BACKGROUND: Soleus muscle flap can be used in different modifications to reconstruct lower limb defects. It can be proximally based, distally based, island or reversed flow flap. The first description of the soleus muscle as an island flap supplied by one distal perforator was reported by Yajima et al (Plast Reconstr Surg. 1995;96:1162-1168). BACKGROUND: Diabetes mellitus is a worldwide pandemic that impacts more than 387 million people, with 29 million individuals affected in the United States alone. Diabetic patients have a 25% lifetime risk of developing a diabetic foot ulcer (DFU). Having a DFU is associated with a risk of recurrence approaching 70%. The authors presented a new technique of using a narrow distally pedicled fascio-vascular flap from the medial aspect of the lower leg to cover soft tissue defects in the lower third of the leg, ankle and foot regions. 7 cases of traumatic scar have been treated with reversed narrow pedicled fascio-vascular flap since January 1987.
