Small bowel magnetic compression anastomosis creation for bypass procedures in a porcine model.

BACKGROUND: Stapled and hand-sewn techniques dominate gastrointestinal anastomotic procedures. These techniques are effective but not without flaws. Retained foreign bodies, pathways from mucosa to serosa, and increased scar tissue are some of the drawbacks, and can lead to postoperative complications. The GI Windows Flexagon™ system utilizes self-forming magnets (SFM's) to create anastomoses by compression, sealing serosa to serosa, leaving no foreign bodies. Combining the Flexagon™ SFM with the OTOLoc™ device (implant with a central lumen which provides radial support to the enterotomies), e

Small Intestine Perforation Due to an Intubation Stylet Fragment: A Case Report.

This case highlights a rare complication of endotracheal intubation: intestine perforation caused by an intubation-stylet fragment. A patient undergoing an appendectomy had an esophageal intubation on the first attempt and a successful endotracheal intubation on the second, using the same stylet. The missing fragment went unnoticed after intubation. Postoperative symptoms led to reimaging, revealing a foreign body in the intestine, later identified as a stylet fragment. The stylet fragment was removed, and the patient recovered by postoperative day 7. This case emphasizes the importance of vig

Ileal bezoar secondary to ileal duplication - a case report.

Ileal bezoars usually occur secondary to gastric bezoars, gastric or bowel surgery, or lumen-narrowing lesions of the ileum. Primary ileal bezoars, being rare, are diagnosed only after excluding secondary causes. Ileal duplication associated with bezoar has seldom been reported.

Medication Blisters: A Rare Cause of Bowel Perforation.

Foreign body ingestion is a common occurrence in vulnerable populations, predominantly at the ends of the age spectrum. While most cases are uncomplicated, some patients may require endoscopic or surgical intervention to prevent serious complications. Therefore, accurate and detailed radiological evaluation is essential for therapeutic decision-making. We present the case of an 80-year-old woman presenting with abdominal pain and radiological evidence of a foreign body in the small intestine. A conservative treatment approach was initially proposed due to the absence of complications and the h

Unpacking the dangers of super absorbent polymer water beads: an in vitro analysis.

OBJECTIVES: This study aims to investigate the risk of pediatric bowel obstruction from various types of super absorbent polymer (SAP) beads in different liquid media, explore treatment options, and develop a clinical decision-making algorithm for healthcare providers. METHODS: Three experiments were conducted involving the measurement of SAP beads' expansion in different liquid media. The first experiment examined the expansion of beads in tap water, gastric fluid, and small intestine fluid. The second compared the expansion of beads from six manufacturers in water. The third tested the effe

Hypertonic liquids: a potential method for reducing super absorbent polymer bead associated bowel obstructions in children.

PURPOSE: Super absorbent polymer (SAP) beads grow in the presence of liquids, leading to a risk of small bowel obstruction (SBO) following ingestion, but there is little data to guide management in children following SAP bead ingestion. We hypothesize that certain hypertonic liquids could limit the growth of the beads and shrink them, potentially serving as a treatment to prevent SBO. METHODS: Specific hypertonic liquids were identified a priori to be studied including 100% fruit juices, mineral oil, 50% and 100% Gastrografin, Pedialyte, Cola, polyethylene glycol, and Milk of Magnesia. First-in-Human Linear Magnetic Jejuno-Ileal Bipartition: Preliminary Results with Incisionless, Sutureless, Swallowable Technique.

BACKGROUND: Minimally invasive surgery may be further advanced with the novel biofragmentable magnetic anastomosis compression system. Two magnets may be swallowed, or placed by flexible endoscopy, in a side-to-side magnetic jejuno-ileostomy (MagJI) bipartition for weight and type 2 diabetes (T2D) reduction. MagJI markedly reduces the major complications of enterotomy, stapling/suturing, and retained foreign materials. METHODS: This was a prospective first-in-human investigation of feasibility, safety, and preliminary efficacy in adults with body mass index (BMI, kg/m2) ≥ 30.0- ≤ 40.0. After

Case of intestinal obstruction due to accidental ingestion of a silicone rubber dental impression.

Accidental ingestion is common and can even necessitate surgery. An 89-year-old man with dementia presented with abdominal distension and pain. A CT scan revealed a high-intensity object in the terminal ileum with distension of the proximal small intestine. Colonoscopy showed ileus caused by a retained foreign body (FB), which was removed via laparotomy the following day. Chemical analysis identified the material of the FB as platinum silicone rubber. Based on its material and irregular shape, the FB was determined to be a dental impression. The patient had dentures made one year prior, and co

Gossypiboma: the hidden surgical threat - delayed diagnosis and management.

Gossypiboma (retained surgical sponge) is a rare, unwanted, avoidable and embarrassing medical event that can complicate the favourable evolution of any surgery. It is a rare but serious complication seldom reported due to its medicolegal implications. Gossypiboma increases the cost of treatment, negatively affects the otherwise good prognosis of the index surgery, and also increases the patient's morbidity and mortality. It can occur after any surgery that requires the use of swabs. Intra-abdominal gossypiboma can migrate into the ileum, stomach, colon or bladder without any apparent opening i

Case Report: Suspected intestinal perforation caused by accidental ingestion of magnet.

INTRODUCTION: Children frequently swallow foreign objects, and ingesting magnets on occasion can cause major problems because they are composed of unique materials that draw to one another in the intestinal wall. As a result, there is a significant chance of intestinal wall necrosis, intestinal perforation, and fistula formation. On the basis of the history of magnet ingestion, clinical signs like nausea and vomiting, and x-ray imaging, it is easy to diagnose magnet-induced complications. However, there was no foreign body shadow in the x-ray, and some intermittent swallowing of magnets in dif

Small intestinal metastasis of hepatocellular carcinoma diagnosed and resected as intestinal obstruction caused by a foreign body.

