Small Bowel Obstruction due to Hemobezoar Following Roux-En-Y Gastric Bypass: A Case Report.

Roux-en-Y Gastric Bypass (RYGB) is a widely performed bariatric surgery with well-documented benefits, but it can lead to rare complications such as small bowel obstruction (SBO). While internal hernias and adhesions are common causes, hemobezoar formation is an extremely rare etiology. We report two cases of acute SBO due to hemobezoar following laparoscopic RYGB. Both patients presented with severe nausea within the early postoperative period. CT imaging revealed marked small bowel distension in both cases, prompting surgical intervention. Diagnostic laparoscopy confirmed the presence of obs

Case Report: Strangulated intestinal obstruction due to chronic migration of an intrauterine device (IUD): a 30-year latent complication.

Intrauterine devices (IUDs) are widely used but carry rare risks of migration and subsequent complications, such as bowel obstruction. This case highlights the life-threatening potential of chronic IUD migration decades after insertion, emphasizing the need for heightened clinical vigilance and long-term surveillance. A 57-year-old female patient presented to the hospital with a 2-day history of abdominal pain, accompanied by the cessation of flatus and defecation. She had one pregnancy and one vaginal delivery 31 years ago, followed by the insertion of a ring-shaped IUD 1 year postpartum. How

Small but high risk: a 10-year single-hospital study on sharp gastrointestinal foreign bodies-the case of the jujube pit.

BACKGROUND: Guidelines from Europe and the United States concur that sharp foreign bodies (FBs) whose length exceeds 2.5 cm seldom traverse the pylorus and are associated with serious complications. To determine whether jujube pits conform to this rule, we investigated the clinical course of patients with jujube pit impaction, characterized the patient populations at greatest risk, and compared the diagnostic performance of barium esophagography (BE) and multidetector computed tomography (MDCT). METHODS: A single-center retrospective study was conducted on cases of jujube pit impaction treate

Accuracy of Computed Tomography in Identifying the Etiology of Small Bowel Obstructions in Virgin Abdomens at a Tertiary Care Military Treatment Facility.

IntroductionSmall bowel obstructions (SBO) account for up to 50% of emergent laparotomies. Computed tomography (CT) aids in diagnosing SBO by identifying transition points and potential etiologies, though accuracy varies. SBO in a virgin abdomen (SBO-VA) presents unique diagnostic challenges. This study assesses CT accuracy in identifying SBO-VA etiologies in patients at a tertiary military treatment facility to help guide future management strategies.MethodsThis retrospective study reviewed SBO-VA cases from 2005-2021 with IRB approval. Patients were identified via ICD codes and included if ≥

Bowel Obstruction Caused by Migration of an Intra-Gastric Balloon: A Rare Case Treated Laparoscopically.

BACKGROUND Endoscopic bariatric therapies, such as intra-gastric balloons (IGB) offer a less invasive, efficacious, and safe approach to facilitate weight loss in obese individuals. While typically tolerated, rare complications, including intestinal migration of the IGB, can occur and lead to serious sequelae such as mechanical bowel obstruction. This case report details the diagnostic process and successful laparoscopic surgical management of an unusual instance of IGB migration resulting in small bowel obstruction. CASE REPORT We present the case of a 32-year-old woman who developed a small

[Peripheral ulcerative keratitis associated with dupilumab: a case report].

A 68-year-old male developed foreign body sensation in both eyes, photophobia and tearing in the right eye approximately 6 weeks after initiating dupilumab for severe atopic dermatitis. The right eye presented a crescent-shaped superficial-stromal ulcer in the peripheral cornea with an undermined edge, which was separated from the limbus of the cornea by a clear zone. The left eye had a pannus at the limbus of the cornea. Corneal scraping, bacterial culture, and fungal culture excluded infections. The patient was administered 0.1% fluorometholone eye drops and recombinant bovine basic fibrobla

Phaeoisaria sp. fungal keratitis resembling corneal rust residue: a case report.

BACKGROUND: Fungal keratitis can develop after plant injury or after prolonged glucocorticoid use. Typical manifestations include corneal infiltrates, satellite lesions, plaques, and an immune ring. Some cases exhibit atypical signs, requiring reliance on etiological examination. Notably, fungi previously deemed nonpathogenic to humans can cause keratitis with rare clinical manifestations. This report details the clinical signs and successful treatment outcomes of keratitis caused by Phaeoisaria sp. CASE PRESENTATION: A 51-year-old man visited the ophthalmology clinic with ongoing eye pain an

Preoperative septic peritonitis, hypotension, and reason for surgery are risk factors for small intestine dehiscence in dogs: a directed acyclic graph approach.

OBJECTIVE: To evaluate risk factors for postoperative intestinal dehiscence, adjusting for potential confounders identified through directed acyclic graphs (DAGs). METHODS: A retrospective cohort study was conducted of dogs undergoing small intestinal surgery at a private referral center between July 2011 and July 2024. Directed acyclic graphs were used to identify risk factors, confounders, and mediators, with regression models developed to adjust for these confounders. RESULTS: A total of 55 dogs were included in the study. Of these, 11 dogs (20%) developed postoperative intestinal dehisce

Intraluminal causes of mechanical small bowel obstruction: CT findings and diagnostic approach.

Intraluminal causes of small bowel obstruction (SBO) are less common than mural or extrinsic etiologies. This review categorizes intraluminal causes of SBO into four broad categories to provide a diagnostic framework for radiologic interpretation: 1) ingested contents, 2) bowel stasis, 3) inflammatory causes, and 4) neoplasms. Ingested materials can result in SBO when individual or accumulated contents are too large to pass, such as in the case of foreign bodies or bezoars. Bowel stasis causing SBO can be secondary to abnormal bowel function, such as in cystic fibrosis, reduced transit of cont

Abdominal Wall Abscess Caused by Small Intestinal Penetration That Was Difficult to Distinguish from a Malignant Tumor: A Case Report.

INTRODUCTION: Fish bone ingestion is common but rarely causes complications such as abdominal wall abscesses, which can mimic malignancies such as sarcomas on imaging. Abscesses require drainage and antibiotics, while sarcomas need wide excision. Therefore, the differentiation between abscesses and sarcomas is important and often requires multidisciplinary involvement. CASE PRESENTATION: A 70-year-old woman presented with anorexia and a painful abdominal mass. Laboratory tests showed inflammation but normal tumor marker concentrations. The abdominal wall mass was hard and poorly mobile. Ultra

Trichobezoar-induced intestinal obstruction-complete bowel obstruction in a young female with underlying psychiatric disorders: A case report.

INTRODUCTION: A bezoar is a mass of undigested or partially digested material in the gastrointestinal tract, with trichobezoar being the second most common type. It is primarily composed of hair and can lead to complications such as bowel obstruction, ulceration, perforation, and intussusception. Early diagnosis is challenging due to vague symptoms, making it critical to have a high index of suspicion. The inclusion of sanitary pad within the trichobezoar makes this case unique. CASE PRESENTATION: A 26-year-old Ugandan female presented with features of complete bowel obstruction. Contrast Enh

Magnetic removal of fragmented Crawford hook during nasolacrimal duct probing.

We present the case of a 13-month-old girl with Down syndrome who underwent probing to alleviate nasolacrimal duct obstruction (NLDO). During the procedure, a distal portion of the deployed Crawford hook fragmented and became lodged in the right nasopharynx. Traditional intraoperative methods of visualization were unsuccessful; however, extraction of the fragment was achieved by using a nasogastric bridle magnet. This case demonstrates a novel method of metallic foreign body removal, which may be useful when exact location cannot be determined.
