Injury patterns from major urban terrorist bombings in trains: the Madrid experience.

BACKGROUND: Terrorist urban mass casualty incidents (MCI) in the last 3 years have targeted commuter trains at rush hour, producing large numbers of casualties. Civilian care providers are usually not familiar with the types of blast injuries sustained by victims of these MCI. METHODS: We focus on the injury patterns sustained by casualties of the Madrid, 11 March 2004, terrorist bombings, at the seven hospitals that received most victims. Data were gathered of casualties who had injuries other than superficial bruises, transient hearing loss from barotrauma without eardrum perforation, and/o

Inhalation injury, pulmonary perturbations, and fluid resuscitation.

Inhalation injury (INHI) associated with thermal injury has been shown to increase the rate of mortality. Several investigators have shown that patients with inhalation and burn injuries will require increased fluid volumes during acute resuscitation when compared with patients with burn injury alone. Other groups have examined the use of lung compliance and airway resistance as predictors of outcome in patients with INHI. We hypothesized that increased fluid requirements would more closely correlate with perturbations in pulmonary performance than with mere presence or absence of INHI or the

Reconstruction of facial burn sequelae utilizing tissue expanders with embodiment injection site: case report.

Although highly specialized burn centers have significantly reduced mortality rates following extensive total body surface area burns, survivors are often left with grotesque facial disfigurement. Hypertrophic scars and tissue defects are the most common cause of functional and aesthetic problems in the head and neck region. Plastic surgeons use full-thickness or split-thickness skin grafts, pedicled flaps, free flaps, transplantation of bone or cartilage and tissue expansion. The authors present a case of a patient who suffered from third-degree flame burns to the face. Prior skin grafting pr

[A second-degree burn related to air bag deployment: a case report].

Although air bags have been shown to reduce mortality and morbidity in motor vehicle crashes, they are associated with specific injuries. Burns due to air bag deployment account for more than 5% of all air bag injuries and typically involve upper extremities or head and neck. Aside from friction burns, thermal and chemical burns are also seen resulting from the use of certain chemicals for the deployment mechanism. We reported a female patient who presented to the emergency department with a second-degree burn in her left breast, six days after a motor vehicle crash. She was treated with irrig

Neuroprotection due to irrigation during bipolar cautery.

OBJECTIVE: To test whether irrigation during bipolar cautery confers thermoprotection from neuronal injury. DESIGN: A rat animal model (15 rats for each treatment group) was used to test the thermoprotective effects of irrigation during bipolar cautery. In this model, the sciatic nerve was exposed, and a 1-second stimulus was applied using bipolar cautery forceps at 40 or 20 W placed directly on the nerve in the presence or absence of simultaneous irrigation. The effects of cautery were determined on the basis of clinical gait analysis by means of the Sciatic Functional Index, temperature res

Pulse oximeter as a cause of skin burn during photodynamic therapy.

Photodynamic therapy is increasingly being used for the treatment of various cancers. However, this technique has a major adverse effect, namely skin photosensitization. An unusual case of skin burn associated with pulse oximetry during photodynamic therapy in a patient treated for an early esophageal tumor is described. The patient, who was treated with mTHPC, suffered a second-degree burn on the index finger with subsequent loss of the nail. The only previously reported case of such a complication occurred with Photofrin, whereby no tissue other than skin was damaged.

New technique for laryngotracheal mucosa transplantation. 'Stamp' welding using indocyanine green dye and albumin interaction with diode laser.

OBJECTIVE: To investigate (1) the possibility of survival of free mucosa "stamp" grafts fixed in the airway with a new technique using indocyanine green-dyed albumin solder activated with a diode laser and (2) the degree of improvement of wound healing in the airway by applying modified microskin transplantation techniques from burn surgery to cover a relatively large wound with a few small pieces of mucosa anchored in place with the previously mentioned technique. DESIGN: Three (one control and two experimental) rectangular (10 x 8 mm) wounds in tracheal mucosa were produced in four experime

Maxillofacial growth after neck burn injury at a young age: an experimental study in the rabbit.