Hematogenous metastases of hepatocellular carcinoma (HCC) to the small intestine are rare. In most cases, hematogenous metastasis is symptomatic, including perforation or bleeding, and the prognosis is poor. A 63-year-old man had lower right abdominal pain, and small intestinal obstruction due to chronic inflammation of the foreign body was suspected on computed tomography. Blood tests revealed elevated alpha-fetoprotein (AFP). He had undergone four surgeries for HCC. Conservative treatment was administered, but there was little improvement. A segmental resection of the small intestine was per

Regeneration of the Gastrointestinal Tract After Using a Small Intestine Submucosa Patch-A Rat Model.

Background: Necrotizing enterocolitis (NEC) is a life-threatening condition characterized by necrosis of the gastrointestinal tract caused by hypoperfusion and hypoxia-induced inflammation. Surgical treatment often requires resection, with high morbidity and mortality. Intestinal tissue engineering using absorbable biomaterials represents a potential alternative. Small intestinal submucosa (SIS) is a biodegradable extracellular matrix (ECM) scaffold that may facilitate regeneration of the native tissue. Objectives: The aim of our study is to investigate the regenerative potential of SIS in a r

Never Brush off Abdominal Pain: A Case of a Grill Brush Bristle Causing Colonic Perforation.

Acute abdominal pain constitutes 7% to 20% of all emergency department visits. While abdominal pain encompasses a broad differential, etiologies of enteritis can be divided into 4 major categories, with infection being the most common. We present the first case of enteritis caused by sigmoid colon and possibly jejunal perforation secondary to grill brush bristle ingestion. Despite comprehensive workup for enteritis, the cause remained elusive in our case, necessitating computed tomography enterography, which revealed the foreign body. Clinicians should maintain a high index of suspicion for fo

Retained crystal ball in pediatric gastrointestinal tract: a cautionary case report.

INTRODUCTION: Decorative crystal balls, also known as superabsorbent polymer gel beads, pose a significant hazard when ingested due to their ability to expand upon exposure to water. Diagnosing crystal ball ingestion remains challenging because of their transparency to radiation, making them difficult to visualize using X-ray imaging. Small crystal balls may not be detected in their early stages, only becoming visible once they fully swell and cause intestinal obstruction. This often results in some crystal balls remaining in the distal intestine after primary surgery aimed at removing the pro

Small Bowel Obstruction due to Hemobezoar Following Roux-En-Y Gastric Bypass: A Case Report.

Roux-en-Y Gastric Bypass (RYGB) is a widely performed bariatric surgery with well-documented benefits, but it can lead to rare complications such as small bowel obstruction (SBO). While internal hernias and adhesions are common causes, hemobezoar formation is an extremely rare etiology. We report two cases of acute SBO due to hemobezoar following laparoscopic RYGB. Both patients presented with severe nausea within the early postoperative period. CT imaging revealed marked small bowel distension in both cases, prompting surgical intervention. Diagnostic laparoscopy confirmed the presence of obs

Small but high risk: a 10-year single-hospital study on sharp gastrointestinal foreign bodies-the case of the jujube pit.

BACKGROUND: Guidelines from Europe and the United States concur that sharp foreign bodies (FBs) whose length exceeds 2.5 cm seldom traverse the pylorus and are associated with serious complications. To determine whether jujube pits conform to this rule, we investigated the clinical course of patients with jujube pit impaction, characterized the patient populations at greatest risk, and compared the diagnostic performance of barium esophagography (BE) and multidetector computed tomography (MDCT). METHODS: A single-center retrospective study was conducted on cases of jujube pit impaction treate

Accuracy of Computed Tomography in Identifying the Etiology of Small Bowel Obstructions in Virgin Abdomens at a Tertiary Care Military Treatment Facility.

IntroductionSmall bowel obstructions (SBO) account for up to 50% of emergent laparotomies. Computed tomography (CT) aids in diagnosing SBO by identifying transition points and potential etiologies, though accuracy varies. SBO in a virgin abdomen (SBO-VA) presents unique diagnostic challenges. This study assesses CT accuracy in identifying SBO-VA etiologies in patients at a tertiary military treatment facility to help guide future management strategies.MethodsThis retrospective study reviewed SBO-VA cases from 2005-2021 with IRB approval. Patients were identified via ICD codes and included if ≥

Bowel Obstruction Caused by Migration of an Intra-Gastric Balloon: A Rare Case Treated Laparoscopically.

BACKGROUND Endoscopic bariatric therapies, such as intra-gastric balloons (IGB) offer a less invasive, efficacious, and safe approach to facilitate weight loss in obese individuals. While typically tolerated, rare complications, including intestinal migration of the IGB, can occur and lead to serious sequelae such as mechanical bowel obstruction. This case report details the diagnostic process and successful laparoscopic surgical management of an unusual instance of IGB migration resulting in small bowel obstruction. CASE REPORT We present the case of a 32-year-old woman who developed a small

Preoperative septic peritonitis, hypotension, and reason for surgery are risk factors for small intestine dehiscence in dogs: a directed acyclic graph approach.

OBJECTIVE: To evaluate risk factors for postoperative intestinal dehiscence, adjusting for potential confounders identified through directed acyclic graphs (DAGs). METHODS: A retrospective cohort study was conducted of dogs undergoing small intestinal surgery at a private referral center between July 2011 and July 2024. Directed acyclic graphs were used to identify risk factors, confounders, and mediators, with regression models developed to adjust for these confounders. RESULTS: A total of 55 dogs were included in the study. Of these, 11 dogs (20%) developed postoperative intestinal dehisce
