knee dislocation (KD) in obese patients have been increasing. This study was undertaken to define the risk factors for KD by LE mechanisms and the outcomes of these patients compared with those with high-energy (HE) trauma. METHODS: All patients with a complete KD presenting to the emergency department of a large urban level I trauma center were reviewed. Patient information collected included age, sex, weight, height, body mass index (BMI), injury mechanism, neurovascular and orthopedic injuries, and operations performed to treat vascul

Complications of regional anesthesia and acute pain management.

Perioperative nerve injuries are recognized as a complication of regional anesthesia. Although rare, studies suggest the frequency of complications is increasing. Risk factors include neural, traumatic injury during needle or catheter placement, infection, and choice of local anesthetic solution. Neurologic injury due to pressure from improper patient positioning, tightly applied casts or surgical dressings, and surgical trauma are often attributed to regional anesthetic. Body habitus and preexisting neurologic dysfunction may also contribute. The safe conduct of regional anesthesia involves k

Selective source blocking for Gamma Knife radiosurgery of trigeminal neuralgia based on analytical dose modelling.

We have developed an automatic critical region shielding (ACRS) algorithm for Gamma Knife radiosurgery of trigeminal neuralgia. The algorithm selectively blocks 201 Gamma Knife sources to minimize the dose to the brainstem while irradiating the root entry area of the trigeminal nerve with 70-90 Gy. An independent dose model was developed to implement the algorithm. The accuracy of the dose model was tested and validated via comparison with the Leksell GammaPlan (LGP) calculations. Agreements of 3% or 3 mm in isodose distributions were found for both single-shot and multiple-shot treatment plan

Opioids in chronic pain.

The advance in our understanding of the biogenesis of various endogenous opioid peptides, their anatomical distribution, and the characteristics of the multiple receptors with which they interact open a new avenue for understanding the role of opioid peptide systems in chronic pain. The main groups of opioid peptides: enkephalins, dynorphins and beta-endorphin derive from proenkephalin, prodynorphin and proopiomelanocortin, respectively. Recently, a novel group of peptides has been discovered in the brain and named endomorphins, endomorphin-1 and -2. They are unique in comparison with other op

Effects of acute and chronic midthoracic spinal cord injury on neural circuits for male sexual function. I. Ascending pathways.

Normal male reproductive function, particularly ejaculation, requires the integrity of urogenital sensory input and its ascending spinal projections. After midthoracic chronic spinal cord injury, sexual dysfunction occurs in both rats and humans. Neurons in the medullary reticular formation (MRF) are involved in the processing of bilaterally convergent sensory inputs from multiple cutaneous, mucocutaneous, and visceral regions of the body, including the penis and male urogenital tract. A variety of acute and chronic lesions were used to determine the midthoracic location of ascending spinal pa