An experimental model was designed to define alterations in the normal mandibular growth process under the influence of postburn neck contractures. Additionally, this craniofacial growth model was used to compare two early treatment modalities of neck burns in their capability to minimize contracture and hence allow for normal mandibular growth and development. Growth implies increase in size as well as change in shape and position. These three aspects of growth were defined accordingly to Björk as rotations. The intramatrix rotation expresses the change in mandibular shape, and the matrix rot

An approach to management of keloids.

Keloids present a major therapeutic dilemma for the surgeon because of frequent recurrences. We use a new protocol for management of large primary or recalcitrant keloids. The technique employs carbon dioxide laser resection of the keloid and then allows the open wound bed to heal by secondary intention. The open wound is treated as though it were a third-degree skin burn. The wounds invariably orient their long axis parallel to the relaxed skin tension lines. No keloid has ever been noted to recur before epithelial migration is complete. Careful follow-up detects early recurrences that are th

Burn of the auricle.

Silvadene cream and Sulfamylon make up the treatment regimen for one contributor (Dr. Crumley). No systemic antibiotics would be given. Any areas of obvious third-degree burns would be debrided and grafted. The tympanic membrane perforation would be treated with antibiotic/steroid drops. The second author concurs with use of Silvadene cream and would avoid any pressure on the area (Dr. Abemayor). While he agrees that systemic antibiotics should be avoided, he also would not prescribe ear drops. He recommends evaluation for a pulmonary or ophthalmologic injury. There is a disagreement regarding

[An assessment of patients hospitalized for burn injuries].

To study the causes of burn injuries and the nature of their medical treatment, we investigated the medical histories of hospitalized patients admitted to the burn center of Taipei City Ho-Ping Hospital from June 6, 1983 to June 6, 1987. Additionally, a phone interview follow-up on recovery conditions was made. Among the 300 patients admitted, the majority (53.7%) were burned by scalding water, while fire was the second most common cause of burns. The rate of incidence was highest for those aged 0-4 years, and 90% of this age group were victims of scalding. For all age groups, 78.9% of the bur

A Phase I-II trial testing the addition of systemic cisplatin (CDDP) to local hyperthermia and radiation was conducted to determine the dose of cisplatin that is tolerable once weekly for 6 weeks and to estimate the therapeutic potential of this trimodality combination in patients with locally advanced malignancies. Cisplatin at 20 mg/m2 (4 patients), 30 mg/m2 (8 patients), and 40 mg/m2 (12 patients) was given rapidly (over 5-10 min) i.v. after prehydration with 1 liter of normal saline. After approximately two-thirds of the cisplatin dose had been delivered, microwave hyperthermia was begun a

[Hemorrhage of the head and neck in death by burning].

The 202 deaths connected with burns in 1976-1986 included 134 in which death occurred during the fire. In 49 cases the face was so badly charred that it was not possible to judge whether hemorrhage had occurred in the eyelids and/or conjuctivae. Such blood extravasation was seen in 14 of the remaining 85 cases, taking the form of petechiae in some cases and in others the form associated with more extensive extravasation, in some cases hemolytic. No other features known to cause such hemorrhages were present, so that they are attributed to the burning process itself. Signs of extensive pronounc

Halo immobilization in the treatment of burns to the head, face, and neck.

The care of burns to the head, face, and neck remains a challenge to the clinician. From 1978 to 1986, halo traction was used as an immobilizing device and as a protective frame for 31 patients with burns to the head (n = 8), face (n = 24), and neck (n = 28). The patients ranged in age from 8 months to 80 years (mean = 16 years) and had second- and third-degree burns covering a total body surface area of 8% to 75% (mean = 28%). The halo was used for an average of eight days (range one to 19 days) and served to immobilize and protect areas of fresh skin grafts, as well as to elevate and protect
